SIXTH FORM PATHWAY 1 APPLICATION
TRADITIONAL (GRADE 12)
The Sixth Form programme requires a high standard of work and any student who wishes to be successful
needs to be fully committed to the set curriculum for each subject area. Therefore, applicants are advised
that the programme is geared to those who are disciplined and hardworking in order to maximize their
full potential. ALL successful applicants who fail to adhere to ALL the rules and regulations will be
IMMEDIATELY removed from the programme and WILL NOT be promoted to Grade 13.
ACADEMIC REQUIREMENTS
Successful applicants to Grade 12, should have:
Five (5) or more subjects at the CSEC General Proficiency Level.
A pass in English Language and Mathematics.
City & Guilds Stage 3 with distinction in English Language and Mathematics.
Grade 1 or 2 in the subjects they wish to pursue in Grade 12.
NON-ACADEMIC REQUIREMENTS
Good conduct
Accumulation of 80% attendance in Grade 11
Involvement in Extra-curricular activities

GUIDELINES FOR SUBMISSION OF APPLICATION FORMS
1. Completed application form along with the processing fee receipt of $1, 500 (non-refundable) is to be
submitted to the coordinators at the Old Harbour High School on August 30 &31 2021 between the
hours of 10A.M. and 2P.M.
2. Students’ applications will be reviewed for the non-academic criteria.
3. Academic assessment will be done upon the submission of the CSEC/City & Guilds results.
N.B. Results must be submitted at the school to the coordinators, the next work day, after they
have been released.
4. An acceptance list of students who have met the requirements to the programme as well as those who
will be interviewed will be posted on the School’s Notice Board on a date to be announced.
5. Interviews will be conducted on Tuesday, October 5, 2021 @ 9A.M. Students may be interviewed to
receive assistance with career guidance and subject selection.
6. The list of successful students from the interviews will be posted on the School’s Notice Board on
Wednesday, October 6, 2021 @ 5P.M.
REQUIRED DOCUMENTS
Two (2) recent recommendations. (Transfer students only). The document should not be older
than six (6) months.
Two (2) recent passport-sized photographs. (No Hair Extensions)
Copy of Birth Certificate.
Copy of CSEC results
Copy of CAPE results (Transfer students applying for Grade 13)
Copies of last two (2) school reports. Applications will not be processed without these.
(DO NOT SUBMIT ORIGINAL DOCUMENTS.)
Transfer Forms (Sold at the Principal’s Office). This form must be completed by all
students applying from other schools. The form is to be signed and stamped by the principal
or vice principal of the applicant’s previous school and submitted along with the Sixth Form
Application.

School Reopens: Wednesday, October 13, 2021 @ 12:10P.M.
Students MUST complete 72 hours of Community Service and the Research Methods course.
At the end of the 6th
Form Programme, a student may be eligible for an Associate Degree.
STUDENTS MUST DO INTEGRATED MATHEMATICS ALONG WITH OTHER
RELATED SUBJECTS TO BE ELIGIBLE FOR THE ASSOCIATE DEGREE.
Associate Degree handbook is sold at the principal’s office. (Must be used for guidance in
selecting appropriate subjects)

CAPE COURSE OPTIONS
At the CAPE level, students are allowed to pursue a maximum of four (4) subjects. These include
Communication Studies or Caribbean Studies which are COMPULSORY subjects. Students are required
to do both, one in each year of the programme.
1. Students must select a maximum of FOUR (4) subjects from either Band 1, 2 or 3. The choices MUST
include ONE of the compulsory subjects. DO NOT select across Bands.
2. A student must have the subjects at the CSEC Level in Grades 1 or 2 to be qualified to pursue them at the
CAPE Level.
