FREE REGISTRATION FOR THE SPECIAL ADMINISTRATION OF2o19 PHILIPPINE EDUCATIONAL PLACEMENT TEST (PEPT)

1. This Office informs the field of the free registration for the forthcoming special administration of the 2019 Philippine Educational Placement Test (PEPT). The Special PEPT shall be administered on June 9, 2019, from 7:30 AM onwards in Bayombong South Elementary School, Bayombong, Nueva Vizcaya.

2. Based on DepEd Order No. 55, s. 2016 entitled Policy Guidelines on the National Assessment of Student Learning for the K to 12 Basic Education Program, the following may register for the PEPT:

./ Learners from schools without government permit,

./ Learners from nonformal and informal education programs;

./ Learners who have incomplete or no record of formal schooling;

./ Learners with back subjects;

./ Learners who need grade level standards assessment; and

./ Learners who are overage for their grade levels.

3. Applicants-registrants shall accomplish the Special PEPT Registration Form. Attach the following documentary requirements:

a. School Form 10 (Form 137) for secondary students; b . School Form 9 (Form 138) for elementary pupils; c. Birth Certificate or Baptismal Certificate d. 2 pes 1 x 1 pictures

4. Deadline of submission of accomplished forms with the supporting documentary requirements shall be on May 27, 2019 to the SGOD - SMME (Sir Noli Mar or Sir Julius).

5. The Division Testing Personnel shall be granted with compensatory time off(CTO) per CSC and DBM joint circular No 2, s. 2004 while the teacher-examiners shalt be given one-day service credit as provided in the DepEd Order No. 53, s. 2003 entitled Updated Guidelines on Grant of Vacation Service Credits to Teachers.

6. Immediate dissemination of this Memorandum is desired.

1. Before signing this form, please see to it that all entries especially those on Date of Birth. Age and Grade Level Finished are legible and correct.

2. Detach Applicant's Copy and give it to the applicant. 3. Keep the LEM's Copy and give it to the Chief Examiner on examination

day for applicant verification purposes. 4. Verify through LIS If the school where the learner comes from has a

government permit . 5. For learners from private schools without government permit,

place the endorsement letter from the Regional Office inside each ETRE.
