OUR FUTUREThe future for South West Healthcare is an exciting one as our outlook is supported by continued growth and development. The advent of our very first tertiary level clinical service (radiotherapy), scheduled to commence in August 2016, will herald a new chapter for our health service. As this is a truly regional service, it is expected it will pave the way for further expansion and support for our surrounding health services.

We will continue to engage with government in an attempt to secure funding for both our Camperdown campus and Warrnambool Stage 2 capital redevelopment projects. We are committed to improving the infrastructure at all of our sites to deliver quality healthcare utilising modern and technologically advanced facilities and equipment.

In recognition of our expanding regional role and in conjunction with our substantial clinical, teaching and research firepower we believe we are well placed to continue on the significant growth trajectory that has become commonplace during the past decade.

It is amazing that our health service continues on this spectacular growth trajectory and for the eleventh consecutive year has treated more patients than the previous reporting period. In conjunction with this level of patient throughput, we have enjoyed much success with the achievement of the majority of initiatives identified in our five year Statement of Strategic Direction.

The highlight has been, of course, the completion of the $30M South West Regional Cancer Centre (SWRCC). This exciting project will provide the very first tertiary level clinical service (radiotherapy) to South West Victoria. Furthermore, the new centre provides a chemotherapy service, specialist consulting rooms and a wellness centre. The South West Regional Cancer Centre has been developed in partnership with Epworth HealthCare and local advocacy and fundraising group Peter’s Project and will consolidate the full spectrum of cancer services in the one complex.

PARTNERING WITH OUR COMMUNITYOne of our health service’s major strategic directions is to develop strong and enduring relationships with our communities and partner organisations. In addition to a number of mature alliances, this year we have undertaken a community wide leadership role in the promotion of non-violence against women and have become a White Ribbon ambassador. This has involved the implementation of a workplace education program for all staff and the provision of prominent street-front signage to raise community awareness.

Another example of our leadership role has been in the battle to combat rising levels of obesity in our community. Along with the majority of health services in the Barwon South West region, we have made the decision to remove the sale of sugar-sweetened drinks throughout our health service in order to encourage our community to consider healthier options.

A new alliance has also been formed with Portland District Health and Western District Health Service in our bid to provide a sustainable specialist medical workforce to the extended region. Due to the scarcity of these specialist resources it has become obvious that our regional health services working in partnership are able to support one another using a joint appointment process.

Our embracing of new ideas, opportunities and collaborations also led to the four Finance Teams of South West Healthcare, Western District Health Services, Portland District Health and Colac Area Health being named 2015 National Finance Team of the Year at the 2015 Australian Health Services Financial Management Association national conference. Working together led to the successful establishment of South West Supplies and Logistics which has consolidated the participating health services’ supply and logistics operations. A central supply hub established in Warrnambool led to an incident-free transition. Further efficiencies are being realised through collaboration with South West Linen Services.

DRIVING A QUALITY AND SAFETY CULTUREIt is vitally important that the organisation continues to deliver high quality health and wellbeing services and pursues an organisational culture that is safe and risk aware. This pursuit remains a challenge as our growth continues on a path that has continually increased for over a decade. This year we are very pleased to report the treatment of a record number of 18,526 inpatients and 26,732 Emergency Department presentations. This is the eleventh successive year that South West Healthcare has treated a record numbers of patients. This highlights our ability to respond to an increasing and ageing population and our increasing regional responsibilities.

The major quality initiative undertaken during the year was the implementation of a subacute redesign project. This was

a whole-of-organisation model of care change process which was developed over a two year period. This redesign process has transformed the patient journey by providing a single point of access and assessment leading to a coordinated treatment pathway, both within the hospital and in the community. The connection of our inpatient units with our community nursing, allied health and rehabilitation services aims to deliver ‘best care, right place, right time’. This complex multi redesign project is considered a leading model in health services in Victoria.

DELIVERING EFFICIENT SERVICES AND INFRASTRUCTUREHealth services are funded using a formula that recognises the number of patients treated each year. Despite the shortage of a number of key surgical specialists during 2015-16, we were still able to approximate the throughput target negotiated with the Department of Health and Human Services. More importantly, we are delighted to point out that we have completed the year with an operating surplus of $598K within a total operating budget of $149.6M.

