Introduction The Office of Global AIDS Coordinator (S/GAC)and the Interagency Collaborative for Program Improvement (ICPI)

worked to provide the Site Imrpovement through Monitoring System (SIMS) 2.0/3.0 Data Extract in (DATIM) Genie

application.

The Genie 2.0/3.0 Data Extract incorporates the following features:

PEPFAR organizational hierarchy that focuses on only those levels that are relevant to program priorities:

Region, Operating Unit, Country, Prioritization-level SNU (PSNU; as applicable), Community (as

applicable), and Facility (as applicable).

Updated mechanism and partner names to align with most current data available in DATIM.

Unnested Core Essential Elements (CEE) data element components as separate columns (e.g., CEE short

and Set ID).

SIMS Coversheet data elements constructed as columns for easier analysis.

Data Sharing Policy The Genie SIMS 2.0/3.0 Data Extract are for use within the PEPFAR USG community. For additional guidance on

PEPFAR data sharing policies outside of PEPFAR USG community, please see Appendix E: PEPFAR Data Governance

Policy of the Consolidated Guidance on Data Collection & Use in PEPFAR (September 2017).

Structure of Genie SIMS 2.0/3.0 Data Extract The PEPFAR PRIME Systems team, in collaboration with the ICPI DAQ and ICPI SIMS Cluster Datastream, pull the

SIMS 2.0-3.0 data1 from DATIM into the PEPFAR Data Hub (PDH). The SIMS 2.0 – 3.0 datat is uploaded by PEPFAR

Implementing Agencies into DATIM twice quarterly. The SIMS 2.0 – 3.0 data includes CEE Score-level rows whch

show what was or could have been assessed for each assessment imported into DATIM from agency headquarters

systems. CEEs that were assessed will have a score value of 1 through 4, which exist on an ordinal scale2

(CEE_SCORE_VALUE):

1 = Red: Needs Urgent Remediation []

2 = Yellow: Needs Improvement []

3 = Light Green: Meets Expectations []

4 = Dark Green: Surpasses Expectations []

Any CEEs that could have been assessed, but were not applicable for the particular site or mechanism, have final

scores of 0 for ‘not applicable’ (N/A), which is colored Gray in SIMS tools [].

1 Datasets contain only SIMS 2.0-3.0 data. Previous SIMS 1.0 data may be incorporated to future versions with careful

thought to address the major changes to the SIMS tools that occurred between SIMS 1.0 and 2.0. SIMS 1.0 data can be extracted using the Data Genie. 2 SIMS scores represent and should be treated as ordinal variables. The scores, or categories, can be ranked low to high (red,

yellow, light green, dark green), but the difference between the score values is not measurable or the same within a CEE or across the CEEs.

What Makes a Unique SIMS Assessment Core Essential Elements (CEE) scores are associated with an individual assessment that either USG or IPs

conducted at a particular point and time for a specified mechanism. Agency-field teams assign each assessment a

unique identifier on the assessment tool’s accompanying coversheet (CS_ASMT_ID) that can identify unique

assessments.

To distinguish further or to group assessments that have a shared relationship, it is helpful to understand which

additional fields (data elements) make a specific assessment (or CEE score) unique:

WHO: Mechanism / Implementing Partner (whose technical assistance or service delivery was assessed?)

WHAT: Assessment Type (what type of assessments was conducted? initial, comprehensive or focused

follow-up?)

Tool Type (what tool was used to conduct the assessment? facility, community or above site?)

WHERE: Assessment Point (where was the actual location, or name, of the assessment point?)

WHEN: Assessment Date (when did the assessment occur?)

WHO: Whose technical assistance or service delivery was assessed?

SIMS provides a snapshot of PEPFAR-supported program quality at the point of service delivery, as well as an

assessment of the quality of technical support provided by PEPFAR supported entities that guide service delivery.

The current SIMS Implementation Guidance requires that “all active mechanisms across all implementing agencies

that support delivery at the site (facility or community, be they classified as “direct service delivery” (DSD) or

“technical assistance” (TA)) or that support entities that guide and support service delivery must receive at least

an initial SIMS assessment over the life of the mechanism” (See SIMS Implementation Guide for more

information).

WHAT: What type of assessment was conducted? What tool was used to conduct the assessment?

How to Distinguish Assessment Types

SIMS assessments use a quality assurance methodology that assesses performance against sets of quality

standards to identify issues or areas to improve through remediation. Currently, all PEPFAR-supported sites must

receive an Initial assessment during the life of a mechanism’s funding agreement (e.g., contract, cooperative

agreement, grant). As per SIMS 3.0, all facility and community sites designated as ‘high volume’ must then receive

a USG Comprehensive Follow-up assessment each subsequent year to resassess all applicable CEEs. Similiarly, all

PEPFAR-supported national and sub-national units or other entities that support and guide service delivery (i.e.,

each above-site entity) must receive a USG Comprehensive Follow-Up assessment annually.

