To establish, implement and maintain an efficient and effective quality system that achieves the quality objectives and meets Contractual and Project’s requirements.

PROJECT QUALITY PLAN

1.0 INTRODUCTION:-

The Project Quality Plan (hereinafter referred to as the Plan) defines the Project Quality System for BATIMAT BUILDING MATERIALS L.L.C (hereinafter referred to as Subcontractor) and is the principal quality document on the project. The Plan describes the Subcontractors Project organization and responsibilities for the performance and completion of Project functions.

2.0 PURPOSE:-

The Purpose of the Plan is to define the Project Quality System, to ensure and demonstrate that the materials, equipment and services provided under the control of the Subcontractor conform to the requirements of the project The Plan demonstrates that Project Management has specified the Project policies and objectives, established a system of procedures to accomplish them, assigned duties, delegated authority and set up suitable inspection, examination and audit programs to verify that the requisite standards of performance are achieved.

3.0 SCOPE:-

The quality system described by the Plan shall apply to all activities within the Subcontractor’s Scope for the Project, from Project Kick-off to final completion. The Plan shall apply to all sub-tier personnel assigned to the Project, and to the associated resources and facilities employed on the Project. The Plan also addresses control of sub-tiers who shall be required to Comply with quality system requirements consistent with their scope of work.

The scope of services for the Project include the following activities:

Project management services, including materials management and automation. Quality Assurance. Business management, including planning and scheduling, progress reporting,

trends and variation processing. Engineering services, including detailed design / shop drawings.

4.0 PROJECT QUALITY SYSTEM:- The Project Quality System shall be applied to all Subcontract activities including the

overall project management and control, planning, engineering, procurement, construction, installation and all related monitoring and inspection.

4.1 Source Document and Precedence Levels: The Project Quality System shall be implemented through three levels of documents: Level 1 – The Project Quality Plan defines the project Quality Policy and Objectives and

demonstrates how the quality system is applied to meet the project requirements. Level 2 – The Project Procedures, prepared and issued by the Project Manager, shall

specify the responsibilities, requirements for implementation of various works activities, delineation of Contractor’s internal interfaces as well as interfaces between client and the Contractor and the Contractor’s Subcontractor (s), as appropriate.

Level 3 – Inspection & Test Plans and Quality Control Procedures shall define, in detail, the

8.0 PROJECT QUALITY SYSTEM AUDITS:-

The QA/QC Officer shall prepare, issue and maintain a Project Audit Schedule of quality systems.

The audits shall be performed by or under the direction of the Q.A. / Q.C. Officer at various stages, throughout the duration of the project.

The audit schedule shall recognize initiation of new work activities, to ensure that the project Quality System is being implemented effectively. This will act towards preventing problems arising at a later stage, which might have adverse effect on project completion.

The result of the audits shall be documented and corrective action requests shall be raised as appropriate, to require implementation of necessary remedial and / or corrective and preventive actions.

INTRODUCTION Accidents take place due to unsafe practices and unsafe working facilities. Hence by continuous and vigilant efforts, accidents can be minimized to a very great extent. The most important point in unsafe actions and unsafe mechanical or physical conditions are the immediate causes of accidents. The main drive force behind Safety movement is that accidents are expensive. Substantial savings can be achieved by preventing them. The Health and Safety Manual forms the basis of the project Health and safety program covers the health and safety philosophy and regulations of the company. It also covers the method and system of work to be adopted on the project in accordance with the contract general provision. The Health and Safety Manual has been prepared to ensure the safety of people, equipment and materials to protect the environment. It is considered the minimum standard necessary to achieve the goal of accident prevention and it also requires whole hearted participation from higher officials to the worker level.

HEALTH AND SAFETY PROGRAM Safety program shall be implemented to prevent accidents that can result in injury to persons and damage or loss to equipment and property. The principles of accident prevention are as follows:

a) Accident prevention is an essential part of good management and good workmanship

b) Top management must be committed to take the lead in Organizing Safety in projects

c) Management and workers must coordinate wholeheartedly in securing freedom from accidents.

d) There must be a definite and known safety policy. e) The organization should necessarily carry out the Safety Policy. f) The best knowledge and method must be applied.

