No hay peligro vital u orgánico pero si alto riesgo

•Hemograma, química sérica, electrolitos séricos, examen de orina•Radiografía de Tórax•Electrocardiograma•Ecocardiograma•Estudio de imagen cerebral
• <25 % (minutos a 1 hora)•L uego 160/100–110 mmHg en las próximas 2–6 h.•Reducción gradual a PA normal en 24 – 48 h.

Nitroglicerina (50mg/5cc)
5-10mcg/min infusión EV
diluido en dextrosa
2-5 min 5-10 min Cefalea, vómitos, metahemoglobinemia
Enalaprilato(2,5mg/2cc)
1,25-5mg c/6 h EV diluido,
15-30 min 6-12 horas Respuesta variable
Otras: fenoldopam, nicardipina, hidralazina
BETABLOQUEANTES

TRATAMIENTO RECOMENDADO SEGÚN CRISIS HIPERTENSIVA
CRISIS TRATAMIENTO
Edema agudo de pulmón Nitroprusiato + diuretico
Sindrome coronario agudo Labetalol/esmolol + nitroglicerina
Encefalopatia hipertensiva Labetalol /nicardipina
Disección aguda de la aorta Esmolol + nitroprusiato
Eclampsia Hidralazina
Falla renal aguda Nicardipina o verapamilo

TRATAMIENTO DE LA URGENCIA HIPERTENSIVA
DROGA DOSIS TIEMPO DURACIÓN EFECTOS ADVERSOS
Captopril 25 mg S L c/30 min hasta 3 dosis
30 min-1hora 6 horas Proteinuria, poliuria, oliguria, rash,
prurito
Clonidina(0,150mg)
0,1mg c/20min VO
0,2mcg/kg/min en infusión EV
30min-2horas 6-8 horas S omnolencia, sequedad de boca, cefalea, nauseas,
fatiga, ortostatismo.
