Prematüre bebek beslenmesinde anne sütü
• Bebeğin gereksinimleri anne sütünün içerdiği protein, Ca, P, Mg, Na, Co, Zn ve vitaminlerden fazla miktardadır.
• ZenginleşVrilmemiş anne sütü alan bebekler daha yavaş büyür; hiponatremi, hipoproteinemi, osteopeni ve çinko eksikliği oluşabilir.

1. ZenginleşVrilmiş anne sütü veya prematüre formül sütleri tercih edilen besinlerdir.
2. Prematüre bebeklerin emme, yutma ve soluma koordinasyonları tam gelişmediği için enteral besleme tercih edilir.
3. Enteral besleme aralıklı (bolus) veya sürekli infüzyon şekillerinde uygulanabilir. Aralıklı besleme daha fizyolojik olduğu için tercih edilir.
4. Orogastrik yol nazogastrik yola tercih edilir.

Fortification of human milk is essential because breast milk con-tent is insufficient to meet the needs of very low birth weight (VLBW) infants. Fortification protocols should be improved to better meet the needs of preterm infants.
Newly ExpressedAggressive fortification by adding higher amounts of human milk fortifier did not significantly affect growth rates and metabolic responses in VLBW infants. The need for further studies to optimize human milk fortification protocols remains.

Human Milk Fortification with Differing Amounts of Fortifier and Its Association with Growth and Metabolic Responses in Preterm Infants

AbstractBackground: Fortification of human milk (HM) is a common clinical practice to adapt breast milk to the nutritional needs of very low birth weight (VLBW) infants. The optimal method for HM fortification remains to be determined, and a variety of protocols are currently used in neonatal intensive care units.Objective: It is believed that standard fortification is insufficient to meet the needs of VLBW infants. Therefore, we designed a randomized prospective study that investigated the effects of varying levels of blind fortification on short-term growth and metabolic responses of preterm infants.Methods: Eligible infants were randomized into 3 groups: standard fortification (SF), moderate fortification (MF), and aggressive fortification (AF). Short-term growth, feeding intolerance, and urea, calcium, phosphorus, and alkaline phosphatase levels were assessed.Results: There were 26, 29, and 29 infants in the SF, MF, and AF groups, respectively. The baseline characteristics of the groups were similar. Daily weight gain and length at discharge did not differ among the groups; however, head circumference was significantly higher in the MF and AF groups compared with the SF group. Urea, calcium, phosphorus, and alkaline phosphatase levels were similar between the groups.Conclusion: We demonstrated that blind fortification of HM, even with higher amounts than recommended by manufacturers, did not cause any measured adverse effects on the metabolic response of preterm infants. Anthropometric measurements (except head circumference) were not different between the different dosages of fortification.

An intention to achieve better postnatal in-hospital-growth for preterm infants:Adjustable protein fortification of human milk

Objective:We assessed the effect of human milk (HM) fortification with extra protein supplement by an adjust-able protein fortification method according to the weekly blood urea nitrogen (BUN) levels on growth in hospi-talized preterm infants.Method: A prospective observational intervention study in 58 preterms born ≤32 weeks of gestation and fedwith breast milk was conducted. Preterms who were given a commercial HM fortifier which provides an addi-tional protein of 0.8 g/3 scales according to the standard feeding strategy served as a historical control group.Infants who were given extra protein in addition to the HM fortifier with another commercial protein supple-ment which provides an additional protein of 2.2 g/1 scale comprised the intervention group. Additional proteinsupplementation was adjusted according to BUN levels weekly in the intervention group. Weight gain velocities(g/kg/day), length, head circumferences (HC) gain velocities (mm/day) and daily growth indexes for weight,height and HC (percentage per day) were calculated.Results: The median amount of daily enteral protein intake [4 (3.4–4.6) vs. 2.78 (2.1–3.1) g/kg/day, p b 0.0001]was significantly higher in the interventional group. Length (p = 0.008) and HC (p b 0.0001) gain velocitieswere significantly higher in the intervention group. Daily growth indexes for weight (2.2% vs. 1.8%, p = 0.026),for length (0.4% vs. 0.3%, p = 0.027) and for HC (0.48% vs. 0.36% per day, p = 0.003) were significantly higher inthe intervention group.Conclusion: A higher protein intake by adjustable protein fortification method without energy or volume changeleads to improved postnatal in-hospital-growth in very low birth weight infants.

1. Introduction
The most preterm infants, especially the smallest and the sickestones have not only high nutrient requirements but also are not fedwith sufficient amounts of nutrients to produce normal fetal growthrates. This results in a lag behind fetal growth curves and ends upwith growth-restriction during their hospital period after birth [1,2].
Human milk (HM) is preferred in feeding preterm infants mostlydue to its immuno-nutritive properties which protect the infant againstinfection and necrotizing enterocolitis [3,4]. Moreover, feedingwithHMis associated with shorter stay in hospital and better psychomotordevelopment as compared with other feeding regimens [5]. However,HM provides insufficient amounts of some nutrients and insufficientenergy for very low birth weight infants when fed at the usual feedingvolumes. HM must therefore be fortified with the nutrients in shortsupply to enhance the growth of this group of infants [6,7].
Recently, it has been shown that feeding preterm infants with pro-tein enriched diet leads to positive nitrogen balance, enhanced whole
body protein synthesis, improved postnatal growth, larger brains andimproved cognitive function [2,8]. Commercially availablemulticompo-nent fortifiers provide additional protein, an energy source,minerals, vi-tamins and trace elements [7,9]. However, it has been shown thatprotein intake provided by standard human milk fortification is inade-quate [10,11] and does not meet the recommended intakes for preterminfants [12,13]. The amount of extra protein provided by HM fortifiers isbetween 0.7 and 1.1 g/100 ml depending on the brand [14]. But theprotein content of preterm HM is relatively low and extremely variableamong mothers and decreases over the time [4].
The challenge in providing the appropriate protein intakes withstandard fortification had been addressed previously and “adjustableprotein fortification method”was designed. Adjustable HM fortificationmethod comprises of adding extra protein to fortified HM based ontwice weekly blood urea measurements and was proven to be safeand effective in enhancing growth and protein intakes for preterminfants [15]. In the present study we applied a modified “adjustableprotein fortification method”: the protein content of human milk wasincreased when needed, by adding extra protein to standard HM fortifi-cation based on weekly BUNmeasurements. We compared this feedingregimen with the previous one which utilized a standard HM fortifica-tion. We hypothesized that an increase in protein intake without any