The main action principles in accident prevention concern the employer providing a safe place of work and a safe system of work. The health and safety programme shall be used to achieve the following:

a) The identification of hazards involved in the work b) Establishing standards within the project for the performance of the work c) Measuring the performance of the work against the established standards d) Evaluation of the performance of the work e) Correction of any deficiencies in the performance of the work or the established

standards, including the identification of key areas to be controlled where total elimination of hazards is not possible

Elements of health and safety program The elements of our safety plan are:

Establishing inspection and monitoring procedures to ensure conformance with the safety program and plan

GENERAL INSTRUCTIONS TO WORKERS ON SAFETY

Read thoroughly the safety hand book and other periodical instructions issued by the

management.

Special Safety instructions, if any, pertaining to each machine and equipment should be

studied carefully.

Follow all safety rules and regulations.

Never operate any machine without the safety device or guards.

Do not keep materials in aisles, walkways, stairways, gangways or other points of access.

Keep all tools in proper working condition.

Do not operate machines if you are not authorized to do so.

Do not stand or sit on the sides of moving equipment.

Avoid attending to work after consuming alcoholic drink.

Take adequate precautionary measures while handling harmful chemicals so that the

poison coming out from such materials is not ingested or inhaled or absorbed through

skin.

Wear all personal protective clothing and gear while on work.

Promptly report unsafe conditions of work to your immediate supervisor.

If injured or contracted any contagious or occupational disease, promptly report to your

supervisor and without delay. Obtain the necessary medical treatment.

Never deface or remove the safety posters, instructions etc.

Risk Assessment A risk assessment is the systematic identification of potential hazards in the work place as a first step of controlling the possible risks involved. A hazard is anything that has the potential to cause harm. A risk is the likelihood of someone being exposed to that hazard and harmed as a result. Risk assessment must be in place for all operations. The risk assessment should assess the risk that may be present in all works, activities and may identify particular area for more detailed specific assessments. Risk assessments are important for two main reasons 1. The process is a fundamental part of National Law 2. It manages potential dangers in the work place A safe system of work

A safe system of work is a considered method of working that takes proper account of the potential hazards to employees and to any others having access to work site. Before a safe system is drawn up for a particular operation the potential hazard should be identified and reduced to the greatest extent practicable.

FIRST AID 1. Where there are less than 50 employees at each site, the contractor must then ensure that

an appointed first aider is present to take charge of the situation if a serious injury or major illness occurs. An appointed first aider is someone of with experience and training in first aid who should be able to handle an emergency, summon help (call ambulance) and also be responsible for any first-aid equipment.

Where there are more than 50 employees including sub-contractor employees, the contractor shall ensure the availability of a full time trained first aider in each site.

2. The telephone numbers of Hospitals or ambulances and police shall be conspicuously pasted. 3. First aid box shall be placed in a clearly identified and readily accessible location. The

contents shall be protected from dampness and dust.

4. Where there are 250 or more persons at work, a suitable staffed and equipped first aid

room shall be provided and maintained. 5. The provision of first aid facilities shall meet the requirements specified in the EPSS (

Environmental Protection and Safety Section ) technical guideline. 6. All first aid cases shall be recorded in the accident register and shall be readily available.

PERSONAL PROTECTIVE EQUIPMENT General The contractor / employer shall provide at contractor / employer’s expense protective clothing including coverall and all other personal protective equipment required for different operations. The contractor or employer shall take necessary measures to ensure that protective clothing and other personal protective equipment are effectively worn. Head Protection Employees working in areas where there is possible danger of head injury from impact or from falling or flying objects or from electrical shock and burns, shall be protected by protective helmets. This includes all building construction and demolition sites. Hearing Protection i) Wherever it is not feasible to reduce the noise level s or duration of exposures to those specified ear protection devices shall be provided and used. ii) Ear protection devices inserted in the area in the ear or earmuff shall conform to international standards and shall be fitted individually Eye and Face Protection i) Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical or radiation agents. ii) Employees whose vision requires the use of correction lenses or spectacles when required by this rule to wear eye protection, shall be protected by goggles or spectacles of one of the following types :

a) Spectacles whose protective lenses provide optical correction b) Goggle that can be worn over corrective spectacles without disturbing the adjustment

of the spectacle. c) Goggles that incorporate corrective lenses mounted behind the protective lenses. d) Face and eye protection equipment shall be clean and in good condition The use of this

type of equipment with structural or optical defects shall be prohibited.

