What we are looking to do in the future

1) Develop a pricing system- A pricing system that pays for outcomes

and rewards keeping people well in their homes (the high risk patients)

2) Focus on prevention

- Link to social determinants of health & provide health improvements in other sectors (welfare, housing etc)

3) Information revolution - Access to all medical records in one place

which different providers and clinicians can access

- Provide patients with choices on where they can get access to the best care
