Page Numbering - Page numbers appear in the upper right corner of the page. Pages are numberedsequentially, however, new pages are occasionally added to the tariff. When a new page is added betweenexisting pages with whole numbers, a decimal is added. For example, a new page added between pages 34and 35 would be page 34.1.
Page Revision Numbering - Revision numbers also appear in the upper right corner of the page. Thesenumbers are used to determine the most current page version on file with the Commission. For example, 4th
Revised Page 34 cancels 3rd Revised Page 34. Consult the check sheet for the page currently in effect.
Check Sheet - When a tariff is filed with the Commission, an updated check sheet accompanies the tarifffiling. The check sheet lists the tariff pages, with a cross reference to the current revision number. When newsheets are added, the check sheet is changed to reflect the revision. All revisions made in a given filing aredesignated by an asterisk (*). There will be no other symbols used on this sheet if these are the only changesmade. 1.1 Definitions of Terms
Access Line - A central office line which provides access to the telephone network for local and longdistance telephone services.
Air Line Measurement - The shortest distance between two points.
Ancillary Devices - All terminal equipment except telephone instruments, PBX-PABX systems, keysystems and data services.
Answering Equipment - Equipment that will automatically answer incoming calls and make anannouncement. It may also be equipped to record messages.
Applicant - Any person, partnership, cooperative corporation, corporation, or any combinationthereof requesting affirmative service or action from the Company.
Authorized User - A person, firm or corporation (other than the customer) who has been authorizedby the Company to be connected to the service of the Company under the terms of the tariff.
Business Customer - A Business Customer is a Customer who subscribes to the Company’sServices(s) and whose primary use of the Service is of a business, professional, institutional orotherwise occupational nature.

Business Service - Telecommunications service furnished to customers where the primary or obvioususe is of a business, professional or occupational nature.
Call - An attempted communication.
Calling Area - See Local Service Area.
Cancellation Charges - A charge applicable under certain conditions when application for serviceand/or facilities is canceled in whole or in part prior to the completion of the work involved or beforecontract period is completed.
Central Office - A switching unit in a telecommunications system which provides service to thegeneral public, having the necessary equipment and operating arrangements for the terminating andinterconnecting of exchange lines and trunks or trunks only. There may be more than one centraloffice in a building or exchange.
Commission - Michigan Public Service Commission
Company - Comtel Telcom Assets LP d/b/a Excel Telecommunications.
Customer Trouble Report - Any oral or written report from a customer or user oftelecommunications service received by the Company relating to a physical defect or to difficultyor dissatisfaction with the service provided by the Company’s facilities.
Delinquent Bills - Outstanding account balances which are not paid by the due date.
Demarcation Point - The point of interconnection between Company communications facilities andthe terminal equipment, protective apparatus or wiring at a subscriber’s premises. The DemarcationPoint is located on the customer’s side of the Company’s protector or equivalent, where a protectoris not used, and consists of a modular jack or equivalent.

Direct Connection - Connection of terminal equipment to the Company’s exchange facilities bymeans other than acoustic and/or inductive coupling.
Directory - A book which alphabetically lists each telephone customer with his/her address andtelephone number.
Directory Assistance Service - A service provided to assist customers in obtaining telephonenumbers which are or are not listed in the directory.
Directory Assistance Call Completion - A service provided that gives customers the option of havingtheir local or intraLATA calls automatically completed when they request a telephone listing fromthe Directory Assistance (DA) operator.
Directory Listing - The publication of the Company’s directory and/or directory assistance records,of information relative to a customer’s telephone number, by which telephone users are able toascertain the call number of a desired station.
Disconnection Notice - The written notice sent to a customer following billing, notifying thecustomer that service will be disconnected if charges are not satisfied by the date specified on thenotice.
Disconnection of Service - An arrangement made at the request of the customer or initiated by theCompany for violation of tariff regulations by the customer, for a permanent interruption oftelephone service. A “final” bill would be rendered showing moneys owed to the Company as of thedate the service was disconnected.
End User - Any person, firm, corporation, partnership or other entity which uses the services of theCompany under the provisions and regulations of this tariff. The End User is responsible forpayment unless the charges for the service utilized are accepted and paid by another Customer.

Exchange - One or more contiguous central offices and all associated facilities within a geographicalarea in which local exchange telecommunication services are offered by a provider.
Exchange Line - Any circuit connecting an exchange access line with a central office.
Exchange Service - Exchange service is a general term describing as a whole, the facilities for localintercommunications, together with the capability to send and receive a specified or an unlimitednumber of local messages at charges in accordance with the provisions of this tariff.
(a) Flat Rate Service: A classification of exchange service furnished a customer under tariffprovisions, for which a stipulated charge is made, regardless of the amount of use.
(b) Individual Line Service: A classification of exchange service which provides that only oneexchange access line shall be served by the circuit connected.
Exchange Service Area - The area within which the Company furnishes complete local telephoneservice at the applicable exchange rates for that area.
Extended Area Service - A type of telephone service furnished under tariff provisions wherebycustomers of a given exchange may complete calls to and/or may receive calls from one or moreexchanges without the application of long distance message telecommunications charges.

Facilities - All the plant and equipment of the Company and all instrumentalities owned, licensed,used, controlled, furnished, or supplied for or by the Company, including any construction work inprogress allowed by the Commission.
Flat-Rate Service - A classification of exchange service furnished a customer under tariff provisionsfor which a stipulated charge is made regardless of the amount of use.
Foreign Exchange Service - Exchange Service furnished under tariff provisions by means of a circuitconnecting a customer’s premises with a central office of an exchange other than that whichregularly serves the exchange area in which the customer is located.
General Exchange Services - Services furnished by the Company connected to or associated withprimary Local Exchange Service.
Harm - Electrical hazards to Company personnel, damage to Company equipment, malfunctions ofCompany billing equipment, and degradation of service to persons other than the user as well as thecalling or called party.

