THE SOUTHWESTERN SURGICAL CONGRESS 2013 FALL NEWSLETTER Dear fellows, residents, and guests of The Southwestern Surgical Congress, The 66 th annual meeting of The Southwestern Surgical Congress is scheduled at the beautiful Westin Kierland Resort in Scottsdale, Arizona from April 13-16, 2014. We hope you are all planning to attend our outstanding annual meeting. Dr. Shanu Kothari, Program Chair, and I are planning an informative and stimulating meeting. The deadline for abstract submission is October 25, 2013 and abstracts will be considered for full podium, quick shot, poster and video presentation. Don’t forget…residents who present full podium presentations can compete for the Jack Barney Award. The scientific quality of the meeting is dependent on the quality of the work submitted and we look forward to reviewing all submissions. Additionally, we are planning (2) outstanding postgraduate courses on the current Management of Hernias as well as Updates in Endocrine Surgery. The American College of Surgeons will again be participating with one or two informative panel discussions. I encourage ALL of you to make your plans to attend. The Southwestesrn Surgical Congress has a strong tradition of educational and collegial meetings combined with a wonderful social program. The Presidential address for the 2013 Annual Meeting was delivered by Dr. David Antonenko and was entitled “Is General Surgery Becoming Obsolete?” This was a review of the current information which suggests that the majority of general surgery graduates proceed with fellowship training rather than going directly into practice.

As introduction he related 2 topics which he initially planned on presenting, new information on fluids, electrolytes and body composition affecting surgery and surgery of the morbidly obese. However, concerns regarding recent reports of shortages in general surgery, particularly in rural areas and reports of increased numbers of general surgery graduates proceeding with fellowship training were the basis for his presentation on the future of general surgery.

Dr. Antonenko reviewed the information available as to why up to 80% of recent general surgery graduates proceed with fellowship training. He noted the increase in the number of fellowships available, the market forces directing residents towards fellowships and the personal reasons for pursuing a fellowship such as less night call, a perceived better income, or personal interest. He also reviewed recent reports suggesting that there is a substantial difference in attitudes towards fellowship training between academic and community programs, whether there was a fellowship associated with the residency program, whether the resident was single or married, male or female and if

residents had children. Residents from academic programs, those with associated fellowships, female residents and those resident with families were more likely to pursue fellowship training.

He pointed out that most of the programs that have associated fellowships are also the ones with the highest numbers of their graduates pursuing fellowships, that program directors that have fellowship training may be influencing graduates to proceed to fellowship training and that there are misperceptions regarding possible financial benefits of fellowship training. He reinforced the suggestion in the literature that in many programs fellowship training is a reflection of fellowships supporting themselves and not providing adequate training to the residents such that residents feel they need additional training.

There is an increasing and critical shortage of general surgeons in all areas of the country but particularly in community and rural areas. He suggested that the RRC’s and the American Board of Surgery need to further evaluate those programs that appear to be giving less than adequate training to general surgery residents, to determine whether there is undue influence of fellowship programs on general surgery resident education and also to begin discussions of how many fellowship trained surgeons in each specialty are needed. His conclusion is not only is the general surgeon not becoming obsolete but that the general surgeon must remain the strength and foundation of surgery.

Original investigations cover all areas of surgical practice, and we anticipate tremendous resident, fellow, and medical student participation in the paper presentations as well as quick shot and poster presentations. We had 180 abstracts submitted last year for the 2013 Southwestern Surgical Congress. The Program Committee did an excellent job of selecting 32 of the top abstracts for eventual publication in the December 2013 issue of the American Journal of Surgery. All the solicited manuscripts were submitted and edited in a timely manner.

I would personally like to extend a sincere thank you to the Publications Committee (Richard Barton, Jeffrey Bender, Anees Chagpar, Michael Cornielle, Robert McIntyre, Susan McClean,

Laura Moore, and Rob Todd), all the authors, and ad hoc reviewers who contributed to the effort of getting all the manuscripts ready for December 2013 publication.

We are excited for the 2014 SWSC meeting in Scottsdale, AZ. I would like to remind all authors that manuscripts accepted for a general session oral presentation must be submitted for publication to the American Journal of Surgery. This year’s manuscript submission deadline is Friday, March 28th I would also like to remind all members that you are encouraged to contact me if you would like to help as an ad hoc reviewer of manuscripts for the 2014 journal!
