    Maddux Elementary School, which is not on the National Register of Historic Buildings, is a 1966 2-story, 57,930 square foot brick school building located in a suburban residentsetting. The existing facility does not use modular buildings. The structure of the original building is a steel frame and nonbearing masonry wall construction except for theGymnasium, which is brick veneer on masonry bearing wall. The floor system consists primarily of concrete slab on grade with precast concrete used over the basement area.The roof structure is metal deck and steel joists except for the Gymnasium, which has tectum deck. The structure of the 1994 addition contains bearing masonry exterior andinterior wall construction. The floor system consists of concrete slab on grade. The roof structure is metal deck and steel joists. The roofing system over the original building is

    EPDM installed in phases over the past 17 years. The roofing system of the addition is built-up with gravel ballast installed in 1994. The ventilation system of the building isinadequate to meet the needs of the users. The classrooms are slightly smaller than the current standards established by the State of Ohio. Physical Education and StudentDining spaces consist of one Gymnasium and separate Student Dining. The electrical system for the facility is inadequate. The facility is equipped with a security system, but itlacks some of the OSFC standard features. The building has a non-compliant fire alarm system.     Facility Assessment

    A. Heating System

    Description: The existing system for the overall facility is a gas fired hot water system, installed in 1966, and is in fair condition. The two hot water boilers,manufactured by International, were installed in 1966 (1 rebuilt 4 years ago) and are in fair condition. Heating water is distributed to terminal unitsconsisting of unit ventilators, cabinet heaters, and air handlers. The terminal equipment was installed in 1966 and is in fair condition. The 1993addition is an extension of the original hot water unit ventilator system. The system does not comply with the 15 CFM per person fresh airrequirements of the Ohio Building Code mechanical code and Ohio School Design Manual. The pneumatic system temperature controls wereinstalled in 1966 and are in fair condition. The system does not feature individual temperature controls in all spaces required by the OSDM.Louvered interior doors are a violation of the Ohio Building Code, and interior doors in this facility are louvered to facilitate corridor utilization asreturn air plenums. Louvered doors are replaced in Items J and O. The existing system is not ducted. The overall heating system is evaluated asnot being in safe and efficient working order, and long-term life expectancy of the existing system is not anticipated. The structure is not equippedwith air conditioning. The site does not contain underground fuel tanks.

    Recommendations: Provide new overall heating, ventilating, and air conditioning system to achieve compliance with Ohio Building Code and Ohio School DesignManual standards. Provide for a ducted system.

    B. Roofing

    Description: The roof over the original construction is an EPDM system that was installed in three phases (1987, 1992, 1997) and is in fair condition. Thereare District reports of current leaking in the corner of the gymnasium. Signs of past leaking were observed during the physical assessment. Theroof over the 1993 addition is a built-up system with gravel ballast that was installed in 1993, and is in fair condition. There are no District reportsof current leaking. No signs of past leaking were observed during the physical assessment. Access to the roof was gained by ladder. There wereno observations of standing water on the roof. Roof drains are adequate in number, properly located, and in fair condition.

    Recommendations: The roof over the original building requires replacement to meet Ohio School Design Manual guidelines for age of system.

    C. Ventilation / Air Conditioning

    Description: The overall facility is not equipped with an air conditioning system. Multiple rooftop condensing units serve individual areas such as the computerlab, art, music, and office areas. Units vary in age and condition from good to poor. Window units are provided in the library. The ventilationsystem in the overall facility consists of unit ventilators, installed in 1966 / 1994 and in fair condition, providing fresh air to classrooms, and airhandlers, installed in 1966 and in fair condition, providing fresh air to other miscellaneous spaces such as the gymnasium. Interior doors arelouvered to facilitate corridor utilization as return air plenums. The ventilation system does not meet the Ohio Building Code 15 CFM per occupantfresh air requirement. The overall system is not compliant with Ohio Building Code and Ohio School Design Manual requirements. Dust collectionsystems are not required in this facility. Exhaust systems for restrooms, kitchen, and custodial closets are adequate.