Household - A Household comprises all persons who occupy a dwelling unit, that is, a house, anapartment or other group of rooms or a room that constitutes separate living quarters. A Householdincludes the related persons (the head of the household and others in the dwelling unit who arerelated to the head) and also the lodgers and employees, if any, who regularly live in the house. Aperson living alone or a group of unrelated persons sharing the same dwelling unit as partners iscounted as a Household.
Individual Line Service - A classification of exchange service furnished under tariff provisionswhich provides that only one exchange access line shall be served by the circuit connected.
Initial Non-Recurring Charge - A nonrecurring charge made for the furnishing of telephone services,which may apply in addition to service connection charges.
Initial Service Period - The minimum period of time for which service is provided.
Installation Charge - A nonrecurring charge associated with optional service features and maysometimes be called an “initial” charge, and may apply in addition to Service Connection Charges.
Intercept Service - A service arrangement provided by the Company whereby calls placed to adisconnected or discontinued telephone number are intercepted and the calling party is informed byan operator or by a recording that the called telephone number has been disconnected, discontinued,changed to another number, or that calls are received by another telephone.

Interface - (a) The junction or point of interconnection between two systems or equipments havingdifferent characteristics which may differ with respect to voltage, frequency, speed of operation, typeof signal and/or type of information coding including the connection of other than Company-provided facilities to exchange facilities provided by the Company.
(b) The point of interconnection between Company equipment and communications facilitieson the premises of the Customer. Also referred to as Demarcation Point.
Interface Equipment - Equipment provided by the Company at the interface location to accomplishthe direct connection of facilities provided by the Company with facilities provided by other thanthe Company.
IntraLATA - Long Distance Message Telecommunications Service where service point locations arewithin the same Local Access and Transport Area.
Local Access and Transport Area (LATA) - Denotes a geographic area established for theadministration of telecommunications service. It encompasses designated local operating telephonecompany exchanges which are grouped to serve common, social, economic and miscellaneouspurposes.
Local Calling Area - See Local Service Area.
Local Channel - Applies to that portion of a channel which connects a station to the interexchangechannel or to a channel connecting two or more exchange access lines within an exchange area.

Local Exchange Service - Provides a Customer with a telephonic connection to, and a uniquetelephone number on, the Company=s switching network which enables the Customer to: receivecalls from other stations on the public switched telephone network, access the Company=s LocalCalling Services and other Services as set forth in this tariff, access interexchange calling servicesof the Company and of other carriers, access (at no additional charge) to the Company=s operatorsand business office for service related assistance, access toll-free telecommunications services suchas 800 NPA; and access toll-free emergency services by dialing 0 or 9-1-1 (where available), andaccess relay services for the hearing and/or speech impaired.
Local Message - A communication between two access lines within the local service area of thecalling telephone.
Local Message Charge - The charge that applies for a completed message that is made when thecalling access line and the called access line are both within the same local calling area where a localmessage charge is applicable.
Local Service Area (Local Calling Area) - The area within which telephone service is furnishedcustomers under a specific schedule of exchange rates (flat or measured) and without toll charges.A local service area may include one or more exchange areas under extended area servicearrangement.
Local Service Charge - The charge for furnishing facilities to enable a customer to send or receivetelecommunications within the local service area. This local service calling area may include oneor more exchange areas.
Maintenance Service Charge - A charge made by the Company when a service difficulty or troublereport results from customer-provided equipment or facilities.

Message - A communication between two exchange access lines. Messages may be classified aslocal or toll.
Mileage - The measurement (airline, route, etc.,) upon which a charge for the use of part or all of acircuit furnished by the Company is based.
Minimum Contract Period - The minimum length of time for which a customer is obligated to payfor service, facilities and equipment, whether or not retained by the customer for such minimumlength of time.
Network Control Signaling - The transmission of signals used in the telecommunications systemwhich perform functions such as supervision (control, status, and charging signals), address signaling(e.g., dialing), calling and called number identification and audible tone signals (call progress signalsindicating reorder or busy conditions, alerting coin denominations, coin collect and coin returntones) to control the operation of switching machines in the telecommunications system.
Network Control Signaling Unit - The terminal equipment furnished, installed and maintained bythe Company for the provisions of network control signaling.
Network Interface - See Interface.
Non-Listed Telephone Number - A telephone number associated with an exchange line which, at therequest of the customer, is not listed in a telephone directory but may be obtained from the DirectoryAssistance Operator.
Non-Published Telephone Number - A telephone number associated with an exchange access linewhich, at the request of the customer, is not listed in a telephone directory and is not made availableto the general public by the Company.
Non-Recurring Charge - A one-time charge associated with certain installations, changes or transfersof services, either in lieu of or in addition to recurring monthly charges.

One-Party Service - Any exchange access line designed for the provision of exchange service to onepremises.
Other Common Carrier (OCC) - Denotes Specialized Common Carriers, Domestic and InternationalRecord Carriers and Domestic Satellite Carriers engaged in providing services authorized by theFederal Communications Commission.
Other Common Carrier Terminal Location - A discrete operational and equipment location of theOCC from which the OCC furnishes and administers common carrier communications services toits patrons.
Permanent Disconnect - A discontinuance of service in which the facilities used in the service areimmediately made available for use of another service.
Person - Includes individuals, partnerships, corporations, governmental bodies, associations and anyother such entity.
Premises - Building or buildings on contiguous property (except railroad rights-of-way, etc.) notseparated by a public highway.
Preassigned Number - A telephone number preassigned before service is actually established.

Presubscription - Presubscription is the process by which end user customers may select anddesignate to the Telephone Company an Interexchange Carrier (IC) to access, without an accesscode, for interLATA/interstate or interLATA/intrastate calls. This IC is referred to as the end user’spredesignated IC.
Primary Termination - Applied to channels which extend beyond the continuous property of acustomer or the confines of a single building housing the premises of more than one customer. Also,denotes the first termination of such a channel at a station or PBX on the continuous property of acustomer. When more than one customer’s premises is located within the same building, the firsttermination of such a channel at that building constitutes a “Primary Termination.” For purpose ofthis definition, the location of a “Primary Termination” for channel services associated with“Switching System Services” is considered to be at the “Switching System Services” serving centraloffice. When the “Switching System Services” serving central office is not in the same exchange asthe main location, the “Mileage Service Area” center for the main location will be used in lieu of the“Switching System Services” serving central office.
Published Telephone Number - A number which appears in the current telephone directory, or isscheduled to appear in a forthcoming telephone directory, and which also appears in the informationrecords for general public information.
Rate Center - A specified geographical location within an exchange area from which mileagemeasurements are determined for the application of rates between exchange areas.
Reference Listing - The listing of a generally accepted name of a firm or corporation followed bya reference to another listing.

