X CountyID: XThis is the screen shot of your community level snapshot. 9
X CountyID: XIn the middle of the page you will see a bar chart. This bar chart shows how your community scored in each of the seven capacity areas. These scores represent an average score based on the responses of all the leadership team members within your community who have taken the tool. 10
X CountyID: XIn the top left hand corner of the page, is your overall score. This score is the combined average of the seven sections. This does not represent a grade so to speak, but rather is an overall measure of leadership team capacity. 11
X CountyID: XJust under the overall score on the top left hand side of your snapshot, is the response rate. We sent the LTAT out to all members of your leadership team (i.e. the list of individuals you indicated were members of your leadership team). Leadership teams ranged in size from 3 to 37. We calculated the response rate by dividing the number of people who completed the tool by the number of people within your community the tool was sent to.
We thought that there was value in having multiple people fill out this tool. It provided an opportunity to see a range of perspectives.12
X CountyID: XNow Id like to draw your attention to the table located at the bottom of the page. This table provides more detail on each of the seven sections of the tool. Column two illustrates the number of questions in each section of the tool.13
X CountyID: XThe next column shows us the total range of possible points.14
X CountyID: XThe next column shows us the range of scores for your community. The lowest score given to the highest score given. 15
If you turn over the snapshot, theres a list of discussion questions on the back. Within your community teams, please spend the next 10 minutes, going through these questions. Please keep in mind, the results from this assessment are intended to provide your leadership team with additional insights about the overall capacity and functioning or your teams. Its our hope that you are able to use this information to strengthen your teams and strategically position your communities as you move forward in implementing your HEAL priorities.1617Veena Pankajvpankaj@innonet.orgThank You!