Following the completion of large-scale capital redevelopment projects in recent years, we continue to improve the quality of our infrastructure across all of our campuses. In addition to the completion of the $30M South West Regional Cancer Centre, other projects completed during the year include the $2.3M Emergency Observation Unit/Radiology Department redevelopment, the commencement of works on the $338K Macarthur redevelopment project, the letting of tenders for the $4.6M Prevention and Recovery Centre (PARC) and the completion of the capital and service review for the

$30M redevelopment of our Camperdown campus. In addition, we continue our advocacy to complete the master plan of our Warrnambool Base Hospital campus which involves the $112M Stage 2 redevelopment project.

We have also been extremely active in the Information and Communications Technology (ICT) space where a number of major projects have been progressed. The largest of these projects is the accelerated adoption of the Electronic Health Record which will replace our paper-based medical record. The implementation of a fully integrated Electronic Health Record has been a strategic priority for nearly a decade and has not been achieved in many health services throughout the world (there are a number of examples in individual departments such as cardiac laboratories and intensive care units but very few examples of electronic integration across an entire health service). The change management associated with implementing this two year project is one of the most difficult and complex assignments we have ever undertaken. As a precursor to the implementation of this project, new Point of Care Terminals have been ordered which will be installed at the bedside of all patient care areas.

We have also developed a process to implement a new electronic Human Resources Information System known as KRONOS. This system provides an electronic time and attendance/rostering/payroll interface which will deliver significant financial and efficiency benefits to the organisation.

The other major ICT project we have progressed during the year is the implementation of a new electronic Learning Management System known as Grow. It is anticipated that this system will provide online mandatory training updates to all our staff and become the central portal for all education requirements.

In pursuing efficiencies, the health service also undertook a number of major tender processes which led to the introduction of a non-emergency patient transport provider (National Patient Transport) and a new pathology provider (Dorevitch Pathology).

DEVELOPING A HIGH PERFORMING WORKFORCEIt goes without saying that healthcare is a labour-intensive industry and, as a result, our workforce is our most important asset. We are obviously keen to strengthen the existing culture that attracts, supports and retains high-calibre people and this year we launched a New Values Statement for all our staff. It is hoped this framework will provide guidance and direction and clearly identify a workplace culture that ensures South West Healthcare remains a great place to work. In conjunction with this new framework, our health service strongly supports the annual Staff Satisfaction Survey (People Matter) and it is very pleasing to report that this year a record 657 individual staff members contributed to the survey. Once the results are available they will be forwarded to all managers to improve processes and drive the continuous improvement culture within our organisation.

ENCOURAGING SERVICE INNOVATIONOur major innovations this year have involved getting specialised services to patients as close to home as possible. In responding to the vast geography of our region we are proud to highlight an innovative service delivery model with the introduction of our South West Healthcare Dental Van. This fully-equipped mobile clinic is now providing dental services to towns that have never previously had the benefit of a public dental service. It visits Casterton Memorial Hospital, Aboriginal Health Services in Dhauwurd Wurrung Elderly and Portland & District Community Health Centre, Winda-Mara Heywood and Balmoral Bush Nursing Centre. In a similar vein, we have also adopted the use of Telehealth as an appropriate means to overcome the tyranny of distance. One of the most outstanding examples of this, this year, is our Counselling and Support Services having utilised videoconferencing technology to enable a family member in America to participate in a Rehabilitation Unit family meeting at our Warrnambool Base Hospital. This greatly assisted in ensuring the patient’s family was informed about his current care needs, future goals and planning, and provided the opportunity for the family to meet all the clinicians involved.

We have been encouraged by the Department of Health and Human Services to increase our leadership role within the region and we will continue to support other health services and other communities in order to deliver on our mission of providing a comprehensive range of high quality health and wellbeing services for the people who live in South West Victoria.

PATIENT EXPERIENCE AND OUTCOMES

Drive improved health outcomes through a strong focus on patient-centred care in the planning, delivery and evaluation of services, and the development of new models for putting patients first.

Be actively involved in developing and strengthening partnerships with consumers and in particular partnering with consumers in the following redesign processes:

− Implementation of the sub-acute redesign patient flow process.

− Emergency Department patient flow.

Achieved.

