    Incident details : February,11 - April, 11The complex completed 206 Man days free of Lost Time Accident

    (LTA), Last Lost time injury accident was on 07.10.2010Injury Details

    1. First Aid Injury on 17.02.11Olefins Plant Victim was opening a

    metal drum lid for charging chemicals, when he sustained minor

    cut at his right hand index.2. First Aid Injury (Two injuries in one incident)03.03.11in Butene-1 Plant

    Cyclohexane splashed on face on two employees doing pump

    dismantling job. These was irritation to the eyes due contact with

    cyclohexane.3. First Aid Injury (Two injuries in one incident)22.03.11 in PP Warehouse

    Two contractor workers of M/S Expansyl project were sitting on a

    steel platforms resting on a decomposed weak plank when the

    planks suddenly collapsed injury he victims on the legs.

    1. Major fire on 07.02.11 in PE plant-Process grease and

    cyclohexane mixed released from 1 dia nozzles and spread

    on the super surface hot Dowtherm lines damaging instrument

    and electrical cables

    2. Minor fire on 07.02.11 in Olefins plant lube oil spilled in the

    hot insulation of the turbine a very minor flaming fire took

    place.

    3. Minor fire on 03.04.2011 in Bagging Plastic pallets ignited

    due to contact with welding slag.

    Please stop what youredoing. The easiest way to stop an unsafe act is to say Please stop

    the job you are doing. Id like to talk with you for a moment

    or Id like to talk with you for just a moment. Could you turn

    off that machine and come down here?

    Keep your voice calm and your tone friendly. Dont shout or

    sound annoyed. If the employee objects to your interruption,

    simply repeat your request: Pleasestop what you are doingso that we can talk.

    Are you having problems?

    You might ask a question that will help you learn why the

    employee is acting unsafely. In this case, you assume

    not wearing his safety glasses, and you know he is asafe worker, you might ask, Have you been having

    problems with your safety glasses?

    If this happened, you could be hurt.You might say,If you keep the forks of the forklift raised

    above the recommended height while youre moving,

    you could puncture a barrel, Or, worse still, injure

    someone. Thats why we always ask operators to lowerthe forks before moving the forklift.

    Remember to give the information and assume the

    employee will act in accordance with it.

    Do you understand why following the rule isimportant?

    When you use the informing approach, you may want to finish

    by checking to see if the employee understood what you

    said. Checking is specially important when youretalking witha new employee or an employee who is performing a new

    job. Heres what Ive done with your suggestion

    Following up on the safety suggestions you receive is the

    best way to show your commitment to making employees

    jobs as safe as possible.

    Use Correcting feedback to decrease at-risk behavior

    Give the feedback one-on-one and privately. Give the feedback as soon as possible after the

    observation process.

    Begin with acknowledgement of safe behavior(s)

    observed. Identify the at-risk behavior(s) observed. Specify the safe alternative to the at-risk

    behavior(s).