Protective Clothing Every contractor / employer shall provide adequate and suitable protective clothing for any person so employed, from hazards such as chemical liquids and vapors, excessive heat and cold, electricity, impacts, ionizing radiations and also general weather conditions. Reference shall be made to EPSS Technical Guideline for the selection and use of protective clothing for various operations. Hand Protection Every contractor / employer shall provide adequate and suitable hand protection for any person handling rough, abrasive, sharp or excessively coarse material such as reinforcement rods, precast concrete etc. toxic and corrosive materials, electricity, hot materials. Foot Protection Contractors / employers shall provide safety – toe foot wear to all employees engaged in construction activities, to protect against any possible foot injuries. Safety belts, Life lines and Lanyards

i) Lifelines, safety belts and lanyards shall be used only for employee safeguarding . Any life line, safety belt actually subjected to in-service loading as distinguished from static load testing, shall be immediately removed from service and shall not be used again for employee safe guarding. ii) Life lines shall be secured above the point of operation to an anchorage or structural member. iii) Safety belt lanyard shall be minimum of 1cm nylon or equivalent with a maximum length to provide for a fall of not greater than 2m

FIRE SAFETY 1. The contractor / employer shall be responsible for the development of a fire protection program to be followed by all phases of the construction work and shall provide appropriate fire fighting equipment as prescribed by the Civil Defence. As fire hazards occurs, there shall be no delay in providing the necessary equipment. 2. Access to all available fire fighting equipment shall be maintained at all times. 3. All fire fighting equipment shall be periodically inspected and maintained in an operating condition. Defective equipment shall be immediately replaced. The contractor / employer shall provide adequate fire fighting training to the Employees.

4. An adequate means of escape shall be maintained. Gangways, doors and ladders shall be kept clear at all times in view of the changing nature of construction operations, escape routes shall be frequently surveyed. 5. Fire extinguishers type, capacity and number as approved by the Civil Defence shall be maintained. 6. Portable fire extinguishers shall be inspected periodically and maintained in good operating condition. 7. Fire extinguishers shall be provided on each floor. In multi-storey buildings, at least one fire extinguisher shall be located adjacent to stairways. 8. Travel distance from any point of the protected area to the nearest fire extinguishers shall not exceed 100 feet ( 30 meters ) Causes for Fire a) Cleanliness and House keeping Rubbish and unsafe storage of materials often contribute to the spread of fires,

therefore general cleanliness and good house keeping are important. b) Electrical Equipments Electrical defects due to poor maintenance of cables, wires, switches, lamps, and hot elements can cause fire accidents. Electrical siring and equipment for light or power purposes shall be installed in compliance with the requirements of electrical rules applicable in U.A.E. c) Storage

It is important that storage of materials is maintained in a classified and orderly fashion. Particular care should be taken that materials are not stored near moving machines, electrical supply boxes or a source of heat. d) Open Flames Burning materials like paper, cotton, clothes, dry leaves etc are some of the common sources to produce fire accidents. Similarly fire chambers welding, cutting etc. cause open flames, if not controlled properly results in fire accidents. e) Smoking When a cigarette is lightened near flammable material storage area, fire is possible, the lightened match stick can cause the fire accident.

b) Dry powder or Carbon Dioxide :- On fires involving either liquids in container or spilled liquids, direct the discharge towards the near edge of the fire and with a rapid sweeping motion, drive the fire towards the far edge until all the flames are extinguished. On fires in falling liquids, direct the discharge at the base of the flames and sweep upwards. c) Foam :- Direct the jet at an adjacent vertical surface above the level of burning liquid. This breaks the jet and allows the foam to build up and flow across the surface of liquid. Alternatively stand well back and with a gentle sweeping movement allow the foam to drop down and lie on the surface of the liquid. d) Halon Vapourising liquid :- Same as for dry powder but the fumes from these extinguishers are toxic and should not be inhaled. This limits their use in confined spaces. Areas where they are supposed to be used should be well ventilated e) For class “ E ” Fire :- Usage is same as above.when the fire is in electrical equipment, direct the discharge straight at the fire or if the equipment is enclosed, direct the discharge into any opening. SAFE WORK PROCEDURE - FIRE SAFETY

