REMINDERS FOR THE ENROLMENT

1. Kindly download and print out the forms and accomplish them.

2. Paste a 2x2 picture with white background. The student must be in school uniform. (Incoming

Gr. 7 must wear the school uniform of the previous school attended.). Female Students must tie

their hair while the male students must have the 2x3 haircut.

3. Attach the necessary documents.

Gr. 7 - Original Form 138 (Report Card)

Form 137 (Permanent Record with school dry seal)

Good Moral with school dry seal (as graduate)

Cedula (Original & Photocopy)

Birth Certificate (Photocopy)

Gr. 8, Gr. 9 ,Gr. 10 Gr. 11and Gr. 12- Original Form 138 (Report Card)

4. Submit the forms and documents in a long brown envelop. Kindly label with your name at the

right side portion of the envelop.

5. The parent from Grade 7 to Grade 10 and Grade 11 from other school will accompany the student

(must be in school uniform) during the enrolment day. Grade 11 and Grade 12 (in school

uniform) students may enroll without parent.

6. Come on the given schedule.
