PREFACE

The Bureau of Health Manpower Education was established to provide leadership inplanning, conducting, coordinating, and evaluating the Nation's health manpower educationprograms. Two recent developments in health manpower education are of particular interestto the public and to the Bureau the training of new types of physician support personneland the extension of the roles of traditional medical personnel to perform tasks which canbe safely delegated by the physician. These tasks include taking of medical histories,physical examinations, therapeutic procedures, as well as other more complicanteddiagnostic and surgical procedures.

These new types of physician support personnel are becoming increasingly popular.Because the field is new, there is little standardization of job definition, course content,prerequisites for entrance, and titles of training programs. Job titles of graduates include:"Physician's Assistant," "Physician's Associate," "Medex," "Clinical Corpsman," "ChildHealth Associate," "Pediatric Nurse Practitioner," "Nurse Clinician," and many more. Someprograms offer college credit, others do rot. Some are developing physician's assistants inprimary care, others are training physician's assistants in medical specialties.

The emphasis of this publication is on new or proposed programs which are in theadvanced planning or pilot stages and on operational programs which have not yet receivedaccreditation or have only recently been accredited. Listing in this publication does notconstitute endorsement of the program by the Bureau. Information about training programsin this emerging occupational field has been collected and published because it is not readilyavailable elsewhere.

This third edition of "Selected Training Programs for Physician Support Personnel" isbased upon survey data. The previous two issues were compiled from available informtionfrom various sources. Because of the rapid growth in the number of such training programsand a desire to collect uniform data, a survey questionnaire was designed in the spring of1971 to gather data for this publication. (A detailed discussion of the survey is found onpage 99.) The growth of this field is reflected by the fact that the first edition in 1970 listed67 programs, while the current edition lists 112 programs.

A number of programs were identified after the survey was completed. The lastsection of this publication contains summaries of programs for which complete informationwas not obtained. For any additional information contact program directors at the addressesindicated.

Accreditation and Guidelines for Developing Programs

Accreditation procedures have not been established for most of the new categories oftraining programs for physician support personnel. Only two types of programs wereaccredited as of January 1, 1972: physician's assistants in orthopedic and in primary care.Persons interested in the accreditation status of other training programs may contactindividual program directors or the American Medical Association, Department of MedicalEducation.

At present, the American Urological Association is working with the AMA to establishthe mechanisms for accrediting training programs for urologic assistants. The AmericanAcademy of Pediatrics and the American Nurses Association have issued joint guidelines foraccrediting training programs for pediatric nurse associates. Similar negotiations are beingconducted between the American Academy of Pediatrics and the American Association ofMedical Assistants to establish accreditation standards for training programs for pediatricassistants. Persons interested in the accreditation status of these programs may contact theprogram directors or the Assiciation and Academies.

Other organizations that have published statements defining types of physician'sassistants include the National Academy of Sciences, the Association of American MedicalColleges, and the American Hospital Association. Guidelines by the AMA Council on HealthManpower for the development of new health occupations were published in the Journal ofthe American Medical Association of August 17, 1970. Copies are available from the AMA.

Legislative Developments

As of January 1, 1972, the following states have enacted exceptions to their Medical

Practice Acts to permit physicians to delegate functions to qualified non-physicians:Arizona, Arkansas, Colorado, Connecticut, Delaware, Florida, Kansas, Oklahoma, and Utah.

The State Board of Medical Examiners is authorized to approve training programs for"physician's assistants" in the following states: California, Florida, Indiana, Iowa, Kansas,

New Hampshire, New York, Oregon, and Washington. Other states considering suchlegislation are: Alabama, Alaska, Illinois, Maryland, Michigan, Minnesota, Nebraska, NorthCarolina, Ohio, Pennsylvania, Tennessee, West Virginia, and Wisconsin. In 1972, additonal

states are expected to enact similar legislation.

For Counselors and Prospective Students

In view of the rapid developments in this field, persons using this information forcounseling students or for the selection of training opportunities should check the status oftraining standards for the specific category of the physician support programs in which theyare interested, as well as the status of the individual training program. Prospective students

are advised to investigate whether graduates of physician support personnel programs are

allowed to work in a State.

INTRODUCTION

The Survey

In April 1971, a survey of 137 training programs for physician support personnel wasconducted. These programs had come to the attention of the Bureau of Health ManpowerEducation from a variety of sources. Responses were received from 132 of the programssurveyed, in the initial mailing. Forty-nine of these did not complete the surveyquestionnaire, stating that they did not meet the requirements for inclusion in the study atthat time. Upon review it was determined that 22 other programs did not meet therequirements for inclusion. Nine of these were excluded because they were involved intechnician or technologist type training, and 13 were excluded because they were trainingpersonnel for intramural use only. Therefore, sixty-one programs from the initial survey metthe criteria for inclusion.An additional 10 programs came to our attention while the surveydata were being processed and were surveyed by telephone for inclusion. Forty-one othersare listed in the last section as programs for which no detailed information was available atthis time.

