BlueAlliance Provider Directory This directory lists physicians, hospitals and other health care providers who have contracted with Blue Cross Blue Shield of Arizona (BCBSAZ) as BlueAlliance providers. Since individual providers and/or BCBSAZ may elect to terminate the agreement at any time, the status of providers is subject to change. In order to avoid increased cost- sharing of your medical expenses by using an out-of-network provider, always confirm the provider’s status prior to your visit. You can contact the provider directly, call the Customer Service number located on the back of your ID card, or visit our website at azblue.com.

Services requiring precertification are listed in the Benefit Plan Booklet you’ll receive after enrolling, or prior to enrollment upon request. If, after reading your Benefit Plan booklet, you have specific questions, please call the BCBSAZ Customer Service department.

EMERGENCIES You do not need to precertify Emergency Services, as defined in your Benefit Plan Booklet, prior to seeking medical care.