Consumer representatives have been actively involved in the patient flow and sub-acute redesign working parties. There is also considerable consumer engagement in developing services in the new South West Regional Cancer Centre (SWRCC) to be opened in July 2016.

− Model of Care for the South West Regional Cancer Service (SWRCC).

The integrated Model of Care has been developed with consultant, chemotherapy and radiotherapy services all delivered from the SWRCC.

Strengthen the response of health services to family violence. This includes implementing interventions, processes and systems to prevent, identify and respond appropriately to family violence at an individual and community level.

Develop and implement a policy and procedure, supported by education that increases staff awareness and responsiveness to family violence both as an employee and community member.

Achieved.

A Family Violence Policy is in place at South West Healthcare (SWH) and a revision titled ‘Dealing with Family Violence’ has been drafted.

Our South West Centre Against Sexual Assault Manager is a member of the Women’s Integrated Family Violence Committee. This supports an integrated multi-agency approach to preventing and dealing with family violence issues.

Senior SWH personnel participated in the Strengthening Hospital Responses to Family Violence Seminar held in May and will use the knowledge gained when developing local strategies.

Local priorities will be determined as part of the formation of the local strategy. It is likely the Emergency Department and Midwifery Services will be the initial focus in relation to community family violence identification and management.

Use consumer feedback and develop participation processes to improve person and family-centred care, health service practice and patient experiences.

South West Healthcare (SWH) will continue to build on our ‘partnering with consumers’ strategy to identify processes to improve person and family centred care. This will be measured by increased involvement by consumers in various hospital committees, workshops and focus groups. The use of patient stories and real-time consumer feedback to drive improvements.

SWH has reviewed the current committee structure in relation to the Community Advisory and the Partnering with Consumers Committees. As a result, the two have been amalgamated and a Board of Directors representative added to the membership.

Consumers have been heavily involved in the development of the new SWRCC Model of Care.

A SWRCC community open day was held in July 2016.

Support the effective delivery of alcohol and other drug treatment services.

Implement, monitor and develop strategies to provide a safe work environment and assist those patients/clients presenting to our Emergency Department under the influence of alcohol and other drugs.

The above will be achieved through implementing safety measures for patients and staff, education and training and targeted referral processes for intervention and other support initiatives.

A 0.5 EFT project worker to support drug and alcohol services has been appointed. This position will work closely with Emergency Department staff to progress improvement strategies. An implementation plan has been developed and adopted.

Improve the health outcomes of Aboriginal and Torres Strait Islanders by increasing accessibility and cultural responsiveness of the Victorian health system.

Implement the SWH Aboriginal Employment Strategy.

The release of the Department of Health and Human Services (DHHS) first report regarding patients over 75 years of age with an advance care plan or substitute decision maker showed SWH was well-placed at a state level with a result of 25%.

GOVERNANCE, LEADERSHIP AND CULTURE

Demonstrate an organisational commitment to Occupational Health and Safety, including mental health and wellbeing in the workplace. Ensure accessible and affordable support services are available for employees experiencing mental ill health. Work collaboratively with the Department of Health and Human Services and professional bodies to identify and address systemic issues of mental ill health amongst the medical professions.

Commence every Board of Directors meeting with a report on any Lost Time Incidents.

All Board meetings commenced with Lost Time Injury Frequency Rates and discussion on any incidents. Lost Time Injury Frequency Rates has remained on target at 10.

Update and implement organisation-wide Safety Plan.

South West Healthcare has been caring for the health and wellbeing of South West Victorians for more than one-and-a-half centuries. This year, our Warrnambool hospital turned 162 years old and our Camperdown Hospital turned 107.

Consisting of two public hospitals, a mental health services division, an aged care facility and five community health centres, in 2015-16 we provided 149 medical, nursing, mental health, allied health and community health services to the 110,000 people who live in Warrnambool, Moyne, Corangamite, Southern Grampians and Glenelg.

OUR SERVICESOf the 149 medical, nursing, mental health, allied health and community health services we provided in 2015–16 (see Services and Programs), the newest included the establishment of our region’s first Hemofiltration Service.

Made possible by 30 Intensive Care Unit staff being trained and educated in the practice of this lifesaving treatment, following the arrival of a $62,000 hemofilter machine donated by Midfield Meat International, 10 local people were spared from having to be transferred to Melbourne with acute kidney failure during 2015–16. They ranged in age from 28–85.

