　 　 A71-year-old　 woman　 with　 pan-pcritonitis　 was　 transferred　 to　our　 hospitaL　 Imaging　 studies

showed　 the　 extravasation　 of　contrast　 mcdium　 from　 the　 bladder　 to　the　 intrapcritoncal　 cavity,　 thc　right

non-visualizing　 kidney　 and　 the　left　hydronephrosis.　 Cystoscopy　 revealed　 bladdcr　 stones　 and　 a　fistula　at

thc　posterior　 wall　 of　the　bladder.　 A　 histological　 examination　 of　specimen　 around　 the且stula　 revealed

squamous　 cell　carcinoma(SCC)with　 stromal　 invasion・

　 Symptoms　 of　peritonitis　 subsidcd　 fbllowing　 the　 placement　 of　bilateral　 urcteral　 cathcters　 and　 a

urethral　 cathctcr.

　 　 Because　 a　 laparotomy　 rcvealed　 peritonitis-carcinomatosa,　 shc　 underwent　 bilateral　 urctcro-

cutaneostomies,　 the　removal　 of　thc　bladder　 stones　 and　 the　 primary　 closurc　 of　thc且stula.　 She　 dicd　 on

the　42nd　 day　 afモer　surgery.

緒 言

　膀胱破裂の多 くは外傷性に起こり,自 然破裂は稀で

ある.今 回われわれは膀胱破裂を契機に発見 された膀

胱扁平上皮癌の1例 を経験 したので報告する.

症 例

　患者:71歳,女 性

　主訴:下 腹部痛,両 背部痛

　既往歴:42歳 時子宮癌にて子宮全摘,当 時放射線療

法が行われたかどうかは不明

　現病歴:数 カ月前から膀胱刺激症状があり,近 医で

膀胱炎と診断され治療を受けていた.2003年4月22日

下腹部痛にて近医を受診,尿 路結石疑いで鎮痛剤など

の投与を受けたが,持 続する疹痛のため同日他院を受

診.腹 膜炎疑いで当院転送 とな り,持 参 したDIP,

CTよ り膀胱破裂の診断で緊急入院となった.

　入院時現症:腸 雑音 は低下　腹部全体に圧痛を認

め,筋 性防御,反 跳痛は共に陽性.肋 骨脊柱角の圧痛

(CVA　 tenderncss)も 陽性で あった.入 院時検査 成

績:血 圧132/80,脈 拍85,体 温37.3度.血 液デー タ

上,CRP　 20.9mg/dl,　 WBC　 I　3,800と 強い炎症所見を

認めた以外は特に異常 を認めなかった.

　 尿 検 査:尿 半 定 量 で 混 濁1+,pH　 6.　5,糖3+,蛋

白3+,沈 渣 で 赤 血 球>100/HPF,白 血 球50～99/

HPF,細 菌3+　 尿 培 養 でCor)nebacterium　 lO4　CFU,

EnterOCOCCtLS　l　O4　CFUが 検 出 され た.

　 画 像 所 見:来 院12時 間 前 に近 医 で 撮 影 し たDIPで

著 明 に拡 大 した左 腎 孟 と右 腎 の 排 泄 不 良,ま た 造 影剤

の溢流 を思わせる膀胱像 を認めた(Fig.1).来 院4時

間前 に他院で撮影 した単純CTで もやはり膀胱からの

造影剤溢流を認めた(Fig,2).入 院後行った膀胱造影

では明らかな造影剤の溢流はなかった.膀 胱鏡を行っ

た ところ,壁 の伸展 は不良で萎縮膀胱 の状態であっ

た.膀 胱内には径2cm程 度の結石が3個 存在 し,後

壁やや右側に縦2cm× 横1cm程 度の痩孔を認めた.

痩孔周囲は軽度隆起 してお り,腫 瘍性病変が疑われ

た.

　病理組織所見:痩 孔周囲から採取 した標本で角化 し

た扁平上皮である癌真珠(cancer　pearl)と その周囲に

異型細胞 を認めた(Fig.3).

以上より膀胱扁平上皮癌(SCC)に よる膀胱破裂 と診

断した.

　臨床経過:入 院後,両 側尿管 カテーテル留置,抗 生

剤投与などの保存的治療で腹膜炎症状は軽減,5月13

日膀胱全摘除術 を予定 した.し か し,開 腹時すでに癌

性腹膜炎の状態で根治術は不可能と判断 両側尿管皮

膚痩による尿路変向術 と膀胱結石の除去,膀 胱後壁の

痩孔閉鎖のみで手術 を終了 した.そ の後術後イレウ

ス　誤嚥性肺炎などを併発 し,6月24日 死亡 した.な

お,剖 検 は行わなかった.

考 察

　膀胱破裂には腹膜外膀胱破裂と腹膜内膀胱破裂があ

り,腹 膜内膀胱破裂には外傷性 と自然破裂がある1)

　これまで膀胱破裂に関する症例をまとめた加納 らの

報告2)に よると281例 中269例(95.7%)が 外傷性であ

り,自 然破裂は12例(4.3%)と 非常 に稀である.自

然破裂はさらに病的破裂 と特発1生破裂に分類 されD,

病的破裂の原因の1つ として膀胱癌があり,頻 度 とし

ては全破裂の約1%と きわめて稀な原因である2)

　自験例では子宮癌術後に放射線療法を行った可能性

があ り,放 射線療法の晩期後遺症 として膀胱穿孔が起

こった可能性は完全には否定できない.ま た膀胱腫瘍

を子宮癌の転移とみる考え方 もあるが,子 宮癌は術後

29年間まったく再発することなく経過 している点およ

び膀胱内に結石が発見されたことから子宮癌手術後の

神経因性膀胱により感染結石が発生 し,そ の結石に起

因する慢性炎症によって扁平上皮癌(SCC)が 誘発さ

れた可能性の方が高いと考えた.

　次に膀胱破裂後に腫瘍 と診断された症例について組

織別に治療,転 帰 をまとめた所,検 索 しえた全14症 例

中,TCCが7例,　 SCCが7例 であった(Table　 l).

その14症例のうちはっきりと予後の記載のあるものは

11例 で,う ち9例 が10日 一数 カ月で死亡 してお り

TCC,　 SCCと も膀胱破裂を起 こした腫瘍は予後が き

わめて悪いと言える.

結 語

　1)膀 胱破裂を契機に発見された膀胱腫瘍 を経験 し

た.

　2)癌 性腹膜炎のため根治術は不可能 と判断,両 側

尿管皮膚痩による尿路変向術のみ施行 し約1カ 月後に

癌死した,

　3)膀 胱破裂の原因として頻度は少ないが,膀 胱腫

瘍も考慮する必要がある.

　本論文の要 旨は第185回 日本泌尿 器科学会 関西地方会 にお

いて発表 した.
