BAB I
PENDAHULUAN
I.1 LATAR BELAKANG
Penyakit kardiovaskuler merupakan peyebab kematian tertinggi di dunia dan
telah menjadi masalah global. Menurut data World Health Organisation (WHO)
terdapat 7,2 juta kematian seluruh dunia pada tahun 2008 adalah disebabkan
penyakit kardiovaskuler. Sedangkan di Amerika Serikat sekitar 400.000-500.000
tiap tahunnya meninggal dunia dan setengahnya meninggal diluar rumah sakit. Hal
ini tidak jauh berbeda dengan kondisi di Indonesia, menurut Survey rumah tangga
DepKes RI tahun 2008 angka kematian mencapai 25%. (Ade, 2011)
Sejak prosedur Percutaneous Transluminal Coronary Angioplasty (PTCA)
pertama kali dilakukan pada tahun 1977, penggunaan prosedur Percutaneous
Coronary Intervention (PCI) telah meningkat secara drastis, sekarang merupakan
salah satu tindakan intervensi medis yang umum dilakukan. Awalnya dikembangkan
di Swiss oleh Andreas Gruentzig, PCI telah mengubah tindakan revaskularisasi
untuk penyakit arteri koroner atau Coronary Artery Disease (CAD)
Angioplasty koroner pada awalnya digunakan dalam pengobatan pasien
dengan angina stabil dan lesi yang terpisah dalam arteri koroner tunggal, saat ini
memiliki beberapa indikasi, termasuk angina tidak stabil, infark miokard akut (IMA)
dan penyakit arteri koroner ‘multivessel’. Dengan kombinasi peralatan canggih,
operator berpengalaman, dan terapi obat modern, PCI telah berkembang menjadi
modalitas non bedah yang efektif untuk mengobati pasien dengan CAD. Kemajuan
teknis yang sedang berlangsung akan memungkinkan lebih banyak pasien dengan
oklusi arteri yang kronis akan berhasil diobati dengan prosedur PCI.
Perbaikan teknik kateter dan pengembangan perangkat baru dan obat-obatan
telah diberbaharui pula dengan meningkatnya pemahaman tentang fisiologi
kardiovaskular, pathogenesis arterosklerosis, dan respon terhadap cedera vaskuler.

Stent intrakoroner dan perangkat atherectomy telah dikembangkan untuk
meningkatkan keberhasilan dan mengurangi komplikasi, serta untuk memperluas
indikasi untuk revaskularisasi. Tindakan intervensi yang aman dapat mengobati lesi
koroner yang lebih kompleks. Pada saat yang sama, kemajuan dalam ultrasonografi
intravascular (IVUS) dan fractional flow reserve (FFR) telah meningkatkan
pemahaman morfologi lesi pada koroner, kerentanan lesi, dan fisiologi koroner.
Selain itu, banyak dari teknologi ini dapat membantu mengidentifikasi pasien
yang akan mendapatkan keuntungan besar dari PCI, Coronary artery bypass
grafting (CABG), atau terapi medis. Terapi farmakologis adjuvant bertujuan untuk
mencegah oklusi yang berulang, juga telah meningkatkan kemanan dan kemajuan
angioplasty koroner. (George A, 2014)

BAB II
TINJAUAN PUSTAKA
II.1 DEFINISI ANGIOPLASTY KORONER
Angioplasty koroner merupakan suatu tindakan intervensi non bedah
untuk membuka kembali pembuluh darah arteri koroner yang menyempit atau
tersumbat dengan memasukkan balon atau stent melalui kateter yang
dimaksukkan ke dalam lumen arteri melalui insisi kecil pada kulit. Angioplasty
koroner dikenal juga sebagai PCI (Percutaneous Coronary Intervention) atau
PTCA (Percutaneous Transluminal Coronary Angioplasty), lebih sederhana
disebut sebagai angioplasty, merupakan suatu prosedur untuk menangani
stenosis atau penyempitan dari arteri koroner. Penyempitan tersebut terjadi
karena plak atheroskelosis. (Sigmund S et all, 2005)

