WELCOME TO WDSRA!WDSRA is THE place for engaging community-based social & recreational programs for individuals of all ages with special needs. Our expert staff is dedicated to making sure every experience is a positive one.

Our WDSRA Programs are designed similar to traditional park district programs. There is something for every interest including: competitive and recreational athletics, Special Olympics, adaptive sports, music, dance, art, theater, social clubs, camps, trips, special events and more!

Through Inclusion Services, we offer assistance to any individual with a disability who registers for a park district program from any of our member districts. Assistance varies depending on the needs of the individual and can range from adapting materials to sending support staff. There is no additional cost for participants to receive inclusion services.

Welcome! WDSRA wants you to know that Outreach can make your transition to our programs as effortless as possible. Staff focuses on welcoming new families, answering questions, navigating forms and processes, and helps recommend appropriate programming for your loved one. In outreach, we try to get to know your participant and family before starting programs so that WDSRA can serve you better. We can also provide contact information for resources of which your participant may be in need. If you are a new family or participant to WDSRA programs, start by contacting WDSRA at 630-681-0962.

Inclusion Services

WDSRA and its member park districts believe that all individuals should be provided with leisure opportunities that allow for performance at their highest level of ability. We understand that not every person who has a special need desires WDSRA programming. In cooperation with our member districts, WDSRA staff will assist and advise any resident who is interested in participating in a local park district program.

WDSRA will provide the type of support deemed necessary for successful participation in a park district program. Supports may include training with the park district staff members, consultation with the individual and that individual’s family members and the assistance of a support staff member. WDSRA provides all inclusion services at no cost to the individual.

WDSRA Board Meetings The WDSRA Board of Directors meets every other month. These meetings are usually held at WDSRA and are open to any community member. Call the office or visit www.wdsra.com for the time and location of each meeting.

Certifications/Credentials Certified Therapeutic Recreation Specialists (CTRS) and Certified Park and Recreation Professional (CPRP) are designations granted to professionals who can demonstrate through education and work experience that they meet the criteria set by the National Recreation and Park Association (NRPA) Certification Board and the National Council for Therapeutic Recreation Certification. These professionals have a Bachelor’s or higher degree from an NRPA accredited program, have passed a certification exam, and earn and maintain sanctioned Continuing Educational Units (CEU). Other recreation staff credentials include; Special Education.

Family Playgroup Dayell Houzenga WDSRA, Child and Family Connections (CFC) and Northeast DuPage Special Recreation Association (NEDSRA) offer a free playgroup for families that have a child with a disability (ages birth to 3). The goal of the playgroup is to bring families together, in an open format, to play with their children and meet other parents who may be going through similar experiences. Toys, balls, bubbles, art, books, music and more are provided to help your children have fun. Siblings are encouraged to attend too. The playgroup happens once a month at the Ackerman Sports and Fitness Center gym (800 St. Charles Rd, Glen Ellyn). The group usually meets on a Friday toward the end of the month from 9:30-11 am. To sign up, please call Child and Family Connections at 630-493-0400 and ask for Jenny Somone.

COMMUNITY EVENTS FOR EVERYONE

Using Essential Oils for Sleep, Focus & Sensory Needs Jorie MeyerPARENTS & CARETAKERS this one is for you! Wendy M. Craven, a doTERRA Essential Oils Wellness Advocate, will share information on how essential oils may help with sleep, focus and brain health and also have a calming effect on our participants who often have anxiety and sensory needs. In addition, sensory stations will be set up and you will learn how to make them! Light refreshments using essential oils will be provided. A small make & take will be included and recipes will be provided. Don’t miss out on this opportunity!