Not only has the establishment of this new medical service decreased transfers to metropolitan facilities and allowed patients to remain with their families, it has ended treatment delays and decreased lengths-of-stay. In the future it may even allow us to provide therapeutic plasma exchange to treat a variety of diseases such as acute Guillain-Barre Syndrome and Myasthenia gravis.

We also established:

> A specialist Allied Health Service at our Camperdown Hospital. Until January, Allied Health clinicians from our Warrnambool Base Hospital visited our Camperdown Hospital inpatients on a needs basis.Our onsite team is a group of young and vibrant professionals, including an occupational therapist, dietitian, speech therapist, physiotherapist and counsellor.

This initiative also provides discharged patients with solid hospital-to-home support. With a large number of people now accessing our Camperdown Hospital (1,522 inpatients in 2015–16) the aim is to help them get well enough to get home, and keep them independent for as long as possible.

> A Warrnambool Mental Health Services Therapeutic Day Program for consumers/carers in the community and consumers who are inpatients. Based at Hider House, the innovative program consists primarily of non-clinical activities including arts and craft afternoons; music making; singing; and exercise, social and activity groups.Hider House includes a designated sensory room, a carer resource room and the beginnings of a music room. Other services are welcome to participate in Hider House activities. There’s space, also, for additional consumer/carer-based groups to meet.

– a 3.28 percent increase on 2015–14’s 16,475 (540 more) > Our Camperdown Hospital cared for 1,511 inpatients

– a 9.79 percent decrease on 2014–15’s 1,675 (164 less).

WHERE THEY CAME FROMThe majority of our hospital inpatients, not surprisingly, came from the Local Government Area in which the hospital they attended is located:

> 59.18 percent (10,069) of our Warrnambool Base Hospital inpatients were Warrnambool City residents

> 88.88 percent (1,343) of our Camperdown Hospital inpatients were Corangamite Shire residents.

24,325 patients – a 0.31 percent decrease on 2014–15’s 24,401 (76 less)

> Our Camperdown Urgent Care Centre treated 2,407 patients

THE AGE OF OUR 18,526 INPATIENTS > The 0–5 age group was the highest rating inpatient group

at our Warrnambool Base Hospital, accounting for 9.19 percent. The 66–70 age group was the second highest at 9.16 percent, followed by the 61–65 age group at 9.15 percent. (In 2014–15 the 71–75 age group was the highest rating at 10.82, followed by the 61–65 age group at 9.71 and the 0–50 age group at 8.59.)

> The 76–80 age group was the highest rating inpatient group at our Camperdown Hospital, accounting for 10.13 percent. The 71–75 age group was the second highest at 9.86 percent, followed by the 61–65 age group at 9.70. (In 2014–15 the 76–80 age group was the highest rating at 10.69, followed by the 71–75 age group at 9.61 and the 81–85 age group at 8.96.)

It is worth noting the 0–5 inpatient figures at both hospitals (8.59 percent at Warrnambool and 4.00 percent at Camperdown) include Midwifery Unit births while our Camperdown Hospital figures do not include our aged care Merindah Lodge residents.

Peter has headed up our Medical Services for the past 20 years. Before commencing a predominantly medical management role he was a procedural (anaesthetics and obstetrics) general practitioner in rural South Australia and a medical officer in the Royal Flying Doctor Service. He is Clinical Associate Professor of the Deakin Clinical School and a member of the Deakin University School of Medicine Advisory Board.

Andrew has worked in the Victorian public health sector for 30 years, joining South West Healthcare in 2005. Originally from Ballarat, with subsequent appointments at Kilmore and Djerriwarrh Health Services (including Bacchus Marsh and Melton Regional Hospital), he has extensive experience, understanding and commitment to the rural and regional health sector.

Caroline commenced her career as a psychiatric nurse in the 1970s. She worked in a range of mental health and substance use services in various settings before joining our Executive Team in 2004. Her Mental Health Division is committed to assisting consumers in their recovery journey and their quality of life whilst experiencing serious mental illness. Caroline is a Department of Health Victorian Travelling Fellowship recipient and recognised in the Who’s Who of Australian Women.