II.2 REKOMENDASI
(2011 ACCF/AHA/SCAI Guideline for Percutaneous Coronary
Intervention)
Class I: Conditions for which there is evidence for and/or general
agreement that the procedure or treatment is useful and effective.
Class II: Conditions for which there is conflicting evidence and/or a
divergence of opinion about the usefulness/efficacy of a procedure or
treatment.
Class IIa: Weight of evidence/opinion is in favor of usefulness/efficacy.
Class IIb: Usefulness/efficacy is less well established by evidence/opinion.
Class III: Conditions for which there is evidence and/or general agreement
that the procedure/treatment is not useful/effective, and in some cases may
be harmful.
UA/NSTEMI: Recommendations
CLASS I
1. An early invasive strategy (i.e., diagnostic angiography with intent to
perform revascularization) is indicated in UA/NSTEMI patients who have
refractory angina or hemodynamic or electrical instability (without serious
comorbidities or contraindications to such procedures) (Level of Evidence:
B)
2. An early invasive strategy (i.e., diagnostic angiography with intent to
perform revascularization) is indicated in initially stabilized UA/NSTEMI
patients (without serious comorbidities or contraindications to such

procedures) who have an elevated risk for clinical events (Level of
Evidence: A)
3. The selection of PCI or CABG as the means of revascularization in the
patient with ACS should generally be based on the same considerations as
those without ACS (Level of Evidence: B)
CLASS III: NO BENEFIT
1. An early invasive strategy (i.e., diagnostic angiography with intent to
perform revascularization) is not recommended in patients with extensive
comorbidities (e.g., liver or pulmonary failure, cancer) in whom (Level of
Evidence: C)
a. The risks of revascularization and comorbid conditions are likely to
outweigh the benefits of revascularization,
b. There is a low likelihood of ACS despite acute chest pain, or
c. Consent to revascularization will not be granted regardless of the
findings.
ST-Elevation Myocardial Infarction (STEMI)
PRIMARY PCI OF THE INFARCT ARTERY: RECOMMENDATIONS
CLASS I
1. Primary PCI should be performed in patients within 12 hours of onset of
STEMI (Level of Evidence: A)
2. Primary PCI should be performed in patients with STEMI presenting to a
hospital with PCI capability within 90 minutes of first medical contact as a
systems goal (Level of Evidence: B)

3. Primary PCI should be performed in patients with STEMI presenting to a
hospital without PCI capability within 120 minutes of first medical contac
as a systems goal (Level of Evidence: B)
4. Primary PCI should be performed in patients with STEMI who develop
severe heart failure or cardiogenic shock and are suitable candidates for
revascularization as soon as possible, irrespective of time delay (Level of
Evidence: B)
5. Primary PCI should be performed as soon as possible in patients with
STEMI and contraindications to fibrinolytic therapy with ischemic
symptoms for less than 12 hours. (Level of Evidence: B)
CLASS IIa
1. Primary PCI is reasonable in patients with STEMI if there is clinical and/or
electrocardiographic evidence of ongoing ischemia between 12 and 24 hours
after symptom onset (Level of Evidence:B)
CLASS IIb
1. Primary PCI might be considered in asymptomatic patients with STEMI and
higher risk presenting between 12 and 24 hours after symptom onset. (Level
of Evidence: C)
CLASS III: HARM
1. PCI should not be performed in a noninfarct artery at the time of primary
PCI in patients with STEMI without hemodynamic compromise (Level of
Evidence: B)

DELAYED OR ELECTIVE PCI IN PATIENTS WITH STEMI:
RECOMMENDATIONS
CLASS IIa
1. PCI is reasonable in patients with STEMI and clinical evidence for
fibrinolytic failure or infarct artery reocclusion. (Level of Evidence: B)
2. PCI is reasonable in patients with STEMI and a patent infarct artery 3 to 24
hours after fibrinolytic therapy (Level of Evidence:B)
3. PCI is reasonable in patients with STEMI who demonstrate ischemia on
noninvasive testing. (Level of Evidence: B)
CLASS IIb
1. PCI of a hemodynamically significant stenosis in a patent infarct artery
greater than 24 hours after STEMI may be considered as part of an invasive
strategy. (Level of Evidence: B)
CLASS III: NO BENEFIT
1. PCI of a totally occluded infarct artery greater than 24 hours after STEMI
should not be performed in asymptomatic patients with 1- or 2-vessel
disease if patients are hemodynamically and electrically stable and do not
have evidence of severe ischemia. (Level of Evidence: B)
II.3 KOMPLIKASI
Stent trombosis
Ini adalah resiko sampai stent menjadi tertutup oleh endothelium