Lekotek Therapeutic Family Play SessionsChildren can go from struggling to succeeding with guided therapeutic play sessions. Lekotek helps children with an area of delay or concern reach their potential and strive for developmental milestones through fun and play. Certified Lekotek Play Specialists guide the whole family through monthly play sessions and introduce you to a variety of adaptive toys, making play possible and enjoyable for all children. Siblings are included which promotes bonding and family unity. Families borrow the toys used in sessions to continue to gain benefits in home and allows the fun to continue from month to month. Kidz & Sibz Night Brianne Carroll Calling all parents! It’s your time to enjoy a night out. Drop your kids off for a fun filled night. Participant and participant’s siblings are invited to join in on the fun. Some activities will include arts, crafts, music and games. Kid/Teen Bowling Mari Stocco SPARE yourself the boredom of having nothing to do after school, grab your friends, and sign up for WDSRA's Kids and Teens Bowling program! Participants work on social skills, taking turns, and behavioral control while improving balance, coordination and gross motor skills. Each week he will lead you through a balanced physical and mental workout. The course will focus on increasing concentration and balance as well as improving overall coordination, gross motor skills, endurance and flexibility. School's Out! Discover the Dinosaurs Kristy Ekner

Spend your day off of school with WDSRA as we venture back to a time when the dinosaurs roamed the earth. We will be heading to Rosemont for the Discover the Dinosaurs Unleashed exhibit. From T-Rex to the Stegosaurus we will be doing our very own Jurassic Park tour. Ticket is included in the fee. Please bring a sack lunch and additional money for concessations if desired.

Christian Youth Theater Presents: Aladdin Erin White

Christian Youth Theater is one of the most respected children’s theater companies in Chicagoland, producing 32 family-friendly productions starring students ages 8-18. Drop In Center Emily AckermanJoin a great group on Wednesday nights to socialize and have fun! WDSRA and the National Alliance on Mental Illness (NAMI) of DuPage County give individuals, who have a primary disability of mental illness, an opportunity to spend leisure time together, in a friendly relaxed atmosphere. Family members are welcome to attend. Dinner is served from 6-7 pm and then a fun activity from 7-8:30 pm. Activities may include going out into the community, playing board games or cards, learning yoga, art, and exercising to name a few. The group meets at the Alfred Rubin Riverwalk Community Center and is for ages 21 and older. The first Wednesday of every month is “Visiting Artist Night,” which is made possible through grants received from the City of Naperville. Aktion Club Stephanie ReeveAktion Club is the newest member of the Sponsored Organizations and Programs at Kiwanis International. Aktion Club is a community-service club for adults who live with a disability. The club typically meets every other Monday from 6:30-8 pm either at the WDSRA office in Carol Stream or at other locations in the community. Members plan and experience community service projects, fund raising, and educational presentations while developing important skills in the process. Special Olympics Powerlifting Justin BanksWDSRA and the College of DuPage have teamed up to offer a quality Special Olympic Powerlifting program! With the help of the football team each powerlifter will be paired up with an experienced lifting partner. These partners will teach technique and push the athletes to give their all and compete at a high level at Spring Games. By registering for this program you are eligible to compete at Spring Games and if you win a gold medal, you then are eligible to compete at Summer Games in June. This software will be a one stop shop with the most up to date information regarding your team's schedule as well as any changes that occur throughout the season. You will be able to access this information on your desktop or laptop as well as a free mobile app that is available for both Apple and Android users. Once you are assigned to your team each season you will receive an email invitation that will link you to your specific team page. This email will be sent to the primary email we have on file here at WDSRA.

Special Olympics Rhythmic Gymnastics Justin BanksJoin a unique gymnastics class and prepare to compete in Special Olympics in March! Find your inner rhythm and express yourself through movement and dance. Participants will master set dance routines using a ball, ribbon, hoop or rope. To be eligible for this program you must have participated in the Fall Rhythmic Gymnastics program. Please note: athletes will be grouped according to age first and level of routine second and will be contacted with their specific 45 minute time slot before the start of the program. Special Olympics Softball Justin BanksIt is time to play ball! Let's take the field and brush up on our skills for the summer competition. All athletes are required to provide their own black pants or shorts; WDSRA will provide the jersey. The team will compete in two tournaments throughout the season.