1.1 Definitions of Terms (Continued)
Residential Customer - A Residential Customer is a person to whom telecommunications servicesare furnished by the Company predominantly for personal or domestic purposes at the person’sdwelling.
Residence Exchange Access Line - An exchange access line used to provide exchange telephoneservice to a residence customer.
Residential Service - Telecommunications service furnished to customers when the actual or obvioususe is for domestic purposes at the customer’s dwelling.
Service Call - A visit to a customer’s premises in connection with a service difficulty. See alsoMaintenance Service Charge.
Service Charges - A nonrecurring charge applying to the provisions of telephone service.
Subscriber - Any person, firm, partnership, corporation, municipality, cooperative organization orgovernmental agency furnished communication service by the Company under the provisions andregulations of this tariff and responsible for the payment of charges and compliance with the rulesand regulations of the Company.
Supplemental Contract - A contract for service, equipment or facilities in addition to that providedfor under the original contract.
Suspension of Service - An arrangement made at the request of the customer or initiated by theCompany, for temporarily interrupting service.

Tariff - The schedule of the Company containing all regulated rates, and charges stated separatelyby type or kind of service and the customer class, and the rules and regulations of the Companystated separately by type or kind of service and the customer class as filed with the Commission.
Telecommunications Services - Includes regulated and unregulated services offered to customers forthe transmission of 2-way interactive communication and associated usage.
Telephone Number - A numerical designation assigned to a customer for convenience in operationand identification. The telephone numbers include the number prefix of a central office, which istermed “Central Office Designation”.
Telephone or Telecommunications Network - The local telephone exchange and long distancemessage telecommunications facilities, or network; both inter and intrastate.
Temporary Disconnection - See Suspension of Service.
Temporary Service - The provision of service definitely known to be required for a short period oftime (generally less then twelve consecutive months) such as service furnished to buildingcontractors, service to a convention, and service for seasonal business including resorts.
Termination Agreement - An agreement between the Company and the customer to provide orfurnish certain lines or equipment representing a comparatively high investment or in lieu of acontribution to construction for temporary service whereby the customer agrees to compensate theCompany in case the service is discontinued prior to the date specified in the agreement.

Termination Charge - A charge made to liquidate a customer’s obligation for termination of serviceprior to the expiration of the initial contract period.
Termination of Service - The discontinuance of service or facilities provided by the Company, eitherat the request of the customer or by the Company, under its regulations concerning cancellation forcause.
Tone Dialing Service - A classification of exchange service whereby calls are originated through theuse of tone dial instruments in lieu of a rotary dial instrument.
Trunk - A telephone communication path connecting a central office and customer premisesequipment, used in the establishment of end-to-end service.
Unbundled Network Element - Physical and functional elements of the network that can becombined into a complete set in order to provide end-to-end circuits.

SECTION 2.0 - RULES AND REGULATIONS
2.1 Application of Tariff
The rules and regulations contained in this Section apply to the services and facilities provided bythe Company in the State of Michigan. When services and facilities are provided in part by theCompany and in part by other connecting companies, the regulations of the Company apply to theportion of the services or facilities furnished by the Company. Failure on the part of the customerto observe these rules and regulations after due notice of such failure, automatically gives theCompany the right to discontinue service to that customer.
a. In the event of a conflict between any rate, rule, regulation or provision contained in thistariff and any rate, rule, regulation or provision contained in the tariffs of Companies inwhich the Company concurs, the rate, rule, regulation or provision contained in this tariffshall prevail.
b. Should there be any conflict between this tariff and the Rules and Regulations of theCommission, the Commission’s rules shall apply unless otherwise established by the courts.
2.2 Availability of Facilities
2.2.1 General
The Company’s obligation to furnish, or continue to furnish, telecommunications serviceis dependent upon its ability to obtain, retain, and maintain, without unreasonable expense,suitable rights-of-way and facilities, and to provide for the installation of those facilitiesrequired to furnish and maintain that service.

2.2.2 Local Calling Scopes and Availability
The Company concurs with the Incumbent Local Exchange Carrier(s) tariff for the provisionof local exchange service in the State of Michigan. Exchange Areas for all Customers whosepremises are located in the ILEC’s territory will be the same as the ILEC’s service areas asdefined in Section 9.0 of this tariff except where noted by service or rate element.
2.2.3 Maps
The Company concurs with the Incumbent Local Exchange Carrier(s) local exchange mapsreflecting base rate areas, exchange area, and zone area boundaries which are part of theirlocal exchange tariff(s).
2.3 Customer Service and Protection
This part of the Rules and Regulations Section of the tariff provides information to telephoneconsumers about: resolving disputes with the Company, applying for service, the classification ofbusiness and residential rates, deposits, billing, refusal of service, disconnection of service andcancellation of service. The Company relies on principles established by the Commission.
2.3.1 Resolution of Disputes
Any customer or applicant for service who wishes to dispute any action or determination ofthe Company under the Company’s customer service rules shall be given an opportunity fora supervisory review by the Company. If the Company and the Residential Customer areunable to informally resolve the dispute, the customer may file a formal complaint with theMichigan Public Service Commission.

2.3.2 Application for Service
Application for service, or requests from customers for additional service or changes in thegrade or class of service, become contracts when received by the Company and are subjectto the minimum contract term of one month unless specified otherwise. The Companyreserves the right to require application for service to be made in writing on forms suppliedby it.
2.3.3 Refusal of Service
The Company may refuse service to a customer or applicant whose service has beendiscontinued for non-payment of bills for any regulated telecommunications service. Thecustomer or applicant will be required to pay all bills due the Company fortelecommunications service or make other arrangements satisfactory to the Company to re-establish credit before service is restored or service started.
If service is established and it is subsequently determined that the customer or applicant isindebted to the Company for service previously furnished, the Company may suspend orterminate such service until satisfactory arrangements have been made for the payment ofprior indebtedness.

2.3.4 Application of Business Or Residence Rates
2.3.4.1 Business rates apply at the following locations:
a. in offices, stores, factories, and all other places of a strictlybusiness nature;
b. in boarding houses (except as noted below), offices of hotels, hallsand offices of apartment buildings, quarters occupied by clubs, orlodges, public, private or parochial schools, colleges, hospitals,libraries, churches, and other similar institutions, except inchurches and lodges as specified below;
c. at residence locations when the customer has no regular businesstelephone, and the use of the service either by himself, members ofhis household or his guests, or parties calling him can beconsidered as more of a business than a residential nature. This factmight be indicated by some form of Customer advertising, or whensuch business use is not typically characteristic of residentialtelephone usage;
d. at residential locations where the service is located in a shop,office or other place of business;
e. in college fraternity houses where the members lodge within thehouse; and/or
f. at any location where the location and expected usage of telephoneservice at that location are indicative of a business, trade orprofession.

