OMCG concerns – Inmate UNS

Sir,

On 31/01/2014 made a statement to SAPO Hadley that he was doing his head in, in unit. Miss Hadley spoke to me and said that concerns were

in relation to OMCG issues he had in . Miss Hadley informed me that

had declared himself as a nominee during his reception and induction

committee meeting held on 29/01/2014.

Author reviewed the case notes from this RIC meeting and gleaned the following;

Reception Induction Committee interview conducted by AS Case Manager G. Comer SAPO M Hadley & SAPO H Zorz. Inmate currently housed in unit 5.1.2 as A2U/RPB. Inmate is normal discipline currently during RIC identified safety issues related to OMCG ( nominee). CCAP: nil issues. Inmate is a non-smoker (completed acknowledgment form). Inmate issued LGW CC inmate handbook. Core skills assessment referred to education for assessment. SDRO: IRI form completed & faxed. AVO's: nil active. LSIR: A&OD:

ALERTS: asthma/ . FURTHER COURT: AVL Central

Local Court. SPA review: nil. CASE PLAN: to be reviewed after court. Nil outstanding OS&P referrals. Presenting issues or concerns - inmate identified during RIC that he has fears for his safety due to OMCG association. AS Case Manager Comer to notify Sector Manager 2 to undertake risk assessment. Nil thoughts of self-harm.

Author interviewed on 31/01/2014. During the interview again

declared that he was a nominee for the OMCG. stated that he had

some concerns regarding his placement in unit. stated that he did not

know why he was moved to this unit. said that he had identified several

OMCG members in stated that he had experienced

OMCG related problems at Parklea CC.

Local action

removed from unit and placed in 5.2 unit pending risk assessment

has submitted an application declaring his OMCG status and his

problems with OMCG members in unit

OMCG information disseminated to local intel

Recommendations

Risk assessment to be conducted on

to be relocated to another centre to facilitate appropriate management

of him as an OMCG member