The ITRS Tournament will be held in July and the District Special Olympics Tournament will be held in August. By registering for this program you are committing to attend both tournaments listed above. If you are unable to participate in either of these tournaments or if you are not interested in a competitive team, please register for the Recreation Softball program. We can accept a maximum of 18 players per roster and will not create another team unless we have another 15 players interested. Therefore, if you cannot commit to these tournaments please do not take up a roster spot for someone who can attend all tournaments.

You will be notified of your field location prior to the start of practices. We will be conducting player evaluations during the first practice and teams will be created based on ability level. Athletes must have a Special Olympics Medical Form on file current through September 2018 to be eligible to register for the program. If the team qualifies for state competition there will be an additional fee assessed. Please note: locations and times may change.

Camp Duncan Emily AckermanDon't forget to sign up for five full days of fun with all your WDSRA friends at Camp Duncan in Ingelside, IL. For the last several summers we have had so much fun, we're going back again! This beautiful YMCA camp is on a private lake. Escape to the great outdoors for swimming, kayaking, paddle boating, hiking, campfires, crafts, dances, horseback riding, and more! Participants must be able to perform daily living skills and function at a 1:4 ratio. We will be staying in rustic cabins with bunk beds. Campers will be contacted two weeks in advance with all information pertaining to the trip. A $100 non-refundable deposit is due upon registration, and registration will begin with Spring programs. The balance must be paid in full by July 20th. This program is not wheelchair accessible and does require participants to do a lot of walking and to keep pace with the group. Program Information Dori NapolitanoRec & Roll is a community based recreation program that emphasizes leisure independence, social interaction, community outings and service projects. Staff utilizes recreational activities to help people with disabilities develop and enhance their quality of life. The daily schedule can include animal therapy, volunteer projects, gardening, field trips, fitness, outdoor recreation, art therapy, crafts and games. Upon occasion additional money is required for activities. Participants must be able to successfully participate in a 1 staff to 6 participant’s ratio and perform all of their own daily living skills. Participants attending Rec & Roll must be toilet trained, responsible for independent care such as feeding and dressing, devoid of emotional outbursts that require 1:1 staffing. Please send a sack lunch daily. The program limit is 12.

2018 REC & ROLL PROGRAM INFORMATION

Rec & Roll Observation Visit

New participants interested in registering for Rec & Roll must pre-arrange an observation day to determine whether they meet the program eligibility requirements. This observation visit will help staff determine which Rec & Roll is most appropriate. Observation day allows the parent and participant to see the program before joining. Staff ratio will be determined at observation day by the day program manager. A visit can be set up by contacting Dori Napolitano, Day Program Manager at 630.384.8569. Rec & Roll Registration & Payment Information

When registering for Rec & Roll a $100 non-refundable deposit is required for each day of the week registered. The $100 deposit secures your spot in the program and does not count as your first payment, which is due by December 15th. Your $100 deposit goes toward your total bill. The remainder of the balance should be divided into 10 equal payments: your total bill for the year must be paid in full before you are eligible to register for the new calendar year. Payments can be made by check payable to WDSRA, Visa, MasterCard or online at WDSRA. If you are paying with a Visa or MasterCard please call the WDSRA front office at 630-681-0962. Current monthly payments are due on the 15th of each month. A $10 late fee will be applied to each day of the week of Rec & Roll you attend. At the end of the month, if you have not made your payment, you will be suspended from the program until your payment is up to date and your late fees are paid. Boccia is a precision ball sport contested by athletes on the local, national, and even international level. The sport is open to individuals with spastic hypertonia, dystonia, athetosis or ataxia in all four limbs or with severe locomotor dysfunction. Adaptive equipment is available for those who need assistance in propelling the ball onto the court.

ARCHERYArchery is an exciting lifetime sport that grows with you. Whether starting at a beginner level to learn the funda-mentals of archery safety and basic technique, or to further develop more advanced skills, Synergy Archery will provide you with the tools and support to reach your goals. This program is open to anyone with a primary physical disability, including individuals with a visual impairment. Adaptive equipment is available.

