The MacKenzie Place and Cancer Service building (B1064G) located in the City of Grande Prairie, AB, is two storey building with two partial basement levels and crawl spaces built in 1984. Some renovations/modernizations were reported in 2000 and 2005. The building provides ~12,980 m² of space. It is used as office space, ambulatory cancer care, child day care and long term cancer care facility. However, most areas of the building were unused at time of assessment.

General Summary:

Structural Summary:The foundations and the structural frame were mostly concealed and no structural drawings were provided. Based on exposed sections and structural frame sections, the building is provided with cast-in-place (CIP) concrete wall foundations and CIP concrete piles with grade beams. The building frame likely consists of structural steel columns and beams. The floor decks consist of metal decking with a concrete topping slab. The roof deck is metal decking supported by openweb steel joists (OWSJ). Repair cost for the exterior ramps are anticipated within 5 years of writing this report. The buildings structural elements are on overall acceptable condition.

Envelope Summary:The exterior is provided with with brick veneer and pre-finished horizontal and vertical metal siding. Aluminum windows with fixed and operable sections are provided throughout the building. The staircases and the enclosed walkway between the MacKenzie building and the hospital are provided with pre-finished aluminum store front windows. Pre-finished aluminum storefront and automatic doors, as well as painted metal utility doors are provided around the building. The roof was not assessed due to snow coverage, but is was observed to be a standard built up roof (BUR) and pre-finished sloped metal roofing at the perimeters of the roof. Pre-finished metal soffits are provided around the building and at the level 0 entrance area at the south-west corner of thebuilding. Capital costs are anticipated to be required for the skylights the joint sealant, exterior doors, roof tiles and BUR roofing, the metal downspouts and the skylights within the next 5 years of writing this report. The buildings envelope elements are in overall acceptable condition.

Interior Summary:The majority of the the building is provided with resilient sheet flooring. The shower rooms are provided with ceramic floor tiles and partly with an epoxy anti-slip flooring. Some offices are provided with rolled carpet. Painted concrete flooring is provided in the mechanical rooms. Stone flooring is provided around the fire places in the common areas. Interior walls are painted CMU or gypsum board walls. Unfinished brick wall sections are located in each wing in the common areas. Ceramic wall tiles are provided in the shower rooms, behind the service sinks and in the galley. Some hallways are provided with wall paper. The basement storage areas are provided with chain link fencing partitions. Painted metal interior windows are installed throughout the building as well as pre-finished aluminum glazing partitions. The vast majority of the the building is provided with suspended T-bar ceiling grid with lay-in acoustic tiles. Painted gypsum board ceilings are provided in the shower rooms. The pavilion in the enclosed walkway between the cancer care center and the hospital is provided with painted metal decking. Unfinished metal decking and metal decking with spray on fire proofing is provided in the crawl spaces. Clear finish wood and painted metal doors with commercial grade hardware, set in painted metal frames are provided throughout the building. Capital cost are anticipated to be required for a barrier free code upgrade, replacement of the resilient and carpet flooring,gypsum wall repair, ceiling tile replacement and fixed casework are provided within 5 years of writing this report. The buildings interior elements are in overall acceptable condition.

Mechanical Summary:The mechanical systems in the Mackenzie Cancer Services building are generally original to the year of construction (1984). Plumbing fixtures include stainless steel sinks, vitreous china medical wash sinks, stainless steel refrigerated water fountains, showers and enameled bathtubs with stainless steel fixtures, vitreous china lavatories, and vitreous china flushvalve water closets. Backflow preventers are provided on the main domestic water supply, irrigation, and sprinkler systems. Low pressure steam water heaters provided domestic hot water to the building. Water distribution is by cast iron or copper type 'L' piping. Lift stations are provided for the waste water and sanitary systems. The building is heated by steam boilers which are located in the Services building. Steam is routed to one of the basement mechanical rooms of the Mackenzie building where it is connected to heat exchangers which provide heating for the hot water and glycol distribution systems. Chilled water is also provided by the Services building. The hot glycol and chilled water is distributed to approximately eight air handling units which provide heated and cooled air to low pressure galvanized steel ducting throughout the building. Hot water is distributed through insulated copper piping to finned tube radiation, unit heaters, and fan coil units throughout. A Liebert air conditioning unit is provided in one of the basement electrical rooms of the building. A packaged rooftop air conditioner is provided on the roof of the building. Medical air, oxygen, and a medical vacuum system with an Ohio alarm system are provided in patient areas of the building. Building exhaust is provided by various interior and rooftop fans. Pneumatic devices control the majority of mechanical equipment in the building. A Reliable building management control system is used to control some of the mechanical equipment throughout. A wet type sprinkler system, a dry type halon gas system fire suppression system, a fire pump assembly, dry type

