2. Click Provider Dashboard box

3. Registered User enter UserName and Password and Click Sign In

4. Click Add New Patient (Initially there will be no Patients)

5. Enter Personnel Information (* are mandatory fields)



6. Enter Optional Payment information

7. Enter Medication information(* are mandatory fields) There are two type of medication you can enter. Prescription and Over the counter (non-prescription) Also you can

upload prescription image



8. Enter Health Record information(* are mandatory fields)

You can enter all immunization, Tuberculosis Test, Medical Condition, Physical Examination, Family Health Record

ere. All of them are optional, but we recommend entering as much as possible.



9. Enter Blood Pressure information(* are mandatory fields)

10. Enter Lab Result information Currently system allows uploading all lab result as file in different file formats. (e.g. .jpg, .doc, .txt etc)



11. Special Notes During patient registration, special notes can be added explaining patients conditions, situation etc.

Special notes is kind of history about various treatment etc.

You are done with Patients/Case Registration Once Patients, record entered, information can be viewed, and edited by same provider and with special

notes.

Once Patients registered, patients can be referred by clicking the referral link as shown below



12. Patients/Case Information View/Edit/De-Activate

Once Patients registered, patients can be easily viewed and edited by the registering provider. Also

information be viewed and edited by the referred provider.

View Click the View button

Edit Click the Edit button

De-Activate Patients If patient die, de-activate the patient by clicking de-activate button.



13. Patients/Case Referral To refer a patient/case there are 4 easy steps as stated below. (1) Click Referral link

(2) Select a Patient by clicking the Radio button

(3) Select a Provider by Clicking Provider Radio Button

(4) Click Refer Patient Button