Criteria for inclusion in this publication were:1. Stage of Program Development The program must be either operational, or in

an advanced planning stage having assurance of financial support and operational statuswithin a year of the survey date.

2. Entrance Requirements Entering students must have completed high school orits equivalent, and the program must provide:

a. Short or long-term training to expand the capibilities of persons withprevious health service training and experience, or

b. Long-term training for persons with no previous formal health servicetraining or experience.

3. Occupational Category The graduates of the program must have an identifica-tion differentiated from traditional health professions as they currently exist, and mustwork under the direction of a physician or physicians to perform tasks the physicians wouldhave to perform in the absence of this specially trained individual.

Selected Findings:

The following analyses are based on the data from the 61 programs in the initialsurvey which met the above criteria. These programs trained 31 different types of personnel(Table 1) and were in various stages of program development, planning, testing, andoperating (Table 2).

There is little information available on applicants, enrollees, or graduates. Theavailable data from the 61 programs show that, in 1971, 5,613 applications for enrollmentwere received compared with a total of 3,097 applications in 1970. Because the survey wasconducted before final admission figures for 1971 were available, many schools were unableto report the number of persons to be enrolled. In the 35 programs reporting suchinformation, 243 males and 185 females had been enrolled. The number of graduatesreported was quite small. In 1970, 164 students were graduated from 17 programs. Reliabledata for 1971 were not available at the time of the survey. Of the 56 programs respondingaffirmatively to the question "Do you anticipate that this program will be training personsin 1975?", 44 provided estimates which indicated they would be training 1,350 students peryear in 1975, compared with 428 being trained in 35 programs in 1971.

A direct question concerning the types of institutions sponsoring programs was notasked. This information was obtained by analysis of the institutional names of respondents(Table 3).

The recent emergence of these programs is reflected in Table 4. Most programs began

operation during the past two years. The fact that a large number of programs were planned

between the time the survey was conducted and the time the results were ready forpublication further highlights the rapid development in the field.

The length of the programs, by academic and calendar years, is in Table 5. Some of

the programs did not specify the time frame within which they operated, but there isindication that in many programs, both the academic year for didactic training and thecalendar year for clinical training were used.

The type of credentials awarded to students is shown in Table 6. A certificate isoffered in 59% of the programs, and the associate degree is the next most conferredcredential.

Table 7 shows the minimum educational and experience entrance requirements forthese programs. The additional preferred entrance requirements were not tabulated becauseof the wide diversity of the answers which made classificable tabulations difficult.

Table 8 shows the sources of financial support for the programs by the stage of

program development. Many programs had multiple sources of funding during each stage ofdevelopment. The questionnaire asked only for funding sources: no attempt was made to

determine the amount of financial support for the program or from any specific source.

Tuition for in-State and out-of-State residents has been tabulated for the programs (See

Table 9).

There was indication that a great deal of activity was going on in the initiation anddevelopment of legislation concerning these new types of personnel. Table 10 indicates that

changes are anticipated in the laws in States where 21 of the programs are located, but not

all of these 21 programs are located in different States.

CURRICULUM DESCRIPTION: The first week is devoted to lectures in the basicsciences, i.e., Anatomy and Physiology of the eye,Pharmacology and Therapeutics. The second andthird weeks consist of didactic teaching insurgical techniques, operating room techniques,and care and maintenance of ophthalmic surgical

instruments. Instruction will also be given inmanagement of the eye patient in the ophthalmol-ogist's office, use of the lensometer, perimetry,tonometry, simple basic refractions, and

muscle imbalances. Clinical experience willbe provided in the various eye departmentsof the affiliated hospitals. During thethree weeks, the student may choose specialinstruction in certain subjects such as ophthalmicsurgical techniques, strabismus, clinical photography,tonography, or care of the contact lens patient.

MILITARY TRAI N I NG PROGRAMS for physician's assistants:The AIR FORCE has a requirement for 150 PA's. Classes of 25 will start every four months untilrequirements are met. The first class began July 1, 1971. The program is located at Shepard Air ForceBase, Wichita Falls, Texas. A minimum requirement for students is two years of experience in contactwith patients. The course will be of 24 months duration with 12 months of academic study and 12months preceptorship. Graduates will have a senior non-commissioned officer rank. For moreinformation write Col. The ARMY has a requirement for 400 PA's. Classes of 60 will begin every six months. The first classwill begin in February, 1972.
