CHAPTER ONE.

1.0 Introduction

This chapter reveals how challenges are associated with the management style of

Mubende among other issues covered Regional Referral Hospital among staff and

patient for the better management and effective service delivery. Through the

background, statement of the research problem, objectives, research questions, scope

and significant of the research findings.

2.0 Background

World over health care provision has not been easy even with the provision of

government subsidies towards revamping the health sector.

In Africa the health sector has equaly been given a lip service interms of basic

requirements of the health sector with a lot of spending on security among other

sectors of the economy, perpetuating poverty en poor health and consequently leading

to a situation sufficing into perspective of human development.

In Uganda the health care situation has also taken the down side providing a big

challenge in health care provision. It also gives the magnitude how this challenges

affect the quality service delivery and how the lost poor common man benefit or are

affected.

It will also come out with services provided within the hospital, catchment area,

administrative structure governance of health system within the hospital, human

resource development, staff ceiling, funding bodies, infrastructure and equipment

management, quality management system. Statement of research problem most

challenges faced by hospital research objectives research question, significance of the

study and communication system within the hospital.

Mubende Hospital started as far back as in 1920s' as small health unit, later on became

a dispensary by the government of Buganda Kingdom. It has been in upgrading by

adding simple structures . Later on become a district hospital in 2009 when it become

regional hospital with the capacity of 350 beds.

Catchment area

The hospitals serve the districts like Mubende, Kiboga and Mityana at regional basis due

to its location. It boarders with other districts who do not have district hospitals like

Kibaale, ssembabule, Kyenjojo, Kyegegwa and Gomba which puts the hospital on high

task to over serve the unexpected capacity.

It also serves the special areas like barracks of Kabamba Army training school, Kalama

mechanical barrack, CAWODISA barracks and Mubende rehabilitation centre. Additional

serves more than 100 prison units and police posts.

There is refugee camps like Kyaka II settlement area and Kyankwale refugee camp. The

estimated service population are over two million people and focusing on national

priorities and significant of local needs where people need to promote good health

service delivery and to promote sustainable development as a state mission.

Service provided within the hospital

In hospital, the researcher reveals the services provided within the hospital and health

system. In hospital it comprises of general curative services throughout the patients

department and inpatient services like Dental Service, General services or paediatrics

services, rehabilitative services, radiography investigative services, laboratory services ,

HIV/AIDS service clinics, Paediatrics Services, community outreach services and diabetic

clinics.

It also a researcher revealed that as the policy of the government of Uganda, all the

health facilities give free services to the people of Uganda who can not afford to go for

private facilities. The mission of the hospital is to serve the people of Uganda through

the coordinated services delivery as an obligation of the government to meet the needs

of its citizens.

Also the study was carried out in Mubende regional referral hospital with the purpose to

understand the major challenges faced by the hospital both negatively and positively.

Also the researcher extracted the administrative structure that governs the facility with

different departments that work inter-related to accomplish the organizational

objectives for better service delivery.

1.3 The statement of research problem

Mubende regional referral hospital is one of the oldest hospitals found in Uganda

situated in central regional in Mubende district, that has experienced a lot of problems

in different factors like political issues has contributed its collapse due to civil wars and

being situated in cross-section of four barracks and being forestry Social economic and

welfare of Mubende and surrounding area.

According to the 2002 population and housing census of November 2002 the provisional

results indicated Mubende as a growing are with the total population 84349 in flow

former countries being with high fertility and poverty of people and illiteracy of local

people.

According to the researcher carried out by the research institute of Makerere

Universityy indicated 79% people within the area are below poverty line that is unable

to meet treatment costs and educational costs.

Therefore. According to researcher views he may urge and suggest that all the ideas

and suggestion, recommendation in this research it will work better if it is used by the

health workers, administrative and management of the referral hospital none

governmental oorganizations social' and public workers and the government of Uganda

and its agents to intervened improve service delivery if the problem is to be solved.

1.4 Objective of the study

1.4.1 General Objectives

To evaluate the challenges faced by both health workers and beneficiaries within

Mubende regional hospital.

1.4.2 Specific objective

1. To describe the characteristics of management style of Mubende regional referral

hospital.

2. To establish the challenges met by government of Uganda in management of

health facilities.

3. To asses the role stakeholders roles in the management of the Hospital.

1. What is the characteristic of management style of Mubende regional referral

hospital?

2. What are the challenges met by government of Uganda in management of health

facilities?.

