LIMITED ASBESTOS REPORT

For

City of Owasso

Limited Asbestos Survey

On August 15, 2013, Mr. Bill Beams and Mr. Andrew Hackler of Blackshare

Environmental Solutions (Blackshare) completed an on-site inspection of a commercialproperty located at 76th Street North & Main Street, Owasso, Oklahoma. Blackshare

was retained by Mr. John Feary.

The scope of the project was to conduct a limited asbestos inspection, collect an

appropriate amount of samples, package samples, prepare an appropriate chain of

custody, ship samples to certified laboratories for analyses, and prepare a final report

summarizing the findings of the investigation. All work was conducted by a certified

asbestos inspector.

Blackshare collected a total of twenty-five (25) individual samples of suspect asbestos

containing materials (ACM) which were analyzed by a certified laboratory (See Table 1).

This survey was limited to materials that were visually accessible. Materials that were

obscured by walls, ceilings or chases were not sampled as a part of this survey. No

Conclusion

Based on the referenced site investigation, sample collection and laboratory analyses,

Blackshare has developed the following conclusion:

A total of thirty-four (34) suspect asbestos containing materials (ACM) were

analyzed at a certified laboratory (Tec-An, Inc.). None of the suspect ACM

samples tested positive (above 1% asbestos content) for asbestos (See

Appendix B- Asbestos Laboratory Results).

Recommendation

No further investigation is recommended in conjunction with the materials