JAMIEBRENNANDirectorofServiceDevelopmentBHealth Science (Physiotherapy), Cert Healthcare Innovation and Entrepreneurship, AFACHSMJamie became our first Director of Service Development in 2014. He has more than a decade of experience in a diverse range of senior project management, support services and medical services roles at large regional health services. His role includes responsibility for the operation of non-clinical support services, our Camperdown Hospital and Merindah Lodge. Jamie is committed to person-centred care, innovation, continuous improvement and the development of commercial opportunities to ensure the sustainability of health service delivery within the public health setting.

He remains committed to improving services to clients and patients and ensuring seamless care that improves the health of individuals and their communities.

HUMAN RESEARCH ETHICS COMMITTEEThis committee ensures that human research undertaken by, or in partnership with, South West Healthcare is designed and conducted in accordance with the Australian code for the conduct of research, and is ethically reviewed and monitored in accordance with the NHMRC National Statement on Ethical Conduct in Human Research. This committee met four times in 2015–16.

CONSUMER & COMMUNITY ADVISORY COMMITTEEOur Warrnambool and Camperdown Community Advisory Committees and Partnering with Consumers Committee merged during 2015–16. Its inaugural meeting was held in June.

This committee provides advice to South West Healthcare to reflect consumer, carer and community views in our service delivery, planning and policy development. This year it participated in the development of our 2015 Quality of Care Report and reviewed issues arising from patient satisfaction surveys and consumers. It also monitored and reviewed the requirements of Standard 2 of the National Safety and Quality Healthcare Service Standards (Partnering with Consumers) which resulted in the development of a Consumer Engagement Framework. This committee met four times in 2015–16.

THE 2015 RECIPIENT OF OUR MOST PRESTIGIOUS AWARDAllied Health and Ambulatory Rehabilitation Manager Kait Brown (pictured) was awarded our 2015 AEW Matthews Memorial Travelling Scholarship.

Created by our Board of Directors in 1991 in memory of former long-serving CEO, the late Allan Matthews, it allows recipients to travel overseas to be exposed to international best practice models, programs and other initiatives. Generously funded by the AL Lane Foundation, it’s the most prestigious honour our employees can receive.

Kait’s itinerary focused on premature babies and children, and learning about ways to help them and their families to reach their full potential. In April she attended the National Association of Neonatal Therapists Conference in Orlando, Florida. She also visited a number of healthcare and research organisations in Boston and New York, including Harvard’s Boston Children’s Hospital, the Massachusetts Hospital for Children, and Lennox Hill Hospital. Each of these organisations has well established follow-up programs where a team of doctors and developmental therapists assess and guide premature babies and children’s development once they are out of hospital and living at home.

With a background in speech pathology, Kait knows firsthand the challenges premature babies and their families can face and hopes South West Healthcare will be able to implement a contemporary, integrated model of care to ensure timely follow up and intervention for the at-risk children she and her colleagues work with.

A Life Governorship is the highest recognition South West Healthcare can bestow. Our recipients have given an outstanding contribution to our health service over a prolonged period of time. At our 2015 Annual General Meeting another five extra ordinary volunteers were added to this elite honor roll:

> Mary Alexander diligently represented our Board of Directors on seven committees between her 2004–2015 tenure, chaired the Human Research Ethics Committee from 2008–15 and was deputy vice chair of the Board in 2010–11 and 2012–14. Her extensive knowledge and expertise was highly valued and she was greatly respected.

> Claire Gibbons and Olive McKenzie have volunteered with our Warrnambool Base Hospital Palliative Care Service for more than 22 years and 19 years respectively. Caring and compassionate, they give willingly and generously to our Volunteer Program and their efficient and friendly nature makes them ideal one-to-one volunteers.

> Elwyn Jasper has volunteered at Macarthur Community Health for 20 years and Murray Jasper for 19. Both drive our bus, taking clients to appointments and shopping trips. Elwyn’s also done long stints in our Adult Day Centre while Murray, renowned for his great sense of humor, also supports our morning teas/luncheons and our training days.

> At our Warrnambool Base Hospital 76 volunteers participate in 24 onsite programs including helping out in our Supply Department, Library and Pharmacy, assisting with patient activities in our Medical and Rehabilitation Units, helping our professional gardener, and bringing our much-loved Delta Therapy Dogs for weekly visits. Another 33 volunteers participate in six palliative care-specific programs.