chemical fire extinguishers, and standpipes are provided for fire protection. Capital expenditures with respect to the sinks, showers, bathtubs, backflow preventors, domestic water pumps, water conditioning equipment, water heaters, the oxygen system, vacuum system, air handling units, exhaust fans, heat exchangers, RTUs, fan coil units, and unit heaters within the next five years. Overall the mechanical systems in the building are in acceptable condition.

Electrical Summary:The electrical systems in the building are generally original to the year of construction (1984) with some updates to the communication systems in 1998 and data systems in 2005. The fire alarm system was updated in 2012. Power is fed from utility owned transformers from the Acute Care building to a 2000 Amp 347/600 Volt main breaker and switchgear unit which is provided in the basement electrical room of the building. Power is then distributed to approximately 4 step down dry type transformers which range from 15 to 300 kVA and provide 120/208 Volt power to secondary panelboards throughout. Secondary electrical panelboards are provided in hallways and electrical rooms throughout and the majority are Federal Pioneer 225 Amp 120/208 Volt units with varying amounts of remaining capacity.Various Square D 600 Amp motor control centers are provided for the major mechanical equipment throughout. Individualmotor starters and electrical disconnects are provided for HVAC fans and pumps. Variable frequency drives are also provided for the air handling units and pumps throughout. Electrical wiring is copper in armoured cable or in conduit. Interior lighting is generally by fluorescent T12 fixtures as well as incandescent lighting in pot type and other decorative fixtures. Metal halide pendant style lighting is provided in the Green Thumb room. Exterior lighting is by incandescent potlighting, high pressure sodium wall mounted fixtures, and metal halide pole mounted fixtures. An emergency generator which is located in the Services building provides power for a portion of the lighting in an emergency situation. LED or incandescent exit signs are provided throughout the building. An original Edwards fire panel as well as a Siemens Pyrotronics fire panel is provided for the building. It is unclear whether these panels are still in use. Two Edwards EST 3 fire alarm panels were installed in 2012 and are used to monitor manual pull stations and heat and smoke detectors throughout. Visual and audible signals are by strobe lights and fire bells throughout. A two way radio is provided inside the main fire panel and likely serves this system. A security access card system along with a video surveillance system is provided. Digital clocks are connected to the master clocksystem in the Services building. A Phillips paging system, a nurse call system, and a television system are also provided. Category 5 cabling and a Cisco local area network is connected to the Alberta "Supernet". Uninterruptible power supply systems are located in the electrical rooms throughout the building and are used to provide power to critical care services in the building in the case of a power outage. Capital expenditures with respect to the secondary distribution, MCCs, motor starters, GFI plugs, interior lighting, fire detection/alarm, and the UPS systems are anticipated within the next five years. Overall the electrical systems in the building are in acceptable condition.

A water softening system with softening and mineral tanks provide softened hot water to the building.

Two low pressure steam heated water heaters are provided in the basement mechanical room of the building and are insulated and canvas covered. The insulation on one of the steam heaters is dented, but the cost to repair this insulation is likely less than the minimum amount ($1000) for a repair event.

A main Edwards EST 3 fire alarm panel is located on the main floor of the building and includes a phone and radio system. A small Edwards EST 3 fire alarm panel is located in the West corridor of Level 1 of the building.
