THE INFORMATION PROVIDED HERE WILL ASSIST YOU IN THE ATHLETIC ELIGIBILITY PROCESS. PLEASE FOLLOW 

THE STEPS BELOW TO JOIN AN ATHLETIC TEAM. 

1. Contact the Athletic Head Coach of the sport you wish to participate. Use the school website for 

information and to determine when tryouts and the official 1st date of practice starts. 

2. Obtain an Athletic Physical Packet. Review and complete all required information with your 

parent/guardian(s). Make sure “ALL” signature locations are complete. 

3. Have the Athletic Physical Packet notarized by a Notary Public. 

4. Take the Athletic Physical Packet to your physician to complete the EL2 form. We do not accept 

Blue/Yellow Immunization Records.  

5. Once athletic physical packet is complete, you must submit packet to the Athletic Office to start the 

clearance process. 

6. The following is required for you to participate in any athletic conditioning, practice or contest: 

a. Athletic Physical (additional medical questionnaire not included in this packet may be requested 

later) 

b. Athletic Eligibility Clearance (2.0 GPA Academics and Conduct/Attendance 10‐20 Rule) 

c. Required (3 Videos) Concussion in Sports, Heat Illness Prevention and Sudden Cardiac Arrest

d. Athletic Insurance (Payments are made directly to the Athletic Office – Cash, Money Order or 

Cashier Check payable to Miami Norland Senior High)

PRACTICE, OFF‐SEASON CONDITIONING, CONTESTS, etc.) WITHOUT ATHLETIC PHYSICAL AND 

INSURANCE. 

BEFORE YOU BECOME ELIGIBLE YOU MUST: 

1. Submit this completed physical packet. a. Make sure all signature locations are signed. Both parents/guardians are encouraged to 

sign all documents. b. Make sure you have read and fully understand the Covid‐19 Waiver. It must be signed 

by the student‐athlete and the parent. c. Make sure this packet is notarized by a Notary Public. d. Make sure your doctor signs and dates the EL2 form. Request doctor to stamp your form 

with the name of Doctor’s Office, including address and phone number. 2. Submit three (3) mandatory video completion certificates. Information on next page.

Concussion and Sudden Cardiac Arrest Course 

Instructions  

 

Log on to:  www.nfhslearn.com 

 

Following these steps once logged on: 

1. Click NFHSLearn FOR YOU Tab then select Students. 

2. Scroll to the bottom of the screen. Under Course click Core Courses to see a list of 

course offerings. 

3. On the next screen, you may filter your search to reflect only student courses by 

checking the box for students at the left of the screen. 

4. Look through the courses for: 

a. Concussion in Sports 

b. Sudden Cardiac Arrest 

c. Other free courses are also provided but are not mandatory for eligibility 

5. Click on the course you wish to take. There will be a button called View Course. 

6. Click Order Course – This will prompt you to Sign In with your account information. If 

you do not have an account, you may click Register Here to create one. 

7. Once signed in, you will be prompted to complete the order process. There is NO COST 

for these courses. The courses are FREE. 

8. After ordering the course you will be able to locate your course on your dashboard. 

9. The courses are approximately 45 minutes in length. Please watch the courses in their 

entirety. Complete any questions. 

10. Print out the Completion Certificate to submit to the Athletic Office.