2.3.4 Application of Business Or Residence Rates
2.3.4.2 Residence rates apply at the following locations:
a. in a private residence where business listings are not provided;
b. in private apartments of hotels, rooming houses or boarding houses whereservice is confined to the customer’s use and which are not advertised asa place of business;
c. in the place of residence of a clergyman, physician, nurse or other medicalpractitioner, provided the office is located in their residence and is not apart of an office building. In any such cases, the listing may indicate thecustomer’s profession, but only in connection with an individual name. Iflistings of firms or partnerships, etc., or additional listings of persons notresiding in the same household are desired, business rates apply;
d. in a private stable or garage when strictly a part of a domesticestablishment;
e. in churches where the service is not accessible for public use as in aclergyman’s study; and/or
f. in fire department dormitories or sleeping quarters where the telephonenumber is not published, and the telephone is not available for use by thegeneral public.

2.3.5 Deposits
a. To safeguard its interests, the Company may require a Customer to make a depositto be held as a guarantee for the payment of charges, except as stated below. Adeposit does not relieve the Customer of the responsibility for the prompt paymentof bills on presentation. The deposit will not exceed $150.00 per access line.
b. A deposit may be required in addition to an advance payment.
c. When a service or facility is discontinued, the amount of a deposit, if any, will beapplied to the Customer’s account, and any credit balance remaining will berefunded. Before the service or facility is discontinued, the Company may, at itsoption, return the deposit or credit it to the Customer’s account.
d. No deposit will be charged for a Lifeline customer that voluntarily elects to receivetoll blocking service.
e. The Company will not require a cash deposit or other guarantee as a condition ofobtaining basic local exchange service, unless the prospective customer refuses toproduce identification that can be readily and inexpensively verified or if theprospective customer has a history of payment default within the past 60 months fortelecommunications services.
f. The Company is not obligated to provide service to a household under any name ifan outstanding bill exists at the address and the person responsible for that bill stillresides at the address.

2.3.6 Billing and Payment For Service
2.3.6.1 General
The customer is responsible for payment of all charges for services furnished,including charges for service originated or accepted at the customer’s access line.
Monthly rates for any service provided under this Tariff will apply for each month,or partial month. When a Customer cancels service or when service is discontinuedby the Company during a billing cycle, new charges will not be assessed for billingperiods subsequent to the discontinuance of service.
2.3.6.2 Disputed Bills
In the event of a dispute between a customer and the Company over any bill forutility service, the Company shall make an appropriate investigation and report theresults to the customer.
a. The customer shall pay all charges for monthly service in advance. Paymentis due on or before the due date which is not less than seventeen (17) daysafter issuance of the bill. The postmark, if any, on the envelope of the bill,or the issuance date on the bill, if there is no postmark on the envelope,shall constitute proof of the date of issuance. If the bill is not paid by thedue date, the Company will notify the customer in writing that the accountis delinquent.
b. Any applicant or one for whom an application is made, owing the Companyfor service furnished under a former contract, shall pay or make satisfactoryarrangements for paying any bill outstanding and unpaid before anyadditional service will be furnished.

2.3.6.3 Adjustments of Charges for Service Interruptions
In the event a customer’s service is interrupted other than by the negligence orwillful act of the customer and remains out of order for twenty-four (24) hours orlonger after access to the premises is made available and after the customer reportsto the Company that the service is out of order, the Company may make anappropriate adjustment to the customer’s bill.
2.3.6.4 Late Payment Charge
If any portion of a Customer’s payment is not received by the Company twobusiness days prior to the next billing cycle, or if any portion of the payment isreceived by the Company in funds which are not immediately available uponpresentment, a late payment charge shall be due to the Company. The late paymentcharge shall be the portion of the payment not received two business days prior tothe next billing cycle, multiplied by 1.5%. For unpaid balances of $10.00 or more,a minimum late payment charge of $5.00 shall apply.
Late payment charges do not apply to the disputed portion of unpaid balances, ifresolved in favor of the Customer. Any disputed portion of unpaid balances, ifresolved in favor of the Company, may be subject to the late payment charge as ofthe original due date noted on the Customer’s bill. Undisputed amounts of the samebill may be subject to the late payment charge if they remain unpaid by the due datenoted on the Customer’s bill.

2.3.7 Discontinuance of Service
2.3.7.1 Discontinuance with notice
Proper notice consists of a separate mailing or hand delivery at least five (5) daysprior to the stated date of disconnection. Telephone service may be disconnectedafter proper notice for any of the following reasons:
a. Nonpayment of a delinquent account for basic local exchange service;
b. Nonpayment of a delinquent account with a delinquent balance of $150.00or more for basic local exchange service and regulated toll service in thename of the Customer;
c. Maintaining a delinquent balance of $125.00 or more for three consecutivemonths for basic local exchange service and regulated toll service;
d. Unauthorized tampering or interference with facilities and equipmentowned by the Company that are situated on or about the Customer’spremises;
e. Refusal to grant access at reasonable times to equipment installed upon thepremises of the Customer for the purpose of inspection, maintenance orreplacement;
f. Misrepresentation of the Customer’s identity for the purpose of obtainingbasic local exchange service;

g. A violation of a tariff provision of the Company that is on file with orapproved by the Commission that adversely affects the safety of theCustomer or other persons or the integrity of the Company’s basic localexchange system;
h. Any other unauthorized use or interference with basic local exchangeservice, including improper use or interference with basic local exchangeservice, including improper use of a party line service by denying otherCustomers on the line an equitable proportionate use of the service.
Customers having their local service terminated by the Company will benotified by the Company in accordance with the applicable rules andregulations of the Commission regarding termination of service.

2.3.7.2 Collection costs
The customer is responsible for all collection costs, including attorney’s fees,incurred by the Company as a result of any breach of contract by the customer.
2.3.7.3 Disputes
In the event of a dispute between a customer and the Company, the Company shallnot disconnect service pending completion of a supervisory review of the disputeas provided in Section 2.3.1., above.
2.3.8 Cancellation of Application For Basic Service
When the customer cancels an application for service prior to the start of installation ofservice, or prior to the start of special construction, no charge applies.
When installation of service has been started prior to the cancellation, a cancellation chargeequal to the minimum service charge shall apply.
Where special construction has been started prior to the cancellation, a charge equal to thecost incurred in the special construction, less net salvage, applies. Installation or specialconstruction for a customer is considered to have started when the Company incurs anyexpense in connection with the installation which would not otherwise have been incurred.

