Attached is a copy of a letter dated 10 April 2019 from Acting Head ElizabethT. Te, Office of European Affairs, Department of Foreign Affairs (DFA), informing allbranches of government of the short-term Schengen visa requirements whenapplications for such visa are filed through the Czech Embassy in Manila.

Dear SIrs/Mesdames,
The Department of Foreign Affairs, acting on the request of the Czech Embassy inManila, wishes to inform all branches of government of the short-term Schengen visarequirements when filing their applications through them:
a. Philippine diplomatic or official passport holders do not need a visa for travel tothe Czech Republic for a period not exceeding 30 days.'' However, please notethat some countries in the Schengen area require diplomatic/official passportholders to apply for a visa.
b. For regular passport holders, visa applicants can apply for a short-term Schengenvisa (up to 90 days) at least three months before or 15 days before the planneddeparture at the latest. The Czech Embassy recommends applying for visa inadvance, otherwise the embassy cannot guarantee the visa application will be
decided on time especially during the peak visa season (March to May, August toSeptember). According to the Czech Visa Code Article 23: "Applications shall bedecided on within 15 calendar days of the date of the lodging of an applicationwhich is admissible in accordance with Article 19."
c. Applications are accepted by the outsourcing company VPS. Applicants shouldmake an appointment through the VFS website:https://www.vfsqlobal.com/Czechrepublic/Philippines/. If the purpose of the travel
is an official visit, the representative of delegates may contact the Embassy of theCzech Republic in Manila directly and delegates can get a VIP appointment at theVFS.
For short-term visa applicants, each visa application must be complete on its own.Documents of one applicant will not be searched in the application of anotherapplicant. Each applicant must submit the following documents:1. Filled out application form;2. Travel document must be valid for at least 90 days after the intended departure
from Schengen and must contain at least two blank pages;

3. One recent photograph sized S.Scm x 4.5cm, with a light colored background.
The photograph must be pasted or glued on the designated space on the upper
right side corner of the first page of the application form;4. Travel medical insurance must be valid for the whole territory of the Schengen
Area and cover the entire period of the person's intended stay or transit. The
minimum coverage must be EUR 30,000;
5. Invitation letter from the inviting organization/institution which contains full
names of visa applicants, passport numbers, dates of travel to the Czech
Republic. It can include a sponsorship confirmation;6. Detailed itinerary (dates, places, program);7. Flight reservations;
8. Hotel bookings for all Schengen countries the applicant is to visit;9. Applicant's original bank certificate;10. Applicant's original bank account statement: three-month history;11. Certificate of employment and leave of absence;
12. Company representatives: company registration/ annual income tax return;13. In case the visa applicant wants to extend his/her trip as a tourist, he/she has
to have enough financial means on his/her bank account to support theextension. Hotel bookings and itinerary for extension will be part of the visaapplication.
e. Additional requirements for short-term visa of accompanying family members areas follows:
14. Affidavit of support (if applicable);15. Minors: Confirmation of school enrollment;
16. Minors: Affidavit of consent (both parents agree with travel abroad)17. Unemployed: ties to the Philippines
