1. Client /s to write on the logbook the following information for record purposes:
- Date -Name -Address -Name of inmate to be verified -Prison No. -Relationship to the inmate
2. Inmate Documents and Processing Division
Officer of the Day (IDPD OD) to fill-up the Inmate Information Center Verification Form and write therein the result of verification.
3. IDPD OD to give a copy of the filled-up
verification form to the client. 4. Client to safe keep the verification form to be
presented on his/her next follow-up.
Note: If there are other clarifications that cannot be undertaken by the O.D., the said matter will be referred to the OIC, Inmate Documents and Processing Division.

III. Issuance of Prison Records

1. For inmate and his immediate family, prison record (PR) is available for issuance and to acknowledge receipt of the same by affixing the signature of the requesting party to the logbook.
2. For Legal counsel and paralegal worker/RVO to submit request letter from the inmate.
3. IDPD OD to receive the letter-
request and to forward the Prison Record direct to the inmate through routing slip.

IV. Issuance of Certificate of Detention/Confinement

Certification for signature of OIC, IDPD.

Confinement to the requesting party.
