Step 2: Select Department Portal. This will take you to the Login page.

Step 5: Enter new subscriber information.First NameLast NameUser ID is optional

Step 6: Select Department: Drop down menu with a list of all

of your departments.Subscriber Group: In most cases this will be

Step 7: Select Service Type

Step 8: Enter New location information.BuildingRoomFloorJack

For Telecom Admin Use

Step 10: Select date you want the service installed.

Step 11: Enter any additional information.- New number will not be published in the Mason directory, provide the phone number that will be published.-New number will appear on other phones.-Who to see for access.

Step 13: Add to Cart OR to enter additional requests

Install new service. See Pam Larkin for access. Published number will be x3-1234. Program main number and rollover as ringing appearance on x3-2345.