ADAPTIVE SWIMMING Enjoy an introduction to swimming and water safety, or work on more advanced stroke technique. All levels of skill and ability, with any primary physical disability, are welcome. Lessons are shaped to fit your individual goals and needs. Synergy Adaptive Swimming is open to individuals learning to swim or those looking to compete, and instruction will be tailored accordingly.

CLIMBINGClimbing fosters an amazing sense of community ,trust, courage and accomplishment. It can be easily adapted to almost any ability level. Through climbing, you can break through barriers and summit to new heights. Suburban climbing first and third Mondays of each month at Vertical Endeavors, Glendale Heights, with additional opportunities in Chicago as well as outdoor trips.

VI BOWLINGSynergy is proud to offer this program that enables blind and visually impaired youth and adults the chance to improve their bowling skills in a social setting. Staff will provide instruction, adaptive resources and support to help you achieve success. This program is run in a drop-in format so you can sign up for one date or multiple dates throughout each season. Guide rails are provided.

Behavior Code Western DuPage promotes the concept of “equal fun for everyone” and strives to accomplish this belief through our program goals. However, certain rules are necessary to ensure everyone’s safety and enjoyment.

Participants, staff, volunteers and parents are expected to exhibit appropriate behavior at all times. The following guidelines have been developed to make WDSRA’s programs safe and enjoyable for everyone. Additional rules may be developed for particular programs and athletic leagues as deemed necessary by staff. Staff, participants, volunteers and parents shall:

• show respect to all participants, staff, volunteers and parents

• refrain from using foul language • refrain from causing bodily harm to other participants,

staff, coaches or parents • demonstrate respect for equipment, supplies and

facilities • represent WDSRA in a manner that is consistent with

the goals, objectives and ethics of the association

Discipline A caring and positive discipline approach will be used. WDSRA reserves the right to dismiss a participant, staff, volunteer or parents if it is in the best interest of the association. Each situation will be evaluated on its own merit.

Sports Leagues WDSRA has developed a Sports League Code of Ethics for athletes, coaches and parents which is based on the Character Counts program. The program emphasizes six core ethical values: trustworthiness, respect, responsibility, fairness, caring and citizenship. Athletes, coaches and parents may be asked to sign this code to participate in a team sports program.

Special Olympics In order for an athlete to compete in a Special Olympics program a current Special Olympics Medical form must be on file at the WDSRA office. This form is separate from the WDSRA annual form. The Special Olympics Medical form is valid for two years from the date of the doctor’s signature.

WDSRA Policy states an athlete cannot register for a Special Olympic program until you have a current form on file. If you are interested in participating in a Special Olympics program please contact the WDSRA office for

a form, use the form from this brochure on page 21 or download a form from our website at www.wdsra.com.

Parents and athletes are responsible for returning the completed form to the WDSRA office before you register. The form must have original signatures of both the doctor and parent/guardian.

The tournament fee must be pre-paid prior to participation in the tournament. The fee is nonrefundable. Any athlete not scratching prior to the scratch deadline will be responsible for paying additional scratch fees assessed by Special Olympics.

Atlanto-Axial Subluxations (ASC) Individuals who have Down Syndrome have a high risk of having ASC. For the safety of WDSRA participants with Down Syndrome, a doctor’s note proving they have been cleared of this condition by an x-ray is required to participate in programs that include swimming, diving, gymnastics, tumbling, alpine skiing, soccer, exercise or any program that could potentially injure the neck area. Participants that have ASC and are in these programs may have limited participation.

Annual Information Form WDSRA requires all participants to complete an annual information form prior to participation. This form expires every year on December 31. The form provides us with valuable information which enables us to offer programs that are safe and appropriate for your needs. If you are new to WDSRA programs or do not currently have an annual form on file, please contact our office and request one. If the agency receives a registration form that does not have a current annual, we will send you one to complete and return.

New Participant Intake WDSRA requires all new participants to complete an intake with the Outreach Manager prior to attending their first program. The intake allows the agency to get to know the participant and their individual needs better. This additional information provides WDSRA staff the opportunity to properly staff programs and provide an environment that will be successful and enjoyable to the participant. If you are new to the programs and have not done an intake please contact April Prescott at 630-681-0962. For safety reasons, an individual without current annual information and intake forms on file will not be permitted to participate in WDSRA programs.

