LSP are often failed to conduct:

1) Internal audit and Management review

2) Update of Business Plan (periodic review of market analysis, strategies, improvement plans

progress and result, etc)

3) Review and monitoring of Business policies and objectives, as well as quality policies and

objectives.

5) Evaluation of learning services; e.g. periodic evaluation of facilitators, gathering feedback for

Reviewed the RA for “Walking on Scaffold” and verified that the Severity was

defined as “4”, even though the possible injury could be death due to falling

from height.

1) Reviewed Practical Training for Work-at-Height for workers course conducted by the

facilitator and verified that he is the only facilitator and he did not walk through the

practical training with the individual learner.

2) At the practical training site, seen only 4 out of 9 learners wearing safety shoes and the

Facilitator is also not wearing safety shoes. Further all the learners are not provided

safety gloves for the practical training.

3) Evidence of PE approval on the erected scaffold is not available. Also Scaffold condition

was not adequate. No capping of pipe end for some pipe & rusty condition of some bolts

and nuts that are used to hold the clamp that hold the pipe structure of the scaffold.

4) Reviewed the assessment conducted by the assessor, Suspension Trauma and verified

that the use of harness suspension straps was not verbalized by some of the learners

and the assessor did not penalized them.

LSP did not indicate version control in standard curriculum they developed, in

order to ensure an updated version of the training course syllabus, lesson plan,

facilitator’s guide and any other relevant course information at all times for

inspection and verification by MOM/CB/SAC.

Course Reference

LSP failed to ensure the time allocated for a topic consistent with the material

and adhere to requirements. E.g. 2.5hours topic of SMSEC - Day 9: Topic of

factors affecting safety of scaffold structure were only covered with 8 slides.
