Assisted Living Facilities

A combination of housing, personalized supportive services and health care designed to respond to the individual needs of those who require help with activities of daily living (ADLs), but wish to remain as independent as possible. An intermediate level of care between independent living and nursing homes, assisted living facilities are for those who are not able to care for themselves, but do not require constant care. Most facilities offer 24-hour supervision and an array of support services to ensure residents health, safety, and well-being.. An Assisted Living Facility helps residents with personal care, such as: bath-ing, dressing, toileting, eating, grooming, getting around and medication assistance. Residents and/or families pay for services, although private insurance may pay for some services.

Home care providers (or private duty care companies) provide home care aides, companion care, homemaker services, assistance with activi-

ties of daily living in the client’s place of residence (i.e. home, hospital, assisted living, or nursing home). Hospice Care Services

Hospice is a method of caring for the terminally ill that helps provide comfort and support to patients and their family members when an illness no longer responds to cure-oriented treatments. Hospice neither pro-longs life nor hastens death. The primary goal is to improve the quality of a patient’s last days. Hospice care can be provided in the home, hospice facility, nursing home, or hospital. Medical, social, spiritual, and counseling services can all be part of hospice care. Medicare and many private health insur-ance programs cover hospice care but require specific patient eligibility, including certification by a physician that the patient is terminally ill with 6 months or less to live based on knowledge about the normal course of a specific illness.

Recreation – Art, crafts, hobbies, games, and trips.

Wellness – designed to support/improve the senior’s mental/physical

well-being through exercise, physical fitness, and health screening.

Family Care-Giver Support – support services that provide a temporary break in the tasks of care-giving. For family caregivers who are providing care for an older individual who is determined to be functionally impaired because of inability to per-form instrumental functions of daily living without substantial supervision and assistance. This service is provided to persons caring for a homebound relative 60+, for a relative 60+ caring

for a homebound child or grandchild.Information and Assistance –

Provides the individual with current information on opportunities and services within the community.

Legal Assistance – providing legal advice, counseling, and representation by an attorney. Lectures are scheduled on a quarterly basis.