WDSRA Weather Cancellation Policy In the event of severe weather, WDSRA may decide to cancel programs in the best interest of our participants. The following guidelines will be used to determine cancellations:

Severe Winter Weather Outdoor program or programs with transportation for people with physical disabilities: Temperature of 10 degrees; For wind chill of 0 degrees or less.

All other programs with transportation: Temperature of -5 degrees or wind chill of -15 degrees or less.

All programs (including indoor): snowstorm or blizzard situations when driving restrictions and emergency accident plans are in effect (i.e. winter storm or blizzard warnings).

Other Weather: All Programs (including indoor)- Tornado warning is in effect in DuPage or surrounding county. WDSRA will use discretion for all other weather warnings and watches.

Canceling Programs: 1. WDSRA staff will look at all circumstances beginning two hours prior to the starting time of the program or first pick-up to determine if the program should be canceled.

2. If the program is canceled, program staff will attempt to reach all participants and staff by phone calling post message to inform them of the cancellation.

3. Missed programs due to weather are made up the week following the end of the program. If this is not possible, you will be issued a refund for the cancellation.

Registration Neatly complete the registration form, filling in all appropriate spaces. Missing information will delay your registration. Registration confirmations are mailed prior to the start of the program season. Phone registrations are not accepted.

Equal Access No participant shall be denied equal access to programs, activities, services or benefits,or be limited in the exercise of any right, privilege, advantage or opportunity on the basis of race, sex, creed, national origin or disability.

Boardmaker If a participant uses a picture schedule program such as Boardmaker, please indicate this on the registration form. Our staff will work with you to facilitate its use in our programs.

Meeting on Location For safety reasons, WDSRA will only allow participants to meet on location for community events (rather than taking the WDSRA transportation) when there is one main entrance/exit. Participants will not be allowed to meet at locations with multiple doors such as, but not limited to a mall, festival, carnival, etc.

Easy Ways to Register 1. Online Registration (Preferred Option) – Online registration offers you the most up to date information on your programs. Upon completion of your online registration you instantly know your registration status.

2. Walk-in on the day of registration – you may bring your registration form to the WDSRA office the day of registration and we will process your requests while you wait for your confirmation.

3. Drop-off or Mail in prior to the start of registration – all mailed and dropped off registration forms will be processed concurrently with online registration and will be processed in random order. SPECIAL INFORMATION

Refund Policies Refunds for all programs may be obtained up to two weeks prior to the first class date. A $5 fee will be charged for each program dropped at the request of the parent/participant. A house credit will be issued for refunds less than $10. Refunds for special events may be obtained up to two weeks prior unless tickets or contracted services have been purchased. All refunds will be issued in check form.

Out of District Policies Families living outside a WDSRA Park District member district and belong to a neighboring SRA are welcome to register in WDSRA program during the out of district registration time frame. This would apply to NEDSRA, FVSRA, NWSRA, and SEASPAR residents.

If you live within a community with a Park District that does not belong to a Special Recreation Association a 50% fee will be accessed per program .

If you live in a community without a Park District or an unincorporated area that falls outside of the Park District boundaries, a 50% fee will be assessed per program. An example of this would be Wayne residents and some parts of West Chicago.

Program Policies When arriving at/departing from a program, parents are asked to check in with the staff person in charge of the program before leaving your child. WDSRA staff are not responsible for supervision of participants who arrive prior to the start of the program. Also, if a participant is to be picked up by an unfamiliar guardian, neighbor, babysitter, etc. the parents are asked to notify the WDSRA staff person in charge of the program prior to the occurrence.

To provide for safe and meaningful learning experiences, a 1:3 staff-to-participant ratio is maintained for children’s activities, a 1:4 ratio for teen activities, and a 1:6 ratio is maintained for adult activities. Some programs may have a higher ratio, depending on individual needs. When children of various ability/age levels are enrolled in the same program, WDSRA will group them according to their abilities.