1. Ensure safe access to and means of escape from site location

2. Construct temporary buildings like site offices and material stores with Non- combustible materials

3. Keep adequate safe distance between temporary building / caravans 4. Provide free space between each section of stored material 5. Store inflammable material separately 6. Remove combustible material ( packing etc) from site periodically 7. Clean site frequently at once a week 8. Display “ NO SMOKING ” poster in the area where inflammable materials are stored. 9. Inspect the workplace thoroughly where welding and gas cutting operations are to be carried out

SCAFFOLD SYSTEM SAFETY Provision of scaffolds etc. Where work can’t be safely done on or from the ground or from part of a Building or other permanent structure, there shall be provided, placed and kept in position for use properly maintained either scaffolds or where appropriate ladders or other means of support, all of which shall be sufficient and suitable for the purpose. Construction and Material 1. Every scaffold and every part thereof shall be of good construction of suitable and sound

material ad of adequate strength for the purpose for which it is used, satisfying international standards. The contractor / Sub-contractor shall be responsible for the design and safety of scaffold system.

2. Sufficient material shall be provided for and shall be used in the construction

of scaffolds 3. Scaffold construction should take into consideration the type of work load,

height and weather conditions. 4. Timber used for scaffolds shall be of suitable quality, be in good condition, and have the bark completely stripped off. 5. Timber used for scaffolds, trestles, ladders and folding step-ladders shall not be so painted or treated that defects cannot easily be seen. 6. Metal parts used for scaffolds shall be of suitable quality and be in good condition and free from corrosion or other patent defect likely to affect their strength materially Partly Erected or Dismantled Scaffolds No scaffold or part of a scaffold shall be partly erected or dismantled and remain in such a condition that it is capable of being used unless.

a) The scaffold as so erected or dismantled complies with there rules b) A prominent warning notice indicating that the scaffold or part is not to be used near

any point at which the scaffold is liable to be approached for the purpose of use c) Access to the scaffold or part as the case may be is as far as reasonably practicable to

be effectively blocked.

c) Do not put ladders on boxes, drums etc to gain additional height ( make shift ladders shall not be used ).

d) Get the appropriate length ladder and load it vertically only, placing the bottom of the ladder stiles ( legs) on affirm level base to avoid slip hazard

e) Do not load it from the side by pushing or pulling from another object horizontally. f) Keep the ladders clean and free of oil , dirt, grease and inspect for defects before use g) Before climbing a ladder, check for overhead hazards not normally encountered from

below h) Only one person at a time should climb a ladder, keeping your body positioned in the

middle of the ladder i) Do not climb or stand on the top two steps or rungs j) When using a step ladder (self supporting ladder), the user must not step onto the top or

onto the rung next to the top. Usually all step ladders have the highest standing level about indicated.

The top rung and the top should be painted yellow and black, and marked to show No Step ahead. Single ladders must be secured when in use.

k) Use portable ladders for up to 4m height only l) Provide fix ladders above 4m height m) Place the ladder at an angle of 75 deg approx from the horizontal n) Extend the ladder at least 1m above the top landing platforms.

SAFETY IN THE USE OF HAND TOOLS It has been estimated that accidents due to various types of tools account for nearly 10 % of total accidents. In any factory or industry, tools are used directly by workmen or they are used along with machines. Tools are generally of two types : 1. Hand tools 2. Portable power tools Hand Tools The common hand tools include chisels, hack saws, hammers, screw drivers, tongs, wrenches and spanners, torsion tools, material handling tools like crow bars, pinch bars, shovels, other tools such as files, cutters etc.