    D. Electrical Systems

    Description: The electrical system provided to the overall facility is a 1600 amp, 120/208 volt, 3 phase, 4 wire system installed in 1966, and is in fair condition.The system in the 1994 addition is an extension of the original construction. Power is provided to the school by a utility owned, pad-mountedtransformer located near the mechanical room, and in fair condition. The panel system, installed in 1966 / 1994, is in fair condition, with limitedcapacity for expansion. The classrooms are not equipped with adequate electrical outlets. The typical classroom contains 4 general purposeoutlets and no dedicated outlet for each classroom television, as well as no dedicated outlets for student and teacher computers. All the spaceshave electrical outlets. The corridors are not equipped with adequate electrical outlets for servicing. There are adequate GFI protected exterioroutlets located around the perimeter of the building. The overall electrical system does not meet Ohio School Design Manual requirements insupporting the current needs of the school, and will be inadequate to meet the facilitys future needs.

    Rating: 3 Needs Replacement

    Recommendations: The entire electrical system in the original building requires replacement to meet Ohio School Design Manual guidelines for classroom capacity,due to condition and age, and to accommodate the addition of an air conditioning system. Provide additional panels (circuits / outlets) to increasecapacity for the 1994 addition.

    E. Plumbing and Fixtures

    Description: The service entrance is equipped with a reduced pressure backflow preventer in fair condition. A water treatment system is provided for thedishwasher only and is in fair condition. The domestic water supply piping in the overall facility is copper in fair to poor condition. The waste pipingin the overall facility is cast iron in fair condition. The facility is equipped with four 200 gallon hot water storage tanks that are filled by the boiler.The storage tanks are in fair to poor condition. Hot water is not available when the boilers are not operating. The school contains two restroomsfor boys, two restrooms for girls, and f ive restrooms for staff. Boys restrooms contain 1 ADA and 2 non-ADA wall mounted flush valve toilets, noADA and 7 non-ADA wall mounted flush valve urinals, and 5 ADA and 1 non-ADA wall mounted lavatories. Girls restrooms contain 1 ADA and 6non-ADA wall mounted flush valve toilets, as well as no ADA and 6 non-ADA wall mounted lavatories. Staff restrooms contain 2 ADA and 3non-ADA wall mounted flush valve toilets, no urinals, and 2 ADA and 3 non-ADA wall mounted lavatories. There are 14 individual restroomslocated in the classrooms. Each individual restroom contains a non-ADA wall mounted flush valve toilet. Classrooms are equipped with a sinkwith a bubbler. The facility is equipped with 4 ADA and no non-ADA electric water coolers, in good condition. The school meets the OBCrequirements for fixtures. ADA requirements are not met for fixtures. Drinking fountains are not adequately provided. Adequate exterior wallhydrants are provided. Project laboratories for sixth grade classes are not equipped with the required gas, compressed air, or safety shower/eyewash.

    Recommendations: Replace water supply piping in the original facility with new copper piping.     Description: The classroom portion of the original building is equipped with steel columns and beams that support the roof structure. The original facility has abrick veneer on a masonry bearing wall system (gym), steel frame with brick veneer over nonbearing masonry wall (classroom), which displayedlocations of deterioration, and is in fair condition. The 1994 addition has a brick veneer on a masonry bearing wall system, which displayed nolocations of deterioration, and is in good condition. The exterior masonry appears to have appropriately spaced and adequately caulked controljoints in fair condition. The exterior masonry has not been cleaned and sealed in recent years, and shows evidence of mortar deterioration innumerous locations. Interior walls are primarily concrete masonry units, and are in fair condition. Some minor cracking was noted on classroomwing interior walls. Separation of playground areasfrom vehicular traffic is moderate. The playground equipment is in fair condition, and on a combination of hard and soft surfaces. The athleticfacilities are comprised of a baseball field and soccer nets, and are in fair condition.

    The facility is equipped with a heating system only. The heating system is in fair condition. Cooling and humidity control are provided in media center, computer lab,art, music and office areas.

    The existing unit ventilator system does not provide the code required amount of fresh air.

    The O.B.C. requires one drinking fountain per 100 students. The building capacity requires 6 drinking fountains. The existing facility has 4 drinking fountains. ADArequires that 50 percent of drinking fountains be accessible. Four of the drinking fountains are accessible. The O.B.C. requires that restrooms be located on everyfloor and that the maximum travel distance shall not exceed 500 feet. The facility meets the requirement. The size of restrooms is such that ADA access isachievable.

    6.11 Communication among students is enhanced by commons area(s) for socialization 10 4
