What is Arthroscopy?• Minimally invasive surgical procedure• Two incisions one for the arthroscope, and one for the

surgical instrument• Can also be used for diagnosis

History• First preformed by Professor Kenji Takagi in 1919• Used 7.3 mm cystoscope• Eugen Bircher started pioneering work in the 1920s• Japanese surgeon Masaki Watanabe first to preform

Arthroscopic surgery in the 1970’s

The arthroscope• A thin flexible fiber optic scope• Inserted into joint for diagnosis or repair• Allows the surgeon to see joint and preform surgery within

the joint• Contains a camera, and a light

Advantages • Less tissue damage• Faster recovery for patients • Less invasive

Page 6: Arthroscopy Matthew Brass BME 281. What is Arthroscopy? Minimally invasive surgical procedure Two incisions one for the arthroscope, and one for the surgical

disadvantages• Irrigation fluid is used to make surgical space and sometimes

leaks into the surrounding tissue causing edema• A condition where fluid accumulates in the interstitium

study's• A study was conducted to compare patient-reported outcome

(PRO) and rates of conversion to total hip arthroplasty (THA) after hip arthroscopy, Bernese periacetabular osteotomy (PAO), and a combined approach for the management of patients with different grades of hip dysplasia.

Page 8: Arthroscopy Matthew Brass BME 281. What is Arthroscopy? Minimally invasive surgical procedure Two incisions one for the arthroscope, and one for the surgical

results• 834 hips treated • mean age of 31 years• 114 were treated with arthroscopy alone• 703 were treated with PAO alone• 17 were treated with both procedures

Page 9: Arthroscopy Matthew Brass BME 281. What is Arthroscopy? Minimally invasive surgical procedure Two incisions one for the arthroscope, and one for the surgical

results• Arthroscopy has resulted in improved outcomes in borderline

dysplastic cases• PAO has primarily been used in true dysplasia with continued

success• There were too few combined procedures of arthroscopy with

PAO to reach a reliable conclusion in this subgroup.
