1. SCOPE AND PURPOSE
1. This QMP specifies the service provision by the Poppy team in offering intrapartum
care as part of a continuity of carer pathway through pregnancy, labour and in the
postnatal period for those women eligible for care by this team of midwives
2. This services aims to enhance care to women with complex social needs in the
intrapartum period through a modified team approach. The named midwife will
continue to provide continuity in the antenatal and postnatal period to individual
women but the team will have a collective responsibility to provide intrapartum care
to a defined cohort of women
3. The team aims to care for 5 women per month in the intrapartum period in all care
settings representing approximately 40% of their total caseload
4. This enhancement by the team will contribute towards the 20% target for women
accessing a continuity of care model within this service (Refreshing NHS plans
2018/19).
Document control
Document control is an essential part of a quality management system. 1 A robust system for the regular review and updating of all guidelines, protocols,
policies and procedures has been agreed by the Maternity Clinical Governance Committee and is monitored through this forum.
2. AUTHORISED STAFF

3. PROCEDURE
3.1 Intrapartum care will be offered by midwives from the Poppy team participating in
a shared on call overnight rota 3-4 times per week (each midwife carries out one on
call per week). During the day intrapartum care will be provided by the woman’s
named Poppy midwife or another member of the Poppy team, where possible. This
service will be between Monday to Friday over the 24 hour period where
overnight on call is available. At the weekend, one midwife is on duty for
essential antenatal and postnatal care during the day. Intrapartum care during this

period will be at the discretion of the Poppy midwife on duty. The off duty will be
available on Optimize.
3.2 Any woman who fits the criteria for Poppy team care (GL889) and booked for RBFT
intrapartum services can be offered continuity of intrapartum care with the exception
of:
Those women who do not engage with getting to know the team of midwives
3.3 Each midwife will discuss continuity of carer in the intrapartum period including how
to get to know the team. The midwives will document women’s choices for labour
care. Every eligible woman will be given information about the service and midwife
availability (see Appendix 1). Team communication will take place each month
identifying the women eligible for intrapartum care by them; this information will be
recorded on the team’s caseload sheet
3.4 Women will be encouraged to ‘get to know’ the team through attendance at
antenatal clinics, parent information classes and coffee mornings
3.5 When in labour, women will call the labour triage telephone line (unless booked for
home birth); the triage midwife will advise the woman as necessary and arrange for
early labour assessment. A ‘Poppy team folder’ will be maintained in the triage area
identifying those women who are to receive intrapartum care by the on call Poppy
team midwife. This information will also be placed in the woman’s maternity records
and also in her handheld records. The on call Poppy team midwife will be notified
via the assessment midwife of established labour and requirement to attend the
woman.
3.6 The service will be monitored via the continuity metrics agreed by the maternity
service and LMS. The data will be collected via CMIS. A service evaluation will be
carried out after 6 months from implementation.
