Supports and Services you can Access with your Home Care Package This guide outlines the care, support and clinical services which your Home Care Package funds may be used for, depending on your care requirements as outlined in your Care Plan. It also includes a list of excluded items as specified by My Aged Care.

If there is a service or an aid that you would like to fund that is not listed below please speak with Let’s Get Care to ensure eligibility.be

Care Services Includes:

Personal services Personal assistance, including individual attention, individual supervision and physical assistance with:

• bathing and showering (including providing showerchairs if necessary), personal hygiene and grooming,dressing and undressing, using dressing aids; and

• toileting; and• mobility; and transfer (including in and out of bed)

• communication including assistance to addressdifficulties arising from impaired hearing, sight orspeech, or lack of common language, assistance withthe fitting of sensory communication aids, checkinghearing aid batteries, cleaning spectacles andassistance in using the telephone

Activities of daily living Personal assistance, including individual attention, individual supervision and physical assistance, with: Nutrition, hydration, meal preparation and diet

• assistance with preparing meals; and• assistance with special diet for health, religious, cultural

or other reasons; and• assistance with using eating utensils and eating aids

and assistance with actual feeding, if necessary; and• providing enteral feeding formula and equipment

Management of skin integrity

• Includes providing bandages, dressings, andskin emollients

Continence management

• assessment for and, if required, providing disposablepads and absorbent aids, commode chairs, bedpansand urinals, catheter and urinary drainage appliancesand enemas; andassistance in using continence aids and appliancesand managing continence

Mobility and dexterity

• providing crutches, quadruped walkers, walking frames,walking sticks and wheelchairs; and

• providing mechanical devices for lifting, bed rails, slidesheets, sheepskins, tri-pillows, and pressure relievingmattresses; and

• assistance in using the above aids

Support Services Includes:

Support Services

• cleaning; and• personal laundry services, including laundering of care

recipients clothing and bedding that can be machine-washed, and ironing; and

• arranging for dry-cleaning of care recipients clothing and bedding that cannot be machine-washed; and

• gardening; and• medication management; and• rehabilitative support, or helping to access rehabilitative

support, to meet a professionally determined therapeutic need; and

• emotional support including ongoing support in adjusting to a lifestyle involving increased dependency and assistance for the care recipient and carer; and

• support for care recipients with a cognitive impairment, including individual therapy, activities and access to specific programmes designed to prevent or manage a particular condition or behaviour, enhance quality of life and provide ongoing support; and

• providing 24-hour on-call access to emergency assistance including access to an emergency call system if assessed as being required; and

• transport and personal assistance with shopping, attend a health practitioner or attend social activities; and

• respite care; and• home maintenance, reasonably required to maintain the

home and garden in a condition of functional safety and provide an adequate level of security; and

• modifications to the home, such as easy access taps, shower hose or bath rails; and

• assisting the care recipient, and the homeowner if the home owner is not the care recipient, to access technical advice on major home modifications; and

• advising areas of concern in the home that pose safety risks and ways to mitigate the risks; and

• assistance to access support services to maintain personal affairs

• arranging social activities, providing or coordinating transport to social functions, entertainment activities and other out-of-home services

Leisure, interests and activities

• encouragement to take part in social and communityactivities that promote and protect your lifestyle, interestsand wellbeing

The following services or items are outside of the Home Care Package program.

Excluded Items

The following items must not be included in a care recipients home care package:

• use of package funds as a source of general income;• purchase of food, except as part of enteral feeding

requirements;• payment for permanent accommodation, including

assistance with home purchase, mortgage payments or rent;

• pay for home care fees (basic daily care fee, income tested care fee or elected voluntary top up fee)

• pay fees or charges for other types of care funded or jointly funded by the Government (residential respite care fees);

• cost of home modifications or capital items that are not related to care recipient care needs;

• cost of travel and accommodation for holidays;• cost of entertainment activities, eg: club memberships or

tickets to sporting events;• gambling activities;• payment for services and items covered by the Medicare

Benefits Schedule or the Pharmaceutical Benefits Scheme.

Clinical services Includes:

Clinical care • nursing, allied health and therapy services such as speechtherapy, podiatry, occupational or physiotherapy services;and

• other clinical services such as hearing and vision services.

Access to other health and related services

• referral to health practitioners or other relatedservice providers

Care Management

• Includes reviewing the care recipient’s home careagreement and care plan, coordinating and schedulingcare and services, ensuring care and services are alignedwith other supports, liaising with the care recipient and thecare recipient’s representatives, ensuring that care andservices are culturally appropriate, and identifying andaddressing risks to the care recipient’s safety.
