Prophylaxis and Aspergillosis –Has the principle been proven?
• Most cases o probable aspergillosis based on a positive test foraspergillus galactomanan antigen (not positive culture or histopathology)
• Remove these probable cases and the advantage for posaconazole vanish
• Similar cases for which the galactomanan test was negative in the posaconazole group?(antigen concentrations are effected by antifungsl drugs)

ConclusionsPosaconazole prophylaxis significantly reduces incidencesof IFI in high risk AML patients. Additionally, infectiouscomplications such as pneumonia and GI-tract infectionsare also reduced, while mortality rates, days ofhospitalization, anti-infective and overall costs are notreduced. In areas/surroundings with high rates of fungalinfections (e.g. due to intensive building (re-)construction), primary prophylaxis with posaconazole isrecommended.

• Eine Prophylaxe während der Induktionschemotherapie einer AML ist sinnvoll, wenn die lokale Inzidenz von Pilzinfektionen über 5% liegt
• Eine adäquate empirische Therapie erscheint eine mögliche Alternative, auch unter Kostengesichtspunkten?
• Eine präemptive Therapie geht mit einer höheren Rate an IFI einher, so dass eine vergleichbare Überlebens-wahrscheinlichkeit bei AML Patienten in Neutropenie fraglich ist.