2.3.9 Restoration of Service
If a Customer’s service is restored after having been suspended and/or disconnected inaccordance with this tariff but a Company service order to terminate such service has notbeen completed when such service is restored, the Customer may be required to pay arestoration of service charge.
When a Customer’s service has been disconnected in accordance with this tariff and theservice has been terminated through the completion of a Company service order, service willbe re-established only upon the basis of an application for new service.
Service initiated after the discontinuance for non-payment will be subject to the standardnon-recurring charges for new service as defined in Sections 3 and 4 of this tariff. Servicethat has been re-established may require the assignment of a new telephone number.
If any Company service has been suspended, restricted, or discontinued for nonpayment,service will be re-established upon receipt of all charges due, which includes charges forservices and facilities during the period of restriction or suspension and which may includea service restoration fee. If the Customer has a history of payments returned for insufficientfunds, the Company may require payment by cash, money order or certified check. If suchpayments are made by personal check, restoration of service will be effected upon bankclearance of the check.

If service is established and it is subsequently determined that the Customer is indebted tothe Company for service previously furnished, the Company may restrict, suspend orterminate such service until satisfactory arrangements have been made for the payment ofthe prior indebtedness.
As Customer who has service restricted, suspended, or disconnected may be subject to allstandard non-recurring charges defined in Sections 3 and 4 of this tariff during therestoration of service.

2.4 Use of Service and Facilities
2.4.1 Use of Service
2.4.1.1 General - The customer is responsible for payment of all charges incurred,regardless of whether such charges are associated with his usage, or that of any ofhis authorized users.
2.4.1.2 Transmitting Messages - The Company does not transmit messages but offers theuse of its facilities for communications between its customers.
2.4.1.3 Use of Lines of Other Companies - When suitable arrangements can be made, linesof other telephone companies may be used in establishing connections to points notreached by the Company’s lines. In establishing connections with the lines of othercompanies, the Company is not responsible or liable for any action of theconnecting company.
2.4.1.4 Unauthorized Attachments or Connections - No unauthorized equipment, accessory,apparatus, circuit or device shall be attached to or connected with Companyfacilities. In case any such unauthorized attachment or connection is made, theCompany shall have the right to remove or disconnect the same, to suspend serviceduring the continuance of said attachment or connection or to disconnect service.The customer shall be held responsible for the cost of correcting any impairment ofservice caused by the use of such attachments or connections and shall be billed foreach service call made to his premises because of the use of such attachments orconnections.
Except as otherwise provided in this tariff, nothing herein shall be construed topermit the use of a device to interconnect any Company-owned line or channel withany other communications line or channel of the Company or of any other person.

2.4.2 Obligations of Customers
2.4.2.1 Alterations - The customer agrees to notify the Company promptly in writingwhenever alterations or new construction on premises owned or leased by him willnecessitate changes in the Company’s facilities, and the customer agrees to pay theCompany’s current costs for such changes.
2.4.2.2 Use of Commercial Power - Unless specifically provided otherwise in this tariff,when commercial power is used for the operation of customer provided premisesequipment, the customer provides the necessary power wiring, power outlets andcommercial power and assumes all responsibility for the safe condition of the powerwiring, power outlets and commercial power.
2.4.2.3 Reimbursement to the Company for damages - The customer shall reimburse theCompany for damages to Company facilities utilized to provide services under thistariff caused by the negligence or willful act of the customer, or resulting from thecustomer’s improper use of the Company facilities or equipment provided by otherthan the Company. Such damages will be the actual cost of the materials and theactual hours required for repair of the damage multiplied by the appropriate laborrate.
Nothing in the foregoing provision shall be interpreted to hold one customer liablefor another customer’s actions. The Company will, upon reimbursement fordamages, cooperate with the customer in prosecuting a claim against the personcausing such damage and the customer shall be subrogated to the right of recoveryby the Company for the damages to the extent of such payment.

2.4.3 Rights of the Company
2.4.3.1 Work Performed on an Overtime Basis - The charges specified in this tariff do notcontemplate work being performed by Company employees at a time when overtimewages apply due to the request of the customer. If the customer requests thatovertime labor be performed, an additional charge, based on the additional costsinvolved, applies.
2.4.3.2 Work Interruption - The charges specified in this tariff do not contemplate workonce begun being interrupted by the customer. If the customer interrupts work oncebegun, an additional charge, based on the additional costs involved, applies.
2.4.3.3 Ownership - Any facilities on the customer’s premises furnished by the Companyshall be and remain the property of the Company, whose agents and employees havethe right to enter said premises at any reasonable hour for the purpose of installing,terminating, inspecting, maintaining or repairing service.
2.4.3.4 Impairment of Telephone Service - When the general telephone service to the publicis impaired by a customer’s use of telephone service, or service directly incidentthereto, the Company shall have the right to require the customer to contract withthe Company for such additional facilities as may be necessary in the Company’sjudgment to remove the cause of said impairment, or if the customer refuses thisrequirement or will not remedy the situation, to discontinue service completely.

2.4.3.5 Abandonment of Telephone Service - The Company may discontinue service whichhas been abandoned.
2.4.3.6 Abuse of Telephone Service - After giving the customer a reasonable opportunityto remedy the situation and after giving proper notice, the Company maydiscontinue service which is used:
a. in such a way that interferes with the service of other telephone users;
b. for any purpose other than as a means of communication;
c. for a call or calls, anonymous or otherwise, if made in a manner whichreasonably could be expected to frighten, abuse, torment or harass another;
d. in any fraudulent or unlawful manner;
e. in manner which violates any of the Company’s lawful regulations; or
2.4.3.7 Telephone Numbers - The Company reserves the right to change the telephonenumber or numbers assigned to a customer or the central office designationassociated with such telephone number or numbers, or both, as reasonablyappropriate in the conduct of its business. The customer has no property right in anynumber or central office designation assigned by the Company.