3. What roles do stakeholders play in the management of the Hospital?.

1.6 scope of the study

The researcher intended to carry out this research focusing on the period between

1920s and 2010 looking into challenges and achievements met by the hospital

management and administration. The study mainly focused on the both the

management challenges and achievement met during the specified period of time.

1.7 Significance of the study

This research has tried to identifies various factors that lead the institution to meet

challenges and overcome their successful basing on data collected with the hospital and

ways how the facilities are utilized to become useful in policy formulation, building

development plans and design programs.

The research probably assesses the effect on challenges met the developments within

the facility and the service delivery, quality of human resource ..... and methods of

enhancing capacity building of staff.

This research will help also to create awareness and good relationship among staffs and

their stakeholders for the benefit of Ugandans.

This research, identifies the gap between the government of Uganda and its people in

order to fulfill the obligation.

The research also works in assisting academic field enter in making references and

sensitizing the public about the results on any epidemic or danger that can occur and

harm the human life. Also this research may help in showing weaker areas when

institution is not doing well in some fields, so it may bring improvement and evaluations

of performance. The research assesses the methods of data collection and record

keeping which adds the value and importance of reviewing literature.

CHAPTER TWO.

LITERATURE REVIEW.

2.0 Introduction

This chapter intends to review the concerned literature that has been gathered and

documented by previous researchers that was regarding with the challenges met by

the Administration of the hospital which are mostly associated with political,

transmission and intervention social, financial and psychological factors.

The influencing factors or problems that faced by Administration and management

of Mubende Regional Referral hospital.

2.1 Political intervention

In Uganda the health workers politically has been characterized as thieves the

people who sell drugs, sundries and getting bribes from the patients, its published

through different medias local politicians ..

The hospital since the time of its infant stages was governed by political bodies

since the stability of Uganda is un certainty the recruitment has been a political

appointment source which implies that if the government is overthrow the hospital

will be on stand or waiting restful of the newly government. Also because of political

appointment it brings to recruit people who lacks qualification that put the hospital

to have unqualified personnel. Due to political appointments these workers was

usually kept on intimidation hence creating fear or sometimes corruption of highest

order hence poor quality of service delivery.

There also abusive political debates on local media. In local areas like Mubende find

several radio station carryout discussions on the air with proper procedures which

creates biasness between the local people and health workers.

The challenges faced by hospital According to the records obtained indicate that

there was lack of national unity between the patient and health workers to meet the

obligation of the government.

Mubende Regional Referral hospital also faced the problem of misunderstanding

between the central ruling government and Buganda Kingdom as foundation body

were funding has been too minimal due to two forces. These challenges on political

ground require a well organized and a sensitized community to keep hospital free

from religious political and traditional beliefs corruption.

2.2 financial challenges.

The financial situation of Mubende a final referral hospital it is known to be poorely

funded institution than other facilities yet it services every wide range of population,

hence the community almost 80% are below poverty line compared to the Uganda

emphasis. On helping the poor mothers who die in the struggle of delivering.

However the non government organization has played a grate role in uplifting the

standard of the economically, financially and social life in terms of development. In

support of this organizations like UNICEF, World Bank, who, Usaid, dish, Gtz, EDF,

RHP, UNFPA and Danida and Amref as development partners of Uganda government

to uplift the health sector.

The hospital is affected by financial problem due to abolition of cost sharing and

since Mubende has been acting as district hospital could only receive little funds

from the central that can not meet the problems of the met by Mubende hospital

report of united nation organization in 1991 that indicated 2/3 of the citizen are

below poverty line which makes the local people of target area to meet their survival

in medical requirements.

The Contributing factors that responsible for the inadequate financial support to

Mubende Regional Referral hospital poor planning and budgeting thus lead to poor

management style. The challenging factor that minimize the system that under go to

streamline the management is poor data collection and poor.

Poor record keeping thus leading poor planning and budgeting hence causing poor

management techniques.

Income social welfare of staffs in Mubende regional hospital has caused insecurity

and poor handling of clients due to the poor motivation the quality health services

delivery through emotional temperaments and extortion of money from clients is

tremendously increasing hence poor service delivery is created

2.3 Infrastructure challenges.

Mubende hospital has met many challenging factors in infrastructures in that most

of the buildings are too old and no renovation has been done and the hospital

development plan was drawn longtime ago this happens both in office premises and

staff quarters and most of the leaking.

Policies are not favor and the find the housing institutional is at 30% most of the

staffs are renting from their salaries which makes on high standard of living.

The staffs are poorly motivated and this affects the quality of lack service delivery

and leading to high turn over of staff. Poorly motivated and hence leading to lack of

qualified staffs.

