FOREWORD

This citizen’s charter booklet is a significant tool that provides information and education for the customers/clients of The Office of the Army Provost Marshal (OAPM). It is our commitment of providing exemplary services for our customers/ clients based on our functions, that is, to conduct investigation in relation to violations of Discipline, Law and Order (DLO) with focus on the administrative

liability of individuals. This citizen’s charter was revised based on the current of-fice mandate with a flow of the activity that was simplified and clearly defined in order to continuously intensify our effort to pro-vide a high level of DLO awareness and adherence among PA personnel towards a well-disciplined Army that is source of national pride by 2028. Written complaint must be in a form of notarized affidavit and must attached with other supporting documentary evidences.

Any personnel of the AFP and their dependents, individual, entities and organization who has personal knowledge of the offense committed by the respondent.

What are the requirements?

Complaint Affidavit, letter complaint, documentary evidence and testimonies of the witness/es (if there is any).

More or less than 30 minutes.

Schedule of available services:

Monday to Friday 0800H to 1700H

Who can avail of the services:

All Army personnel.

What are the requirements:

- Written request from concerned person;

- Written request from unit commander; or

- Written request from any active commissioned officer authorized to act for and in behalf of the unit Commander.

Duration: 30 Minutes PROCEDURE: 1. The admin NCO or record NCO will receive the written request from the concerned person or from the unit commander. 2. The admin NCO will forward the request to record NCO for verification in the OAPM data base. 3. Record NCO will prepare the certification and forward the same to Chief, Admin Branch for signature in behalf of APM. 4. Release of Certification

FILING OF COMPLAINT Procedure: Upon receipt of complaint, duty personnel will conduct initial investigation and verification of substantial documents from complainants; and refer the complainant to Gender and Development (GAD) Office with complaint slip.

GAD personnel will conduct initial interview to determine the nature of complaint; if domestic or non-domestic violence. If domestic, said personnel will make an assessment and referral to OESPA. If non-domestic violence, endorse complainant to Legal and Investigation Branch (L&IB).

L&IB personnel will conduct initial interview and screening of complaint affidavit, evidence and other supporting documents. If the supporting documents is complete in substance and in form, the complainant will fill-up complaint sheet. Complaint will be accepted with supporting documents and complainant will be advised accordingly that the complaint will be referred to unit of concerned personnel for appropriate action/investigation.
