Conclusiones:
• En pacientes con enfermedad multivaso sometidos a ICP-p es probable que realizar una revascularización completa se asocie a un mejor pronóstico.
• Debe definirse el mejor momento para tratar el resto de lesiones
• ¿Papel de la guía de presión en este contexto?
• Necesitamos un gran ensayo (por tamaño muestral) para clarificar estas cuestiones.

Conclusiones
• En el contexto de la ICPp, la trombectomía de rutina no reduce la muerte CV, el ReIAM ni la ICC/shock cardiogénico a 180 días.
• La trombectomía de rutina se asoció a un incremento del riesgo de ACV.
• Los estudio TOTAL y TASTE alertan de la necesidad de grandes ensayos para comprobar la idoneidad de técnicas realizadas comúnmente y aceptadas como beneficiosas, incluso cuando parecen respaldadas por ensayos más pequeños.

Conclusiones
• En pacientes con SCACEST y sin opción de acceso rápido a un centro con posibilidad para realizar ICP-p, la fibrinolisis es una alternativa valida como terapia de reperfusión, con resultados similares a la ICP-p en pacientes tratados precozmente.
• El estudio STREAM respalda las recomendaciones actuales de contemplar la fibrinolisis como opción terapéutica en pacientes de alto riesgo (IAM anterior) que llegan precozmente al sistema sanitario si no se puede ofrecer ICP < 60 min.

Treatment of patients with DES-ISR remains a challenge
In this setting, DES provide better results than classical coronary interventions.
Drug-eluting balloons (DEB) are also very effective in patients with BMS-ISR and DES-ISR, with results superior to those obtained by conventional BA and equivalent to those seen with 1st Generation DES
However, the value of DEB compared with 2nd Generation DES in patients with DES-ISR remains unknown

Conclusions:
In patients with DES-ISR EES provide superior late angiographic results than DEB (MLD 1ry end-point)
In these patients EES also provide better late clinical results, driven by a significant reduction in the rate of TLR
Treatment of DES-ISR remains challenging and associated with poorer clinical and angiographic results than treatment of BMS-ISR. Further studies (more patients & longer follow-up) are still warranted in this adverse setting.

Conclusiones
En pacientes con SCA, sea SCASEST o SCACEST, sometidos a tratamiento invasivo,
el acceso radial frente al femoral reduce la incidencia de eventos clínicos adversos.
Por lo tanto el acceso radial debería el acceso vascular por defecto en pacientes
afectos de SCA que van a ser sometidos a intervencionismo coronario

Los resultados del estudio PRECOMBAT a largo plazo (5 años), en línea con otros como el SYNTAX, corroboran las recomendaciones de las guías actuales en las que se contempla la ICP como una opción terapéutica para pacientes con enfermedad de TCI y anatomía favorable.
CVLPRIT ICP-p paciente multivaso: Probablemente sea mejor revascularización completa, ¿cuándo? ¿papel de la guía de presión?
TOTAL Trombectomia ICP-p: no de forma sistemática. Nuevos estudios para detectar pacientes que puedan beneficiarse ¿más ACV?
STREAM 1 año fibrinolisis opción válida para pacientes que contactan pronto con el sistema sanitario y con tiempos P-B elevados.
RIBS IV E-DES 2º generación mejor que DEB para reestenosis de DES.
MATRIX radial vs femoral SCA en SCA, acceso radial mejor que femoral.
PRECOMBAT 5 años ICP no inferior a CABG en enfermedad de TCI con S-DES.