The contributing factors responsible for infrastructure in hospital un availability of

office equipments and old all are Most of this have broken down.

2.4 Administrative challenges

The hottest factors that affect the management style in Mubende regional hospital is

political interference this cuts across the board in day to day activities, where we

find most of the female staffs are married by politicians others are relatives and

friends who interfere with management and administration due to internal forces.

The poor health has been noted as one of the most challenging factors the

international organizations has solutions of how health workers over come the

problem of overwhelming of patients to medicines the number of patients/clients

increasing tremendous due to poor facilitates surrounding the hospital that can not

meet the problems of communities and poor laws of Uganda concerning with the

ministry of migration and settlements. Let the medical care are inadequate supply

and expensive at most of the people in rural area are below poverty line.

Also administratively it is challenged with the issue of corruption in public utilities

like drug abuse; extortion of money from clients, selling of medicines and sundries

has affected the level of administration to meet the target of the government of

Uganda to its local people.

02.5 Staffing

According to the staffing level of the hospital the records available indicate that it is

recommended for 300 beds patients and special clinics however the staff calling is of

350 personnel both qualified and UN qualified. But current staffs available are 129

with gap of 122 staff which has totally contributed to the failures in administration of

hospital. However these issues has contributed for the downfall of hospital because

the ratio of health worker to a patient is quite abnormal when you compare that are

of Mubende hospital and for the ministry recommended thus this makes the health

workers to work tiresomely The hospital has three peers where by the medical

superintendent his the principal signatory

2.6 Capacity building

The training committee is used to select the staff to attend the trainings annually

based on level of the cadres the hospital adopted the system of the district to

release 6 staff per year for the further studies but few of them receive assistance

and the mode of the selection is un certain the capacity building of human resource

it is reportedly non functional due to the lack of fund on that vote of training

however some staffs like mechanical staffs have benefited that non medical staffs.

2.7 Transport system

Mubende Regional Referral hospital has the problem of transport which is poorly and

hence affects the management style of institution the referral hospital is supposed

to have the administrative vehicles Ambulance for outreaches, emergency vehicles

and motor cycles the institution has only one Ambulance which puts the

management on risk and affects service delivery. And transport is seen as the

contributing issue that has disorganized the institution for its progress and meeting

it target.

2,8 Library and record keeping

The hospital has well established library for staff to go for review whatever case or

to consult when there is challenging issue also used as revision room The library

even for the purpose references and headed by records officer at facility addition

the hospital has both clerical records and health management information system.

2.9 Retirement age.

The challenging and contributing factor on inefficiency and ineffective service

delivery is that the health workers at their exit it has been identified that are

handled poorly and most of them do not get their packages and gratuity which also

has affected the service delivery because the employees most of their time is spent

on their own business prepare for their own old ages with the view that the

government at last year it will not consider their service at the attainment of 60

years

2.10 Developmental factors.

Being Mubende at the stream end of central region and well located it is indicated

by census of 1992/2002 that every year it has 3% of population increase of the new

settlements thus coming with new development plan and ideas.

According to the reports available Mubende Regional Referral hospital started as

dispensary and become upgrading into current stage of referral being in changing

status at different levels it has been achieving a lot assets and personnel to change

the altitude of the others.

2.11 Performance level.

Available record indicates that Mubende Regional Referral hospital with that trying in

hardship to keep staffs willing the medical ethics and following the dressing codes,

coding of conduct that keeps he staff in professional manners through the provision

of assets has been poorly done although mubende hospital normally is graded in the

most best performing district.

2.12 Cleanness of environment

With the first appearance at the entrance the hospital gives a good picture and the

impression the one get implies that the performance level of management it really

working because the grass is short every time, building clear and although are old in

structure

2.13 Smartness

Generally the staffs are devoted to the dressing code of health professional and the

male do as public officers also adhering code of conduct of public service

CHAPTER THREE

METHODOLOGY

3.0introduction

This chapter mostly focuses on the rules and procedures of presenting, justifying,

analyzing and unifying the information under the subjects involved on research design,

area of study old study population analysis, methods of collecting of data in

government institutions sample selection, the size the method of collecting data, the

size where data to be collected and interpretation and analysis.

3.1 Research design.

With the purpose of data collection as the reliable methods the whole study used the

both theories and descriptive systems of research design that involves a well known

system in data collection and presentation of information to produce a clear picture of

the situation at hand. In this view it will cover a various changes frequently the

performance of the hospital to be perfective it will consider informative actions both

independent and dependent variables.