2.4.4 Liability of the Company
2.4.4.1 The liability of the Company for damages arising out of the furnishing of itsServices, including but not limited to mistakes, omissions, interruptions, delays, orerrors, or other defects, representations, or use of these services or arising out of thefailure to furnish the service, whether caused by acts or omission, shall be limitedto the extension of allowances for interruption as set forth in 2.3.6.3 The extensionof such allowances for interruption shall be the sole remedy of the Customer andthe sole liability of the Company. The Company will not be liable for any direct,indirect, incidental, special, consequential, exemplary or punitive damages toCustomer as a result of any Company service, equipment or facilities, or the acts oromissions or negligence of the Company's employees or agents.
2.4.4.2 The Company shall not be liable for any delay or failure of performance orequipment due to causes beyond its control, including but not limited to: acts ofGod, fire, flood, explosion or other catastrophes; any law, order, regulation,direction, action, or request of the United States Government or any state and localgovernments having or claiming jurisdiction over the Company, or of anydepartment, agency, commission, bureau, corporation, or other instrumentality ofany one or more of these federal, state, or local governments, or of any civil ormilitary authority; national emergencies; insurrections; riots; wars; unavailabilityof rights-of-way or materials; or strikes, lock-outs, work stoppages, or other labordifficulties when it does not involve the company=s employees.
2.4.4.3 The Company shall not be liable for any act or omission of any entity furnishing tothe Company or to the Company's Customers facilities or equipment used for orwith the services the Company offers.

2.4.4.4 The Company shall not be liable for any damages or losses due to the fault ornegligence of the Customer or due to the failure or malfunction of Customer-provided equipment or facilities.
2.4.4.5 Explosive Atmosphere. The Company does not guarantee nor make any warrantywith respect to installations it provides for use in an explosive atmosphere. TheCustomer indemnifies and holds the Company harmless from any and all loss,claims, demands, suits, or other action, or any liability whatsoever, whethersuffered, made, instituted, or asserted by any other party or person(s), and for anyloss, damage, or destruction of any property, whether owned by the Customer orothers, caused or claimed to have been caused directly or indirectly by theinstallation, operation, failure to operate, maintenance, removal presence, condition,location, or use of any installation so provided. The Company reserves the right torequire each Business Customer to sign an agreement acknowledging acceptanceof the provisions of this section (2.4.4.5) as a condition precedent to suchinstallations.
2.4.4.6 The Company is not liable for any defacement of or damage to Customer premisesresulting from the furnishing of services or equipment on such premises or theinstallation or removal thereof, unless such defacement or damage is caused bynegligence or willful misconduct of the Company's agents or employees.

2.5 Undertaking of the Company
2.5.1 General
The Company undertakes to provide the services offered in this tariff on the terms andconditions and at the rates and charges specified. Local Exchange Service consists of theprovision of a telephonic connection to, and a unique telephone number on, the Company=sswitching network which enables the Customer to: receive calls from other stations on thepublic switched telephone network, access the Company=s Local Calling Services and otherServices as set forth in this tariff, access interexchange calling services of the Company andof other carriers, access (at no additional charge) to the Company=s operators and businessoffice for service related assistance, access toll-free telecommunications services such as800 NPA; and access toll-free emergency services by dialing 0 or 9-1-1 (where available),and access relay services for the hearing and/or speech impaired.
Service is provided on the basis of a minimum period of at least one month, 24-hours perday. For the purpose of computing adjustments or prorating charges in this tariff, a monthis considered to have 30 days.
Service will be provided where facilities, billing capability and the resale of total localservice or underlying network elements are available, or a combination thereof.
The Company will provide to its customers quality of service at a level at least equivalentto the service provided to them, or through them, by the incumbent local exchange company.
The Company’s obligation to furnish service, features and/or facilities is also dependentupon its ability to provide, secure and retain, without unreasonable expense to the Company(a) suitable facilities and rights for the construction and maintenance of the necessarycircuits and equipment, (b) suitable space for its plant and facilities in the building whereservice is or is to be provided, (c) facilities for interconnection from alternate suppliers.
In the event of a dispute, the non-prevailing party may be liable for reasonable court costsand attorney’s fees.

2.5.2 Terms and Conditions
Customers may be required to enter into written service orders which shall contain orreference a specific description of the service ordered, the rates to be charged, the durationof the services, and the terms and conditions in this tariff. The Customer may also berequired to execute any other documents as may be reasonably requested by the Company.The Company reserves the right, at its discretion, to change the terms and conditionsspecified in this tariff, including, but not limited to, the rates for services. Any changes tothe terms and conditions will be applicable to services provided after the effective date ofthe change.
At the expiration of the initial term specified in each service order, or in any extensionthereof, service shall continue on a month to month basis at the then current rates. Anytermination shall not relieve the Customer of its obligation to pay any charges incurredunder the service order and this tariff prior to termination. The rights and obligations, whichby their nature extend beyond the termination of the term of the service order shall survivesuch termination.
2.5.3 Interconnection of Facilities
Any special interface equipment necessary to achieve compatibility between the facilitiesand equipment of the Company used for furnishing Local Exchange Services and thechannels, facilities or equipment of others shall be provided at the Customer’s expense.
Local Exchange Services may be connected to the services or facilities of othercommunications carriers only when authorized by and in accordance with the terms andconditions of the tariffs of the other communications carriers which are applicable to suchconnections.

2.5.4 Inspections
Upon reasonable notification to the Customer, and at a reasonable time, the Company maymake such tests and inspections as may be necessary to determine that the Customer iscomplying with the requirements set forth in the section for the installation, operation andmaintenance of Customer-provided facilities, equipment and wiring in the connection ofCustomer-provided facilities and equipment to Company-provided facilities and equipment.
If the protective requirements for Customer-provided equipment are not being compliedwith, the Company may take such action as it deems necessary to protect its facilities,equipment and personnel. The Company will notify the Customer promptly if there is anyneed for further corrective action. Within ten days of receiving this notice, the Customermust take corrective action and notify the Company of the action taken. If the Customer failsto do this, the Company may take whatever additional action deemed necessary, includingthe suspension of or disconnection of service, to protect its facilities, equipment andpersonnel from harm.
2.5.5 Assignment or Transfer of Service
The Customer may not assign or transfer its rights or duties in connection with the servicesand equipment or facilities provided by the Company without the written consent of theCompany. The Company may assign its rights and duties (a) to any subsidiary, parentcompany or affiliate of the Company, (b) pursuant to any sale or transfer of substantially allthe assets of the Company, or (c) pursuant to any financing, merger or reorganization of theCompany.

