ภาวะช็อก
Shock is the clinical syndrome that results from inadequate tissue perfusion.
ภาวะช็อก (shock) หมายถึง กลุม่อาการท่ีเป็นผลจากการลดลงของการไหลเวียนโลหิตสูอ่วยัวะและเนือ้เย่ือสว่นปลาย (inadequate tissue perfusion) ท าให้เนือ้เย่ือสว่นปลายได้รับออกซิเจนและสารอาหารไมเ่พียงพอ สง่ผลให้เซลล์ขาดออกซิเจน เสียสมดลุระหวา่งออกซิเจนที่ขนสง่ไปที่เซลล์ (oxygen delivery) กบัความต้องการใช้ออกซิเจนท่ีเซลล์ (oxygen consumption)

Trauma
Warmed fluid bolus of isotonic fluid 1,000 ml ในผู้ใหญ่และ 20 ml/kg ในเดก็ท่ีน า้หนกัน้อยกวา่ 40 kg และประเมินการตอบสนองตอ่การให้สารน า้โดยแบ่งเป็น 3กลุม่คือ rapid response, transient response และ no response

Case 1
ในผู้ป่วย post-cardiac arrest รายหนึ่ง ต้องการให้ adrenalineขนาด 0.1 ug/kg/min จะให้ยาในขนาดเท่าใด (ผู้ป่วยหนัก 50 กิโลกรัม)
Dose ของ คือ adrenaline 0.05-0.5 ug/kg/min (MAX dose 20-50 ug/min)การผสม epinephrine นิยมผสม 10 mg ใน NSS 100 mL (1 vial = 1 mg ทั้งหมด 10 vial หรือ 10 amp น้ า 100)
= 3.0 ud/minค าสั่งการรักษาAdrenaline 10 mg in NSS 100 mL IV drip 3.0 ud/min (0.05 ug/kg/min)
100 × 1/10 (mg/mL)ud/min = 6 ×50(Kg) × 0.05(ug/kg/min)
หลังได้ Adrenaline 10 mg in NSS 100 mL IV drip 3.0 ud/min (0.05 ug/kg/min) ยังมีภาวะ shock สามารถให้ยาขนาดสูงสุดได้ rate เท่าไร
Maximum dose adrenaline คือ 0.5 ug/kg/min
= 15 ud/min
Case 1
100 × 1/10 (mg/mL)ud/min = 6 ×50(Kg) × 0.5(ug/kg/min)
ผู้ป่วย Complete heart block มี symptomatic bradycardia ไม่ตอบสนองต่อการให้ atropine ต้องการให้ dopamine เพื่อเพิ่ม heart rate จะต้องให้ dopamine กี่ ud/min (ผู้ป่วยหนัก 70 kg)
Preparation : 250 mg/ 10 mL (1 vial = 10 mL)นิยมผสม 1:1 2:1 3:1 4:1 Dose 0.5-20 ug/kg/minควรเลือก dopamine ขนาดตั้งแต่ 5 ug/kg/min ขึ้นไป
= 21 ud/minค าสั่งการรักษาDopamine 100 mg in NSS 100 mL IV drip 21 ud/min (5 ug/kg/min)

ผู้ป่วย septic shock ได้ NSS 30 ml/kg ยังมี ภาวะ shock ท่านจะให้ vasopressors ชนิดใด ในขนาดเท่าไร (ผู้ป่วยหนัก 50 kg)
Firstline Vasopressure in septic shock norepinephinePreparation : 4 mg/ 4 mL (1 vial = 5 mL แต่ยาที่อยู่ใน vial = 4 mL)นิยมผสม: 4:500 4:250 8:250 Dose 1-50 ug/min หรือ 0.02-2 ug/kg/min (Max dose 35-90 ug/min)
ud/min =6 × 50(Kg) × 0.05(ug/kg/min)
100 × 4/250 (mg/mL)
= 9.375 ud/min 10 ud/min ค าสั่งการรักษาNorepinephrine 4 mg in 5% DW 250 mLIV drip 10 ud/min (0.05(ug/kg/min)

Septic shock
ABC Obtain blood C/S, serum lactate Begin antimicrobial therapy; attempt surgical drainage Administer 30 mL of crystalloid/kg and titrate infusion
based on dynamic indices volume responsiveness urine output.
If volume restoration fails to improve organ perfusion, begin vasopressor support with norepinephrine,
infused up to 30-70 ug/min
Treatment
Management of Sepsis and septic shock
Diagnosis Take microbiologic cultures (H/C x 2, Urine C/S) be obtained before starting antimicrobial therapy
Initial resuscitation
Crystalloid fluid at least 30 ml/kg in first 3 h.following initial fluid resuscitation, additional fluids be frequent reassessment of hemodynamic status.
Fluid therapy Use crystalloid, consider albumin, avoid HES
Antibiotic therapy empiric broad-spectrum ATB with in 1 h. avoid combination therapy
Source control As soon as feasible (with in 12 h.)
Vasopressor Use norepinephrine, avoid dopamineAdd epinephrine if NE inadequate
Corticosteroid No steroid, unless shock refractory to adequate fluid and 1 st vasopressor
Glucose control Target an upper blood glucose ≤180 mg/dL (Keep 140-180 mg/dL)
Bicarbonate Against the use of sodium bicarbonate if pH > 7.15
Goal Target MAP 65 mmHg, normalized lactate

Cardiogenic shock
Ameliorate increased work of breathing; provide oxygen and positive end-expiratory pressure (PEEP) for pulmonary edema.
Fluid challenge if no sign of volume overload
Begin vasopressor or inotropic support; • norepinephrine (0.5 μg/min) to maintain SBP• dobutamine (5 μg/kg/min)
Seek to reverse the insult (eg, thrombolysis, percutaneous transluminal angioplasty).
Consider intraaortic balloon pump counterpulsation (IABP) for refractory shock.(not routine use)
Short-term mechanical circulatory support may be considered
