Testing/Assessments/Psychological Evaluations (if applicable)We would like to understand your child’s academic needs so that we can guide him/her in the educational path. If your childhas been assessed by an educational psychologist, an occupational therapist or a speech therapist, please provide the schoola copy of the evaluation.

Student Introduction Essay (must be handwritten)Lower School (Grade 3-5) students – Tell us about yourself, your family & friends in 80 to 100 words.Upper School (Grade 6-10) students – Tell us about yourself, your aspirations and goals in 150 to 200 words.

Application FeeA non-refundable application fee of RM1,075 per applicant. Payment may be in Cash, Cheque, Banker’s Draft or bycredit card in Ringgit Malaysia (RM), or via Telegraphic Transfer payable to PHOENIX TREE INTERNATIONAL SDN BHD