Portable power tools They are either electrically operated or of pneumatic type. Common power tools are grinders, drillers, riveters, etc. Accidents due to wrong or unsafe way of using the tools a) Some workers use screw driver in place of chisel or they use wrenches in place of hammers. Some times a knife is used in place of a screw driver or a flat spanner is made use of in place of box or ring type spanner. All these wrong action of workmen lead to hand or finger injuries. b) Application of hammer against hard face or due to improper pressure applied on hammer results in accidents either to the workman himself or to his neighbours. c) Some workmen get finger or thumb injury when they use hack-saw. Such accidents take place since the worker keeps his finger very close to the blade. d) Chisels should be used in such a way that cutting is directed away from the body. If cutting is done towards the body, cut chips are likely to injure the worker. e) Some workmen carry the tools in their pockets, belts or in hand at the time of climbing a ladder. These situations result in dropping of tools causing accidents, not only to the workmen themselves but also to others in some cases f) Sometimes when workmen carelessly store or keep tools on unsafe place like scaffolds, platforms without toe boards etc, accidents take place due to dropping of tools. Accidents due to unsafe conditions or defective tools 1. Hammer accidents take place due to cracked or oily wooden handle or due to loose or

unsecured fitting of handle with the hammer head. 2. Chisel and punch accidents take place due to mushroomed head, blunt cutting edge or due

to their improper heat treatment sometimes when their body is too short for safe holding, hand injuries are possible.

3. File accidents are likely when their handles are loosely fitted or when their teeth are missing and clogged with metal chips. Similarly knife can cause accidents, if its handle is broken or too short and if its cutting edge is blunt.

4. Saws create accidents when their teeth are ground improperly and when saws are fitted too

tight or in the reverse direction. When saw handles are loose or cracked, accidents are possible.

5. When spanner and wrench are oily, then they slip and cause accident. When jaws are worn

out or the serration or teeth are rounded off accidents takes place. 6. Pliers and tongs cause accidents , when their handles are improperly shaped or when their

hinged points are cracked or too loose. Accidents also take place when the serrations are rounded off.

Prevention of Hand tool Accidents The following precautions can reduce the number of tool accidents a) Hand tools must be periodically examined and maintained by competent persons b) They should be made of good quality materials c) They should be kept in cabinet, or racks when they are not in use d) The wooden handles of tools must be made of best quality and durable wood e) Only after receiving good training and safety instructions, he use of hand tools should be permitted by the workmen.

HOUSE KEEPING House keeping can be explained in a general way as “ A place for everything and everything in its place ”. Bad house keeping is responsible for about 15 % of total number of accidents in the industry. Following are the common types of accidents due to bad house keeping :

a) Slipping Accidents

It is common to see that workmen slip and fall due to oily floor due to wet floor conditions.

b) Falling Accidents If open tanks in level with the floor are not kept closed with suitable lids , people are likely to fall into them. Similarly wherever excavations are made for the sake of buildings, laying foundation work etc. and if such excavated pits are not protected with suitable barricade with warning boards, men are likely to fall into such pits resulting in falling accidents. c) Tripping Accidents If unwanted objects are scattered on the floor and walkways, they happen to cause tripping accidents for those who pass through such areas. Such accidents are more pronounced if the scattered materials are on the floor and walkways, they can’t be seen easily. d) Hitting Against accidents If some objects like steel rods, rails and other objects happen to protrude into the walkways, they cause the persons to hit against such object.

e) Accident due to poor lighting

If materials or objects are stored on windows, they are likely to block the natural lighting available to the work area. In such areas, visibility becomes poor and the same is likely to cause accidents. Again, wherever natural lighting is not possible, adequate lighting with artificial means should be ensured so that the working area does not suffer due to the poor lighting. Accidents due to improper disposal of scrap or Effluents If the scrap, waste etc…. are not disposed off in a planned manner, they get accumulated in the yard, blocking useful moving space . In the same way, the materials in the yard should be properly stacked without causing blockage to the movement of vehicle etc … if rail rods are there inside the factory, then adequate area adjacent to both sides of the rail should be kept clear without any blockage. Environmental pollution due to untreated effluents, improper exhaust of smoke or gases etc …constitute bad house keeping and they led to the occupational diseases of the workers. Fire accidents due to bad house keeping Fire accidents can take place due to oily wastes, accumulation of dry leaves, faulty electrical wiring etc………. Steps to be taken for avoiding bad house keeping 1. Walkways, stairs, yard flooring etc….. should be kept clean and dry to avoid accidents due to slipping. Oil or grease accumulations etc. shall be removed promptly. 2. Stored materials, tools, equipment should not be kept blocking the pathways both inside and outside the working area to avoid tripping accidents. 3. Materials should be kept in bins in an orderly manner. Storage of flammables in bins should be avoided. Yard, floor storage be well defined with proper markings or signs. No protruding materials such as rods, nails etc ….shall be allowed to infringe the moving areas. 4. Both natural and artificial lighting in adequate measures at all times, should be available in the working areas. There should be no blockage of lighting due to stored materials. 5. Accumulation of scrap, dust etc should be avoided. Disposal of effluents after proper chemical treatment , exhausting of smoke, fumes, gas etc …by proper exhaust arrangements, etc …. should be carried out as per statutory requirements. 6. Incinerators located at marked places will help the burning away of the unwanted rubbish such as dried leaves, oily waste etc…..