> Meals on Wheels is the domain of eight volunteers at Lismore Community Health, ensuring a nutritional meal is delivered to the doorsteps of clients (and often their carers)

who are frail-aged and/or living with a disability. Many of them are also rurally and/or socially isolated. Another eight volunteers form our SWH Lismore Community Health Women’s Auxiliary and two assist with our Planned Activity Group.

> Of Camperdown Hospital’s 92 volunteers, 66 support our Meals on Wheels initiative. Others volunteer via their involvement with our Camperdown & District Hospital Auxiliary and our Camperdown Hospital Trolley Auxiliary.

> Volunteer assistance at Merindah Lodge sees 12 volunteers and nine Friends & Relatives of Merindah (FROM) members assisting with a variety of activities including bacon and egg mornings, craft, music, outdoor gardening, social outings, pet therapy visits and bus driving.

> David Newman Adult Day Centre volunteer activities include serving meals to clients, kitchen duties, delivering a music-based program, bus driving and supporting and assisting our clients during activities and outings.

> At Warrnambool Community Health four volunteers staff our National Diabetes Services Scheme (NDSS), seven volunteer in our Ostomy Department and two provide program-related administration support.

> At Manifold Place six trained NDSS volunteers provide test strips, needles, syringes and lancets for clients with diabetes types 1 and 2.

> Bus driving, transporting clients to medical appointments, Planned Activity Group assistance, gardening, Telecare and Broadband for Seniors are just some of the activities our 25 volunteers at Macarthur Community Health help with.

> Four volunteers support our newly-established Warrnambool Mental Health Services Hider House therapeutic day program for consumers/carers in the community and consumers who are inpatients.

> Additional to our registered volunteers we have hundreds more who volunteer for us on our Board of Directors and Principal Committees, other committees and auxiliaries, at our own fundraising events and at fundraising events held for us.

THE EDUCATION AND TRAINING OF OUR VOLUNTEERSOur volunteers receive regular training and upskilling as individual and group needs arise. Training provided during 2015–16 included dementia awareness training, bus and defensive driver training and palliative care training.

RECOGNISING OUR VOLUNTEERSThe dedicated commitment of 26 individuals and one service club who, collectively, have volunteered at our Camperdown facilities for 320 years, was formally recognised at our 2015 SWH Camperdown Year in Review. Service Awards went to:

As further recognition and gratitude for everything they do for us, we held a morning or afternoon tea at every one of our campuses during National Volunteers Week in May.

The loyal and generous support of hundreds of donors allowed us to raise $499K during 2015–16. This much-needed assistance saw to the purchasing of dozens of pieces of medical equipment and to the financing of otherwise unaffordable initiatives to help treat and care for our communities.

We celebrated four particularly significant donor-related achievements:

> Four generous donors were responsible for the $82,981 replacement of our Warrnambool Base Hospital’s aged theatre trolleys (used by more than 123,000 patients over 19 years). A $58,687 bequest from the late Graham Surridge, $13,207 from the $18,000 proceeds of our staff organised 2015 Warrnambool Corporate Charity Golf Day, a $400 Christmas gift from the Panmure CWA and $10,687 from Midfield Meats International are to thank.

> Midfield Meats International also contributed $33,747 towards $85,747 of ENT medical equipment purchased for our Camperdown Hospital. This will allow us to commence, this coming July, an ENT Service (elective surgery) to reduce the waiting time for people in our region to access ears, nose and throat-related surgery. This broadens the scope of surgical procedures available in Camperdown and will spare local residents having to travel away.

> Our SWH Woolsthorpe Auxiliary’s fundraising for our Warrnambool Base Hospital earned them the Group Volunteer Award at the 2015 Australian Government National Volunteer Awards for Wannon (page 9/photo 18). They held 52 events in 2014–15 to donate $25,200 for the 2015–16 purchasing of three sets of mobile chair scales for our Day Stay, Medical and Acute Units; two ICU humidifiers; an Acute Unit lifting machine; exercise bike for our Extended Care Inpatient Unit residents; Midwifery Unit phototherapy unit; our first-ever fleet of palliative care concave mattresses, and an infusion pump.
