Libertyville Gymnastics Academy’s Policies and Information

1. There is a Non-Refundable Annual Registration Fee. $45.00 per child, or $65.00 per family, in addition to your class fee. 2. There will be a $25.00 service charge for all returned checks. 3. ALL make-up classes MUST be scheduled with the office. YOU MAY ONLY SCHEDULE TWO (2) MAKE–UP CLASSES PER SESSION AND IT MUST BE USED DURING THAT SESSION. NO MAKE-UP CLASSES WIL BE ALLOWED TO BE CARRIED OVER INTO THE NEXT SESSION. 4. A waiver must be filled out and on file for every child using our facility. 5. CLASS DISCOUNTS: Any student enrolling in more than one class will receive a 10% discount on their 2nd and 3rd classes. The full annual registration fee still applies. 6. FAMILY DISCOUNTS: A 10% discount will be given on the lower fee of a sibling’s class. The full annual registration fee still applies. 7. Throughout the registration process, we will attempt to honor teacher requests, however, we can not guarantee these requests. 8. A full refund will be given if cancellation occurs on or before the last Saturday of the previous session. A 50% refund will be given if cancellation occurs on or be the first Saturday of the session you are registering for. NO REFUNDS WILL BE ISSUED AFTER THE FIRST SATURDAY OF WEEK 1 OF THE SESSION YOU ARE REGISTERED FOR. 9. We do not accept expired coupons. 10. We offer $10.00 off per family, for active fire, police, or military.

Ninja Zone Uniform We are a club member of Ninja Zone and they

require our little members to purchase a uniform and wear it to class every week. This is a one time $35 uniform fee.

Uniform consists of a Ninja Zone t-shirt and a

Ninja Zone headband.

Our Session is 10 Weeks long but due to the week we are

closed, please schedule a make up class during this session.