3.2 Area of study.

The study will be carried out in Mubende Regional Referral hosp The official catchments

area of Mubende Regional Referral hospital it covers the Mubende districts with the

total population of people 792434 and Mityana district total population of people

596472and Kiboga district total population of people 684692 officially catchment's area

however it serves the districts that has no district hospital like Kyegegwa. Kyenjojo,

Kibaale, Ggomba parts of Ssembabule there all came to Mubende to receive the medical

assistance from Mubende hospital that makes over tasking the hospital workers.

Mubende district in extreme end of central regional of Uganda approximately 100 miles

away from Kampala the capital city of Uganda.

3.3 The Target population

Mubende Regional Referral hospital has the catchment's area worth 2,00,000

population that officially covered Mubende, Kiboga and Mityana according to

available records from planning units basically it is hospital for human being and

comes rehabilitate and prevent the lives of the people the area is a attractive to

almost all the tropical savannah diseases the area mostly occupied by Baganda

indigenous people Bayoro, Baafumbira, Bakiga, Banyarwanada, Barundi, Basoga,

Banyankole, Bakonjo, Baba, Bateso, Bacholi, balongo Bagweri Baluli, Batara etc,

which has put the area at high rate of contracting sickness

3.4 Sample size and techniques to use

Sample size

The study considered 100 staff as respondents whereby both male and women will

be chosen by the use of random sampling techniques the size is preferred by the

reaches because she finds it very convenient and too adequate enough for purposes

of both qualitative and quantitative data interpretation, presentation and analysis.

3.5 Sampling techniques.

The methods to used be two the random sampling and purposeful sampling

whereby the researcher will automatically use a well systematic random sampling

because it can be done without any personal biases or hatred.

Method of sampling will be adopted very easily for the researcher because it can

identify key respondents who can provide rightful information in right time and right

place.

3.6 Data collection methods

The researcher used four methodologies to obtain data from the respective areas.

These will cover personal interviews, questionnaires reviewing records and

observational method.

3.7 The personal interviews.

Here the researchers carried out personal interviews with individuals using an

interview guide. This method is appropriate and convenient for the illiterate semi

literate respondents through this system the researchers free to use the present

respondent questions personally. Basing on the interview guide and question design.

3.8 Questionnaires method.

These are forms which were designed in format and contains asset of questions

which the researcher will submit to her respondents to extract information the

method is more convenient for the elite class who can read and interpret.

3.9 Reviewing of records.

Also the researcher! reviewed records that are contained and concerned with the

required information to confirm that the information got from the respondents are

rightful this which consists the researcher to confirm and produce well detailed and

criticized information.

3.10 Observation data collection

In this method data was obtained from the researcher where by this methods is

very appropriate and more convenient because the information collected is done on

researchers own observation and the system is consists observation and description

of events and behaviors within a given population. And gives the researcher to hear

and conduct also seeing and use of experiment in order to produce valid

information.

3.11 Data processing and analysis

Data processing.

In this context both qualitative and qualitative methods were used to process data

and below are their explanations.

Qualitative the researcher here identifies topics of study and will analyze data during

the process of data collection before and after the interview guide the researcher

will be required to check whether the final answer can match the formatted

question.

Quantitative method data will be collected edited coded and tabulated and

interpreted through editing the researcher will be assured that the questions have

answers of completeness, consistency, uniformity very accuracy, legibility and

comprehensiveness through coding

The researcher respected the answer to a very essential information and then

arrange properly accordingly for simple interpretation and representation.

3.12 Data Analysis

Data was collected, interpreted presented and analyzed basing on the major

themes and code of categories of the study which include the descriptions that

might influence political financial social sector that led to influence the challenges of

management of hospital to realize and intervenes very quickly for making the

proposal and be ready for presentation.

CHAPTER FOUR

Data Presentation, Interpretation and Analysis of the key Findings

4.0 Introduction

In this chapter an attempt was made to interpret, analyses and carryout discussion in

all the findings as was conducted in field of study.With these findings in effect

contained the effects of management and administration challenges with in Mubende

Regional referral Hospital the quality of service provided.

Characteristics of the sample

In the field study of research, it covered a total of one hundred respondents ten (10)

were part of administration and Finance department. 20 were in Nursing section in

senior level, 20 were enrolled nurses, 20 were in Mid-wife section,10 nursing assistant

and ten support staff and technical officers from all these respondents primary data

were collected through informal interview and focus discussion so that the researcher

identify the way hospital management conduct and treat its administration and

management styles.

In this research other respondents include the patients and charts to hospital

management board. Hospital host District Mubende Local Government from top

management level of the District and Ten (10) from the Elderly on behalf of Buganda

Kingdom. All these respondents were adults because they are experienced and with

Hospital history all these comprise fifty (50) respondents.