Many of WDSRA’s programs are open to sibling participation. Look for any program that states “siblings” under the “who.” Siblings may also register for any special event, but are required to wait until after the registration deadline.

If a program closes due to large enrollment, a paid waiting list of those interested will be kept. If space does not open in the program, your payment will be automatically refunded.

A class may be cancelled if enrollment is less than four participants for children’s programs and five participants for adult programs, unless otherwise noted.

WDSRA requires a minimum of 3 participants at a pick-up location. Transportation will be cancelled if the minimum is not met. You will be notified if this occurs for any program. If parents wish for their son or daughter to walk home from a program, please send a signed and dated notice attached to the registration form, or mail the written notification to the WDSRA office prior to the first day of the program.

If a participant cannot attend a program during the session, he/she cannot send a substitute in his/her place.

On occasion, WDSRA staff may photograph or videotape participants in programs or events. These photos are for WDSRA use only and may be used in publications, brochures, pamphlets, flyers and/or videos.

WDSRA Annual Scholarships The scholarship request process is intended to allow participants/families who qualify for financial assistance to take part in recreation and leisure opportunities through the use of available funds.

All funds for scholarships are obtained through the WDSRA Foundation’s annual fundraising campaigns. Each year the amount available fluctuates based on the amounts received. Scholarships are only available for residents of the association’s nine member park districts.

Scholarships are awarded to participants who have completed the annual scholarship application and qualify for financial need. Applications are accepted anytime during the year but all applications expire on December 31. Fifty percent of the program fee for three (3) programs per season may be awarded. Payment plans may be arranged if needed. For more specific guidelines regarding scholarships, please visit our website at www.wdsra.com and go to the links tab for WDSRA forms or contact the WDSRA office.

Late Pick-Up Policy In order to be fair to our participants and program leaders, WDSRA has enacted a late pick-up policy and we would appreciate your cooperation in this matter. Many of our programs have more than one pick-up location and if one person is late, it may disturb the schedule of others who are being picked up at another location. In addition, many of the WDSRA program leaders have responsibilities after everyone is picked up. We request that you double-check the pick-up time and be prompt.

Dietary Restrictions Due to the amount and various types of dietary needs, WDSRA will only provide an alternative option for meals when the program is preparing/making their own food, If the program is attending a special event or eating at a restaurant in the community, the family must look ahead at the menu and relay the participant’s order to the head instructor upon arrival.

You may be asked to send or make appropriate accommodations for your participant on occasion.

The Western DuPage Special Recreation Association (WDSRA) is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. WDSRA continually strives to reduce such risks, and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities.

You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

Warning of RiskDespite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Participants must understand that certain risks, dangers and injuries due to acts of God, inclement weather, slipping, falling, equipment failure, failure in supervision, premises defects and all other circumstances inherent to recreational activities/programs exist. The Western DuPage Special Recreation Association (WDSRA) is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. WDSRA continually strives to reduce such risks, and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities.

You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

Warning of RiskDespite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Participants must understand that certain risks, dangers and injuries due to acts of God, inclement weather, slipping, falling, equipment failure, failure in supervision, premises defects and all other circumstances inherent to recreational activities/programs exist. In this regard, it must be recognized that it is impossible for WDSRA to guarantee absolute safety.

Waiver and Release of All Claims and Assumption of RiskPlease read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation services, when provided).

I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in this program/activity against WDSRA, including its officials, agents, volunteers and employees (hereinafter collectively referred as WDSRA).

I do hereby fully release and forever discharge WDSRA from any and all claims for injuries, damages, or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with this program/activity.

In the event of an emergency, I understand and authorize WDSRA staff and officials to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for immediate care for myself or minor child/ward and agree that I will be responsible for payment of any and all medical services rendered.

Photo/ Video ReleaseI hereby authorize and give my consent to WDSRA to photograph/video my child (or me) or to obtain outside photographs/video of my child (or me) participating in WDSRA activities/events/programs, and without limitation, to use such photographs/video in connection with promoting/advertising the services, programs, and facilities of WDSRA, without consideration of any kind
