MechanismsofDrug-DrugInteraction

• A druginteraction resultswhentheeffectsofadrugarealteredinsomewaybythepresenceofanotherdrug,byfood,orbyenvironmentalexposure.

• Theriskofdevelopinganadversedrugreaction(ADR)secondarytoadrug–druginteractionincreasessignificantlywiththenumberofmedicationsapatientisreceiving.

• Mechanismsof druginteractions:• Apharmacodynamic interactionresultswhenadruginterfereswithaseconddrugatits

targetsite,orchangesinsomewayitsanticipatedpharmacologicresponse.Theconsequenceofthisinteractionresultsinadditivity,synergy,orantagonismoftheintendedeffect.Anexampleofapharmacodynamic interactionisthesynergismthatresultsfromcombiningtwoormoreanti-infectives inthetreatmentofaresistantpathogen.

• Pharmacokineticinteractionsoccurwhenonedrugalterstheabsorption,distribution,metabolism,oreliminationofanotherdrug,therebychangingitsconcentrationinplasmaand,consequently,atthetargetedsiteofaction.Clinicallysignificant druginteractions aremostoftenduetoalterationsinpharmacokinetics,secondarytomodulationofdrugmetabolism.