Biasanya muncul sebagai MI akut, dengan angka kematian yang tinggi
Hal ini paling sering terjadi selama bulan pertama, namun dapat terjadi
beberapa bulan atau tahun setelah PCI
Terjadi pada 1-2% pasien.
Restenosis stent
Hal ini disebabkan oleh penyembuhan yang berlebihan dari dinding
pembuluh darah sehingga melewati batas dari stent
Biasanya berkembang dalam waktu 3-6 bulan
Muncul sebagai angina pectoris, jarang menyebabkan MI
Terjadi pada 4-20% kasus
Komplikasi utama lainnya jarang terjadi, tetapi meliputi kematian (0,2%),
MI akut (1%) yang mungkin memerlukan CABG darurat, stroke (0,5%),
tamponade jantung (0,5%) dan perdarahan sistemik (0,5%).
Komplikasi lain yaitu alergi terhadap kontras, nefropati, dan komplikasi di
lokasi akses, seperti pendarahan dan hematoma. (Grech, 2003)
II.4 PROSEDUR
Akses dimulai dari arteri femoralis pada kaki (atau yang lebih jarang
menggunakan arteri radialis atau arteri brachialis pada lengan) dengan
menggunakan suatu alat yang disebut jarum pembuka.
Sekali jarum sudah masuk, "sheath introducer" diletakkan pada jalan
pembuka untuk mempertahankan arteri tetap terbuka dan menontrol
perdarahan.

Melalui sheath introducer ini, "guiding catheter" dimasukkan. Ujung
“guiding catheter” ditempatkan pada ujung arteri koroner. Dengan "guiding
catheter", penanda radiopak atau kontras diinjeksikan ke arteri koroner,
hingga kondisi dan lokasi kelainan dapat diketahui.
Selama visualisasi X ray, ahli jantung memperkirakan ukuran arteri koroner
dan memilih ukuran balon kateter serta guide wire koroner yang sesuai.
“Guiding wire koroner” adalah sebuah selang yang sangat tipis dengan
ujung radio opak yang fleksibel yang kemudian dimasukkan melalui
“guiding cathether” mencapai arteri koroner. Dengan visualisasi langsung,
ahli jantung memandu kabel mencapai tempat terjadinya blokade . Ujung
kabel kemudian dilewatkan menembus blokade.
Setelah kabel berhasil melewati stenosis, balon kateter dilekatkan
dibelakang kabel. Angioplasti kateter kemudian didorong kedepan sampai
balon berada di dalam blockade.
Kemudian baru balon balon dikembangkan dan balon akan mengkompresi
atheromatous plak dan menekan arteri sehingga mengembang.
Jika stent ada pada balon, maka stent diimplantkan (ditinggalkan pada
tubuh) untuk mendukung arteri dari dalam agar tetap mengembang. (Grech,
2003)

II.5 PASCA PROSEDUR
Terapi obat antiplatelet setelah tindakan
Pasien sebaiknya menggunakan aspirin untuk seumur hidup (sebagai
pencegahan sekunder penyakit kardiovaskuler)
‘Dual antiplatelet therapy’ diperlukan untuk pasien pasca PCI / angioplasty
koroner, untuk mengurangi resiko thrombosis pada stent.
Biasanya terdiri dari aspirin dan clopidogrel. Beberapa ahli jantung
menyarankan untuk menggunakan clopidogrel seumur hidup
Jika operasi diperlukan, pertimbangkan apakah obat antiplatelet dapat
dilanjutkan

Penggunaan proton pump inhibitor (PPI) bersamaan dengan clopidogrel
masih kontroversial, setelah bukti yang menunjukkan PPI dapat
memperburuk hasil dan kedua obat dapat berinteraksi. (Chen, 2009)

BAB III
KESIMPULAN
Angioplasty koroner dikenal juga sebagai PCI (Percutaneous Coronary
Intervention) atau PTCA (Percutaneous Transluminal Coronary Angioplasty), lebih
sederhana disebut sebagai angioplasty, merupakan suatu prosedur untuk menangani
stenosis atau penyempitan dari arteri koroner, merupakan suatu tindakan intervensi
non bedah untuk membuka kembali pembuluh darah arteri koroner yang menyempit
atau tersumbat dengan memasukkan balon atau stent melalui kateter yang
dimaksukkan ke dalam lumen arteri melalui insisi kecil pada kulit.
Beberapa kemungkinan perkembangan di masa depan masih terus diuji coba,
beberapa contoh misalnya penggunaan stent bioabsorbable yang sedang diuji coba
dan memiliki potensi keberhasilan; ‘triple antiplatelet therapy’ mungkin berhasil
dan bermanfaat; antitrombolitik baru seperti reseptor thrombin (protease-activated
receptor-1 (PAR-1)) antagonis dapat mengurangi thrombosis tanpa mempengaruhi
hemostasis.