2.5.6 Notices and Communications
All notices or other communications required to be given pursuant to this tariff will be inwriting. Notices and other communications of either party, and all bills mailed by theCompany, shall be presumed to have been delivered to the other party on the third businessday following placement of the notice, communication, or bill with the U.S. Mail or aprivate delivery service, prepaid and properly addressed, or when actually received orrefused by the addressee, whichever occurs first.
The Company or the Customer shall advise the other party of any changes to the addressesdesignated for notices, other communications or billing.
2.5.7 Provisions of Certain Local Taxes and Fees
Any assessments, franchise fees, privilege, license, occupation, excise, or other similartaxes, whether in a lump sum or at a flat rate, or based on receipts, or based on poles, wireor other utility property units, imposed upon the Company by any governmental authorityshall be added pro rata, insofar as practical, to the rates and charges stated in the Company’sstandard schedules, in amounts which in the aggregate for the Company’s Customers of anypolitical entity shall be equal to the amount of any such tax upon the Company. Companyshall, so long as any such tax or fee is in effect, add to the bills of the Customers in suchpolitical entity pro rata on the basis of the revenue or as otherwise required and/or allowedby law derived by Company from each such Customer, an amount sufficient to recover anysuch tax or fee.

2.5.8 Special Arrangements
2.5.8.1 Contract Service Arrangements - When economically practicable, Customer specificcontract service arrangements may be furnished in lieu of existing tariff offerings.
a. Rates, Charges, Terms and additional regulations, if applicable, for thecontract service arrangements will be developed on an individual casebasis, and will include all relevant costs, plus an appropriate level ofcontribution.
b. Unless otherwise specified, the regulations for contract servicearrangements are in addition to the applicable regulations and ratesspecified in other sections of this tariff.
2.5.8.2 Promotional Offerings - The Company from time to time, may make promotionalofferings of its services, which may include waiving or reducing the applicablecharges for the promoted service. Promotions will be filed as separate tariff sheets.
All promotions will be limited in duration and will identify a definite, reasonabletime period during which the promotion will be in effect. A promotion may also belimited as to the locations where the offerings are made. A promotion may also, ifso designated, provide for its conclusion upon the occurrence of a promotion-relatedevent that is reasonably certain to occur; although, the timing may not be certain.
All promotions shall be made available to other carriers for resale, and shall bepriced, in conjunction with the entire service offering as a whole, above theCompany’s total service long run incremental cost.
Promotions will be implemented with at least one day’s notice prior to the effectivedate of each promotion.

2.5.8.3 Special Construction
If the provision of service to a Customer would require the construction ofadditional facilities, replacement facilities or special facilities designed to meet theCustomer’s particular needs, the Company, at its option, may seek to obtain thefacilities from another carrier, or may construct the facilities. The Customer maybe required to pay all of the Company’s actual expenses incurred in obtaining orconstructing the facilities, including but not limited to, any unusual maintenancecosts or removal costs. Construction charges may be payable, at the Company’soption, prior to acceptance of the Customer’s application for service or when billingis rendered. The Customer may be required to enter into a written agreement to paythe construction charges if they are not paid prior to initiation of service.
Any facilities constructed by the Company by the use of construction charges,however financed, shall be and remain the property of the Company, unlessotherwise agreed to by the Company pursuant to a written agreement. The Customerdoes not obtain any rights of ownership in facilities provided by the Company.
The charges and regulations applicable to special construction apply in connectionwith all classes of service, facilities or equipment furnished by the Company and arein addition to the installation charges, service connection and move charges andmonthly service charges otherwise applicable to the provision of service to theCustomer pursuant to other sections of this tariff.

2.5.9 Special Services
2.5.9.1 Emergency Services - Allows Customers to reach appropriate emergency servicesincluding police, fire and medical services. The 911 Service includes lines andcentral office features necessary to provide the capability to answer, transfer anddispatch public emergency telephone calls originated by persons within thetelephone central office areas arranged for 911 calling. If 911 Service is notavailable in an area, the Company shall make arrangements for the Customer toreach the appropriate emergency services through dialing “0”.
2.5.9.2 Telecommunications Relay Service - Telecommunications Relay Service enableshearing-impaired or speech-impaired persons who use a text telephone or similardevices to communicate freely with the hearing population for using the texttelephone and vice versa. The Company does not impose any charge to end usersfor access to Telecommunications Relay Service. However, persons using thisService are liable for applicable per call/increment charges.
2.5.9.3 Call Blocking Service - Call Blocking Service is a Service which providesCustomers with the capability to block originating calls to the 1-900 callingnetworks or 976 services. When Call Blocking Service is requested, all originatingcalls to 900 numbers nationwide will be blocked. Calls to a 976 service will also beblocked. Customers with Call Blocking Service attempting to dial a 900 numberfrom a restricted line will reach a Company-provided or DUC-provided interceptannouncement. Call Blocking is provided at no charge.

Excel Telecommunications (the Company and/or Excel) is a telecommunications provider incorporated underthe laws of the State of Texas. Excel provides telecommunications services in the areas certified to it withinthe State of Michigan.
All rules, regulations and rates of the Company apply to its certified area unless otherwise specifically notedin this tariff.
Excel Local Exchange Services may be offered in conjunction with an associated long distance serviceoffering provided by Excel.
3.1 Non-Recurring Charges
3.1.1 Service Connection Charges
These charges are nonrecurring and apply to various Customer requests for connecting,moving or changing service. They are in addition to any other scheduled rates and chargesthat would normally apply under this tariff. The charges in this section for the connection,move, or change of service contemplate work being performed by Company employeesinvolved at a time when overtime wages do not apply. No distinction is made between aNew Installation and Connection and an Outside Move . All changes in location ofCustomer s equipment or service from one premises to another, are treated as new serviceconnections with service charges applying. Service charges are in addition to other rates andcharges normally applying under this tariff. They apply in addition to construction chargesmade because of unusual costs in establishing service and are not in lieu of usage charges.A maintenance service charges will apply in those instances where Excel makes a repairvisit to the customer's premises and the service difficulty or trouble is not on Excel's side ofthe demarcation point.

c. Restoration of Service Charge:
If service is disconnected for nonpayment, service will be reestablished only uponreceipt of payment of all charges due, which include charges for service andfacilities during the period of disconnection and which may include a servicerestoration fee. If the Customer has a history of payments returned for insufficientfunds, the Company may require payment by cash, money order, personal check orcertified check. If payment is made by personal check, restoration will be effectedupon clearance of the check. When a Customer’s service has been disconnected inaccordance with this tariff and the service has been terminated through thecompletion of the Company’s service order, service will be reestablished only upona basis of an application for new service. During the period of disconnection,Customer’s telephone number will not be reassigned. Once service has beenterminated, the telephone number may be reassigned to another Customer.