Joints and connections Electrical joints and connections should be of proper construction as regards to conductivity, insulation, mechanical strength and protection. Handle spindle should be close fit in the hole in the cover to prevent injury from arcs inside.

4. “ON” and “OFF” position in switches and circuit breakers should be clearly marked. . 5. Ample space between opposite poles of double and multiple air break switches and circuit breakers. 6. Should make and maintain good contact on being operated without any special skill or manipulation. 7. Switches and circuit –breakers should be of such construction that they cannot be left in partial contact, that there is speedy breaking of contacts when operated to off position. there should be other extra means of controlling the arc where necessary. 8. Handle should be of sufficient size to withstand the effort required to operate. Protective Earthing Earthing of the conductors is an additional precautions which may be necessary to ensure that they are not charged inductively or by any other chance Earth is to connect the general mass of earth in such a manner as will ensure at all times and an immediate discharge of electrical energy without danger and to allow sufficient current to flow to blow fuses or operate protective devices and thereby disconnect faulty apparatus. Working space and means of access Adequate working space and means of access free of danger should be provided for all apparatus which have to be worked by any person and sufficient lighting should be provided. Personal Protective Equipment Portable or permanent insulation stands or screens, boots, gloves or other suitable means should be used whenever necessary to prevent danger. Competency of personnel Only authorized and competent persons should undertake any work where technical knowledge or experience is required to avoid danger. Cable and connections Connection of the portable apparatus to the system should be by efficient permanent joints or connections. The cable should be reinforced at the place where it enters the plug with stout rubber tubing or metallic spiral. The cable grip should be such that outer covering or sheathing is securely held and no strain is placed on conductors.

Hand shield, disc or other type of guard between grip or handle part and the portion containing cable and contact pins should be provided. Electrical Shock Treatment 1. If the accident occurs indoors switch off the current from the mains supply. Rescuing a victim by a) pulling away by clothes b) re-moving live wire from the body c) cutting off live wires 2. If this cannot be done immediately, remove victim without directly contacting his skin 3. If the accident occurs outdoors, push the line wire or conductor away from the victim with a dry, un-painted wooden pole or pull it away that a loop of dry rope or any other non-conductive material 4. Send for medical help and start artificial respiration immediately 5. Continue persistent artificial respiration till the patient revives or till there are no chances left. There are cases on record where revival has occurred after even 3 hours of persistent effort.

SAFE WORK PROCEDURES – ELECTRICAL SAFETY 1. Only skilled experienced and authorized personnel are allowed to carry out electrical works Local rules and regulations strictly to be followed if required any authorization 2. All distributions strictly through ELCB only 3. Temporarily joints are not allowed, joints only with joint kits 4. For extensions use weather proof male / female , plug / sockets in humid area and normal with cover in other places. 5. Distribution boxes extensions must be weather proof and with cover type socket on height work use 110 V supply 6. Ensure proper / solid earthing Main and for all electrically operated equipment, hand tools etc. 7. Regular tests must be made and registered to ensure that effective earthing is maintained and it should be in a good condition 8. Required work permit must be obtained before starting electrical work 9. Rules and regulations of local authority to be followed strictly 10. In one socket multi connections are not allowed , any loose connection are strictly not allowed. 11. Three phase equipments must be with starters and operational OLR’S 12. All cables for temporary lighting must be specified rated safe carrying current capacity. 13. Warning tags must be placed where required , 415V. like DO NOT OPERATE, MEN AT WORK , ELECTRICAL CABLES etc. 14. All tools must be insulated. 15. Count your tools before starting the job and after completion of the job. 16. Temporary electrical circuits should be inspected daily by on-duty electrical personnel, if need any maintenance, repair etc to be carried out immediately.
