(antidromic atrioventricular reentrant tachycardia (antridromic AVRT) ในผู้��ป�วยที่��เป�นโรค Wolff-Parkinson-White syndrome)

Ventricular pace rhythmsมี�การพยายามีแยกภาวะ VT หร#อ SVT ด้�วย criteria ที่าง ECG เช่�นด้�จาก

QRS complex duration, QRS axis, concordant pattern, presence of Q-waves, AV dissociation, fusion beats, absence of precordial RS complex, QRS alternans,

แต่�จร&งๆแล้�วจากประว)ต่&ก*พอจะที่+าให�ค&ด้ถึ-งภาวะ VT หร#อ SVT ได้�เช่�น ล้)กษณะที่��น�าจะเป�น VT

อาย0มีากกว�า 35 ป1 มี� structural heart disease (ischemic heart disease,

previous MI, congestive heart failure, cardiomyopathy)

มี�ประว)ต่& sudden cardiac death ในครอบคร)ว (อาจมี� HOCM,

congenital long QT syndrome, Brugada syndrome หร#อ arrhythmogenic RV dysplasia)

ล้)กษณะที่��น�าจะเป�น SVT with aberrancy

Previous ECG มี� bundle branch block pattern ซึ่-�งร�ปร�างล้)กษณะเหมี#อนก)บ ECG ต่อน tachycardia

Previous ECG มี� WPW pattern (short PR < 120 ms,

presence of delta wave)

มี�ประว)ต่&เคยเป�นมีาก�อนแล้ะสามีารถึหายได้�จากการที่+า vagal maneuvers

หร#อฉี�ด้ adenosine

ส+าหร)บ ECG criteria มี�หล้าย criteria ด้)งต่�อไปน��

1. Brugada criteria

Morphology criteriaล้)กษณะร�ปร�างของ QRS complex ที่��สน)บสน0น VT ได้�แก� Right bundle branch block complex (RBBB) VT

V 1 : monophasic R หร#อ qR V6: R/S ratio < 1

Left bundle branch block complex (LBBB) VTV1 แล้ะ V2 : 1 : duration r > 30 msec

2 : notched downstroke ของ S wave

3 : interval ระหว�างจ0ด้เร&�มีต่�นของ QRS แล้ะ จ0ด้ต่+�าส0ด้ของ S wave > 60 msec

V6 : มี� Q wave หร#อ qR

ล้)กษณะของ wide QRS ที่��สน)บสน0น PSVT with aberrant ventricular

conduction ได้�แก� aberrant conduction ที่��เป�นแบบร�ปร�างของ RBBB

V1 : เป�น triphasic QRS complex ช่น&ด้ที่�� R wave ของต่)วหล้)งส�งกว�า R wave ของต่)วหน�า rSR’ complex

V6 : มี�ล้)กษณะเป�น qRs โด้ย R/S ratio มีากกว�า 1 aberrant ventricular conduction ที่��เป�น แบบร�ปร�างของ LBBB

V1 : 1. Duration ของ initial r wave < 30 msec ไมี�มี� notching

ใน downstroke ของ S wave

2. interval ระหว�าง onset ของ QRS complex ก)บจ0ด้ต่+�าส0ด้ของ S wave ไมี�เก&น 60 msecV6 : no q wave

ถึ�าหากสงส)ยแล้ะแยกไมี�ได้�ว�า wide complex tachycardia ที่��เห*นเป�น VT หร#อ SVT แนะน+าให�ร)กษาแบบ VT จะปล้อด้ภ)ยต่�อผู้��ป�วยที่��ส0ด้