According to the above table it shows how the management department of Mubende

Regional referral hospital, they conduct a fact the hospital execute its duties, it shows

the hardships and the challenges that met by the administration and entire body of

staffing the respondents arguments in research findings indicated the nature of work

done by the health workers are risky needs a lot of concentration because in the

interview, the researcher findings reviewed that the hospital has inadequate funding

which cannot sufficiently be enough to run the hospital.

Under staffing

The hospital records indicated that the staffing level is recommended as 360

establishment, however the current staff available its 46% which stress the service

provider for over tasked and the hospital is 300 bed occupancy inpatient

accommodation.

The available information indicate the hospital has no records beyond 20 years back yet

its one of the oldest hospitals in Uganda but currently the records managed to meet the

focal person in record. This according to the respondents has made it relentlessly

difficult to comprehend with a situation leading to in effectiveness in service provision.

Transport System

The researcher reviewed that the hospital has got the problem for transport means

which effect the Management executing the duties effectively. In reply of respondents

CHAPTER FIVE

SUMMARY, CONCLUSION AND RECOMMENDATIONS

5.0 Introduction

The research aimed at how the quality of Management style within Mubende Region

Referral hospitals has been effected where the study is conducted. It used a sample of

one hundred respondents from different departments and field data were collected

through informal interviews with staff, Management board, District leaders and Elders

from Community by using questionnaires and were administered to the respondents.

5.1Summary

The respondents generated various findings as was summarized below;

During the research, most of the respondents reported more than one problem that

were experiencing shortage of drugs and sundries within the hospital and yet the

Government emphasizes that the drugs are available.

The researcher also noted the hospital receive inadequate funding from the central

government and other stakeholders like world bank, Japans Government ,and non

nongovernmental organizations.

Even the researcher reviewed that most of the people are on dual thinking mostly

concentrating on local medical due to the traditional beliefs here researcher reviewed

that there is much more needs as sensitization as the government pro grammes.

There is also a problem of male staffing due to shortage of staff within the hospital and

labour turn over is very high due to poor motivation and also poor policies policies

imposed on staff.

So many factors were discovered that lead to influencing the challenges faced by the

management and administration in duty execution and outcome to be effectively in

service delivery due to rapid population growth within the locality.

However the nature of work and the roles in management style being as potential

factors as the department in providing quality work due to quality of personnel

employed.

Also the study identified that the staff have different attitudes towards the mission and

objectives of the organizationals that are put into place for quality service delivery.

Even the researcher reviewed that still may difficulties as far as put into consideration

that health workers provide themselves staff uniform, buying code of conducts.

5.2 Conclusion

the management challenges in Mubende Hospital is a social economic, political and

psychological problems that should not be neglected without paying much attention

minimized there by improving the general capacity and technical support of the

institution in order to utilize little resources and practice fair and justice within the

facility which can bring peace mind at work to avoid the social instability and examine

unfairness so that Ugandan can get better service delivery by avoiding inadequate

supplies to the facility and reduce as starvation as both for public and sector and

private support for evaluation.

It also brings the view of researcher that by helping both women and men who visit the

unit to get treatment in provision of treatment the researcher noticed that for better

Referal Hospital and its effects on the quality of human social life in management and

administration of human resources within the health facilites as general. This has been

as existing in health workers as results of poor quality and facilitates poor practices at

Public Offices also the implementers explains more that the government of uganda has

to play a big role and fulfill its obligation to the people of Uganda. Despite the exisiting

factors on the ground, where by the implementers mainly viewed that it may be lack of

inadequate resources to run effectively the government business which is limited to

serve the grass roots as people efficiently due to inacceesibility of remote areas where

the out reaches are hard to reach.

5.3 Recommendation

There is an urgent need for a number of actions to be implemented to address a certain

issues about management challenges faced by the administration and management and

its style as was highlighted in the research to avoid a portential crises that may occur in

future as it is unlike that Mubende Hospital will mobilize enough resources and to

consolidate capacity and incentives to the extend of a meaningfully found to meet the

needs of the increasing problems to the hospitals which requires the serious planning

and policies to restructure the existing culture within the institution for both public

offices like National Development Plan and non governmental organisations and all

stakeholders that may have strong impacts in evaluation and monitoring of health

sector particularly Mubende Hospiatal. Should be noted how ever

the hospital. This according to the research will develop a lasting partnership that could

lead to a host of benefits accruing from multilateral partners in the provision of
