Hypothesis of Zika situation & risk assessment, based on current evidence

Zika virus could have been circulating/endemic in SEA & most other tropical countries for decades.

Zika associated microcephaly & GBS might have existed along with its circulation, although undetected, likely at low levels.

Recent increase in numbers of Zika cases & microcephaly in Thailand & other SEA countries is likely the result of increased detection rather than increased outbreaks or virus introduction from Latin America.

Whether Zika virus characters & its behavior in SEA are different from what happen in Latin America is subject to study.

Joint Statement of the ASEAN Health Ministers’ Special Video Conference on the Threat of Zika Virus in the Region ( 19 Sep 2016)

1. Strengthening in-country disease surveillance and leveraging on existing regional risk assessment mechanisms on Zika Virus Disease with the technical support of international organizations including WHO;

2. Leveraging on the International Health Regulations 2005 (IHR, 2005) and other existing mechanisms to ensure relevant and timely information sharing with other member states for accurate risk assessment;

3. Enhancing the effectiveness of regional surveillance and response on Zika Virus Disease and other emerging and re-emerging diseases through the existing regional initiative mechanisms including ASEAN-EOC network, and ASEAN Plus Three Field Epidemiology Training Network (APT-FETN);

4. Taking appropriate measures to manage risks by intensifying vector control measures, ensuring access to diagnostic testing for Zika virus within our countries, including strengthening national laboratory network and communicating such risks appropriately;

5. Conducting research and sharing of new knowledge and best practices with regards to Zika Virus Disease, through relevant ASEAN-based cooperative mechanisms including APT-FETN, SEAMEO-TROPMED and also, other existing platforms including the Global Health Security Agenda (GHSA).

แผนยทธศาสตรมทศทางและเปาประสงคหลก (Goals) ลดการแพรเชอและการปวยดวยโรคตดเชอไวรสซกา

ลดการเกดภาวะแทรกซอนจากการปวยดวยโรคตดเชอไวรสซกา ไดแก ทารกทม ความผดปกต ศรษะเลก และ ความผดปกตแตก าเนดอนๆ ทตามมาไดในภายหลง ทเกดจากมารดาทตดเชอไวรสซกา รวมถงผปวยกลมอาการกลแลง-บารเร หรอผปวยโรคทางระบบประสาทอกเสบอนๆ ภายหลงการตดเชอไวรสซกา

มระบบการดแลรกษาและตดตามผเกดภาวะแทรกซอนจากการปวยดวยโรคตดเชอไวรสซกาตอเนองในระยะยาวแบบองครวมทมประสทธภาพ

1. อตราการตดเชอไวรสซกาในหญงตงครรภลดลงจากปกอนอยางนอย รอยละ 50 ทกป

2. อตราการเกดภาวะแทรกซอน ไดแก ทารกทมความศรษะเลก หรอ ผดปกตแตก าเนดลดลง และ

เปนศนย ภายในป พ.ศ. 2562

3. อยางนอยรอยละ 80 ของผปวย GBS ไดรบการตรวจวนจฉยเพอตรวจหาเชอไวรสซกา

4. อยางนอยรอยละ 90 ของผเกดภาวะแทรกซอนจากการปวยดวยโรคตดเชอไวรสซกา ไดรบการ

Materno–fetal transmission of Zika virus and link between the virus and severe congenital CNS malformations confirmed

What is the percentage of transmission from infected pregnant women to fetuses? What is the percentage of infected fetuses that will develop malformations? What are the long-term outcomes for infected neonates without detectable abnormalities at birth? Can markers of fetal infection or predictive markers of congenital syndrome be identified in peripheral maternal blood?

Immunity against Zika virus

Does Zika virus infection confer lifelong immunity? What is the effect of previous flavivirus infection on severity of Zika virus infections? How much background immunity is required to prevent emergence of new Zika virus strains? What is the effect of previous vaccination against a heterologous flavivirus (eg, yellow fever virus or Japanese encephalitis virus)? WHO has prioritized as the most viable R&D options to help fight the spread of Zika virus in the immediate future:

Multiplex tests for ‘flaviviruses’ (viruses related to Zika,

such as dengue, chikungunya), in addition to more traditional

tests;

Protective vaccines to prevent congenital Zika syndrome; and

Innovative vector control tools that reduce the mosquito

population.

Zika vaccine research ZIKV is a flavivirus, the effect of pre-existing antibody to dengue and other

flaviviruses may affect the safety and immunogenicity of ZIKV vaccines.

In vitro studies have demonstrated both neutralization of ZIKV as well as the

enhancement of ZIKV titer by dengue antibody.

Safety and effectiveness of candidate ZIKV may differ in populations with

differing dengue endemicity

Currently 45 ZIKV candidate vaccines in pre-clinical development; 5 enters

phase 1 clinical trial

WHO has identified women of child-bearing age / pregnant women as the primary target during outbreak, then most appropriated vaccine platforms include PIV ZIKV vaccines, sub-unit protein vaccines, and VLPs.

Live attenuated vaccines/ chimeric viruses could still be used in an outbreak but may be used more generally in long-term control strategies that would target children.

ZIKV vaccine development had a quick start, further development may be hampered because of the inability to conduct large efficacy trials with the decline in cases and unpredictability of new outbreaks
