Herts Healthy Homes – Local Initiatives Project Fund Guidance Document Herts Healthy Homes is a year round seasonal strategy delivered through local partnerships with a focus on social and environmental factors within the home that impact upon health and wellbeing. Herts Healthy Homes will provide early interventions and support to householders and identified carers. The Local Initiatives Project Fund has been set up to enable local groups and organisations to bid for some funding which will support the ethos of the Herts Healthy Homes Project, making use of local intelligence, to ensure vulnerable people receive support to remain living independently in their own home. Key Themes Priority will be given to projects which support the following key themes. to identify risks to persons within the home such as falls prevention. to deliver health prevention measures within the home/home environment. to identify carers and offer early interventions that will enhance or maintain resident’s health and wellbeing. other environmental and social factors within the home that have the potential to affect health and wellbeing. Applicants must demonstrate how their project will contribute to the aims set out above.

Who can apply?Any voluntary or community group can apply, including those already in receipt of funds. This fund is intended to be flexible, however, for an application to be considered, the project/ initiative requiring funds must have a set of rules and comply with the following conditions.

Hold a bank account in the name of the group, requiring at least two signatories. Have a Management Committee of at least three people. Be non-profit making or a social enterprise where profits are reinvested for

community or social benefit. Be committed to equality and diversity in employment and service delivery.

What can be funded? Revenue as a ‘one-off’ grant for a specific project, event or programme of events New projects, a project where no funding has been allocated or an extension of an

existing project Short term staffing costs which are needed to extend an existing project on a short

term basis Volunteer Management expenses

What cannot be funded? Capital items.

long term or on-going revenue costs.

activities promoting religious or political beliefs.

projects involving improvement or repair to buildings which are the responsibility of another statutory body.

capital projects in buildings or other capital cost improvements e.g bus stop, street lighting.

projects that take place outside Hertfordshire.

When can I apply?Applications must be submitted by the 28th November 2014. Funding will not be reserved for later rounds but if there is any money left over a second grant round will be opened in 2015.

How much can I apply for?Applicants can apply for up to £3,000 for local initiatives, where an application is for a Countywide project the panel will consider applications for up to £5,000.

What documentation will I need to supply with my application? Set of rules which govern your group (or constitution).

Charity number.

Recent bank statement or 1 years accounts.

Safeguarding vulnerable people policy or statement.

How will my application be assessed?Applications will be assessed by a grants panel and will be scored according to how well they meet the following criteria.

• how the application supports the Herts Healthy Homes priorities and key themes as set out in this guidance document

• the extent to which partnerships are developed though the project/ initiative• whether the application represents “value for money”

When will I be informed about the outcome of the application?The grants panel meets two weeks after the closing date. Formal notification can take up to four weeks from the closing date of that round due to the approval process. Any grant offer is subject to the availability of funding and approval by Hertfordshire County Council. The decision of the grants panel is final. Unsuccessful applicants can request feedback and are welcome to reapply (if appropriate) in later rounds.

Are there any special conditions on the Project Funds? All successful applicants will be required to sign the Data Protection Declaration as a condition of receiving funds (appendix one). Applicants may be subject to individual conditions, which relate to the application and will be confirmed in the project funds offer letter. In addition:

• We reserve the right to signpost your organisation or community group towards local partners that you should work with to ensure a joined up approach across the County.

• We may ask for your support in future to publicise consultation and key Hertfordshire County Council messages to your users.

• Where projects are dependent on another source of funding, Hertfordshire County Council may make any funds conditional on the success of the other application/s.

How will the outcomes of the project be monitored after the project funds have been received? All successful applicants should take photographs and keep evidence of the success of the project. You will be required to submit an update report within 6 months of the funding being received. Additional controls may be put in place and the Council may request receipts/invoices/other proof of spend so make sure to keep records.

Data protection and Declaration

The Council may share the information supplied to other Council departments and Herts Healthy Homes partners.

Project funding applications paperwork is kept in accordance with the Council’s retention policy. Hertfordshire County Council is the data controller for the purposes of the Data Protection Act 1998.