Customers subscribing to the Company’s Local Service must purchase a local packageconsisting of an access line (dial tone). Customers who order an additional line(s), areexceptions to the local package purchase requirement. The Excel Classic MyLineK LocalService Packages described below in sections b, c and d are only available to existingcustomers of the Excel Classic MyLineK Local Service Packages (formerly known as ExcelMyLineK Local Service Packages) as of February 16, 2004. 3.3 Directory Listings
3.3.1. General - Customers shall provide the Company with information for all listings. TheCompany will include the Standard listing in the White Pages (Alphabetical Section) of thetelephone directory and will offer one additional listing to the Customer. The Customer must identifyits non-published and non-listed telephone numbers for directory purposes.
3.3.2. Directory Listing - Standard - The Customer will receive one Standard listing per telephonenumber, in the alphabetical section of the directory, at no additional charge.
3.3.3. Directory Listing - Additional - The term Additional listing denotes any White Page listing,regardless of form, in addition to the Standard listing. A monthly rate applies for oneAdditional listing. An additional listing may be any of the following:
a. Names of members of the Customer s family or of persons residing in the Customers household.
b. When the Customer s name or names of other persons residing in the household isspelled in more than one way, additional listings of the alternative spelling are permitted.

3.3.4 Non-Published Directory Listings - Non-published telephone numbers are not listed in eitherthe Company s directories or directory assistance records available to the general public.When a call is placed from a telephone number associated with a non-published listing, thenumber and name may be disclosed if the called party has equipment to display CallingNumber Delivery and/or Calling Name Delivery. Customers may prevent the display of thecalling number and name by activating Caller ID blocking.
3.3.5 Non-Listed Directory Listings - Non-listed numbers are not listed in the Company sdirectories but are included in directory assistance records available to the general public.
3.3.6 Regulations - Incoming calls will be completed by the Company only when the calling partyplaces the call by dialing the telephone number. The Company will adhere to this practicenotwithstanding any claim of emergency the calling party may present. The acceptance bythe Company of the Customer s request to refrain from publishing his/her telephone numberin the directory does not create any relationship or obligation, direct or indirect, to any otherperson than the Customer.
The Company s liability, if any, for its gross negligence or willful misconduct or the rightof the Customer to seek any legal remedy available for the same is not limited by this tariff.In the absence of gross negligence or willful misconduct with respect to any claim or suitbrought by, or other legal remedies available to, the Customer for damages associated withpublishing the telephone number of non published service in the directory or disclosing saidnumber to any person, the Company s liability, if any, shall not exceed the monthly chargeswhich the Customer may have made for such unpublished service for the period duringwhich the service was affected. Except as otherwise indicated, the Customer indemnifiesand saves the Company harmless against any and all claims for damages caused or claimsto have been caused, directly or indirectly, by the publication/non-publication of the nonpublished listed number or the disclosing/non-disclosing of said number to any person.

Allows Customers to obtain help determining listed telephone numbers. A maximum of two(2) requested telephone numbers will be provided for each Directory Assistance call.
A residential Customer is allowed three (3) direct-dialed Directory Assistance calls perresidential exchange line per month at no additional charge. No credit will be given for anyunused portion of the Customer’s allowance, and call allowances are not transferrablebetween separate accounts of the same Customer. No credit will be given for requests inwhich the telephone number is non-published or unlisted, or for telephone numbers whichare not found in the directory.

3.4.2 Directory Assistance Call Completion Service
Directory Assistance Call Completion (DACC) provides a Customer calling DirectoryAssistance with the option of having the call to the requested numbers completed. DirectoryAssistance Call Completion Service is furnished only where facilities are available.
For local calls, charges for DACC are not applicable to Customers with disabilities thatqualify for exemptions from Directory Assistance charges. Two types of DirectoryAssistance Call Completion are offered:
a. Fully Automated: The Customer receives the requested directory number from anautomated voice system. The Customer accepts DACC by depressing 1 from aTouch-Tone telephone when prompted by the DACC announcement.
b. Semi-Automated: The Customer receives the requested directory number and thenrequests the operator to provide call completion to the requested number.

3.5 Lifeline Service and Link Up America
3.5.1 Lifeline Service
Lifeline Service is a telecommunications service assistance program designed to provideeligible residential customers with a reduction in the price of Basic Residential LocalService. The purpose of Lifeline Service is to maintain and preserve universal service. Tolllimitation service may also be provided as part of Lifeline Service. Toll limitation mayinclude toll blocking, which prevents the placement of all long distance calls for which thesubscriber will be charged, or toll control, which limits the toll charges a customer can incurduring a billing period to a preset amount.
Eligible Residential Customers will receive a discount off the normal applicable tariffed ratefor their local exchange service.
Lifeline Service will be provided to those applicants who provide proof that their annualincome does not exceed 150% of the federal poverty income standards. Applicants shall notbe provided assistance for more than one basic residential access line in the state. Anapplicant for Lifeline Service may report only one address in the state as the principal placeof residence.
All charges, either recurring or nonrecurring, for any service or feature other than LifelineService as described herein, shall be billed at the tariffed rate. Lifeline Service shall not beavailable on a retroactive basis.

SECTION 8.0 - PROMOTIONAL OFFERINGS
From time to time, the Company may provide certain special offerings to its Customers in the form of servicepromotions. These special offerings may be limited to certain dates, times and locations. The Company’sservice promotions are listed below:
8.1 MyLine Basic Caller ID Promotion
Beginning September 1, 2003 and ending September 30, 2003, Excel offers the MyLine Basic andCaller ID Promotion to new residential Customers in the State of Michigan. New Customers whosubscribe to Excel MyLine Basic Local Service Package and Caller ID during the aforementionedtime period will be eligible to receive a billing credit to reimburse the Caller ID monthly recurringcharges for the first three months of service. The billing credit will be applied to the Customer’saccount during the third month of service and should appear on the Customer’s invoice with the thirdMonthly Recurring Charge for the Caller ID feature. The Customer must remain subscribed to boththe Excel MyLine Basic Local Service Package and Caller ID for the entire three-month period inorder to be eligible to receive the billing credit, and the Customer’s account must be in goodstanding.