For any Initial or USG Comprehensive Follow-up facility or community-level assessment that scores red on 25% or

more of all scored CEEs or both yellow and red on 50% or more of all scored CEEs, USG staff from the agency

overseeing the IP must conducted a USG Focused Follow-up assessment to rescore the yellow and red CEEs

within six (6) months of the assessment.3 This threshold is referred to as the “25/50 Rule.”4 Regardless of whether

an assessment’s CEEs met, or passed the 25/50 Rule, IPs are expected to address all PEPFAR-funded CEEs scored

yellow or red in the quarter following the yellow or red score. These yellow or red scoring CEEs are expected to be

reassessed by the IP within six (6) months of the previous assessment the the rescore should be report to the

agency activity manager and submitted into agency systems as an IP Focused Follow-up assessment.

Given that yellow and red scoring CEEs are expected to be reassessed to assure and document that quality

improvements have been made by IPs to meet PEPFAR’s standards for delivering service and providing technical

support, multiple assessments can be conducted to the same site, or location, and to the same mechanism at

different points in time. These sequence of assessments to the same site and mechanism are distinguished by

their Assessment Type (Initial, USG Comprehensive Follow-up, USG Focused Follow-up, IP Focused Follow up),

which is designated by agency field staff on the SIMS Coversheet (CS_ASMT_TYPE or

CS_ASMT_TYPE_DESCRIPTION).

How to Distinguish Tool Types

There are three different SIMS tools used to conduct an assessment (Facility, Community, and Above Site) that are

defined by the location and type of services offered at the assessment point:

A. Facility Assessments: A facility is a PEPFAR-supported institution that provides service delivery at a

defined location. Examples of facilities include clinics, hospitals, laboratories, and other ‘brick and mortar’

structures where services are provided.

B. Community Assessments: There are 3 types of assessment points were community assessments are

conducted:

1) Organizational Assessment Point (OAP)

2) Organization Assessment Point that also offers direct service delivery (OAP+)

3) Service Delivery Point-1 (SDP1)

NOTE: SDP2’s are locations where services are provided but no records are kept. SDP2 locations

are not assessed.

C. Above-Site Assessments (or Above-Service Delivery): An above-site entity is a PEPFAR-supported

institution that guides and supports service delivery. A PEPFAR-supported institution can either be

PEPFAR-funded OR a recipient of PEPFAR-funded technical assistance. Its function is to execute system

strengthening and/or nonservice-delivery functions considered essential to the successful implementation

of HIV prevention, care and treatment programs. The Above-site SIMS assessment tool and associated

CEEs are designed to be assessed at institutions at the national or subnational level.

WHERE: Where was the actual location, or name, of the assessment?

How to Distinguish the Assessment Point from the Org Unit

Org Unit refers to the specific geographical area or facility site within PEPFAR’s organizational hierarchy where

data is associated. Assessment Point refers to the actual location where the SIMS assessment occurred.

For facility-level assessments, the Assessment Point and the Org Unit are identical (i.e., facility assessments occur

at the facility org unit as listed in PEPFAR’s organizational hierarchy). For Community5 and Above-Site

assessments, however, PEPFAR’s organizational hierarchy does not manage the actual location of these

assessments’ sites. Agency field teams instead associate these assessments with the lowest Org Unit possible and

separately provide the name of the Assessment Point, or location of where the assessment occurred.

Community assessments will be associated within the Org Unit or SNU that serves as the OU’s “community”.

Above-Site assessments can be associated at the OU’s Org Unit or at lower SNU levels to align with the Org Unit

that represents the programmatic reach of the subnational entity. The exact name of the point where the

Community or Above-Site assessment occurred is captured as a SIMS Coversheet data element. Depending on the

type of site, the name is entered as:

CS_ABOVESITE_ASST_PNT_NAME

CS_COMMUNITY_ASMT_PT_OAP_NAME

CS_COMMUNITY_ASMT_PT_SDP_NAME

Therefore, multiple SIMS assessments can be associated with the same Org Unit, but will have actually taken

place at separate Assessment Points or locations. To support identifying and analyzing different Assessment

Points – regardless of the SIMS tool type used – the SIMS 2.0/3.0 Data Extract has calculated a new field to

combine the Assessment Point names into a single column (ASMT_PNT) (see Calculated Fields: Assessment Point

for more information).

WHEN: When did the assessment occur?

Agency field teams enter the actual date of the SIMS assessment in the Coversheet. The Assessment Date is a

separate data element (CS_ASMT_DATE) than what is imported into DATIM as the “Period” field. As described in

the Where section above, multiple SIMS assessments can be associated with the same Org Unit, but have taken

place at a distinct locations. Since DATIM requires unique assessments to have distinct values in either of its

Period, Org Unit, or MechanismID fields – agency headquarters systems need to manipulate the Period field for

any Community or Above-Site assessments that occurred on the same date for the same mechanism within the

same Org Unit. This manipulation is required in order to accommodate having multiple records associated at the

correct Org Unit level within DATIM.6

Due to data entry issues and earlier systems-related import methods, some assessments may not have a value

entered in the CS_ASMT_DATE fields. In these instances, the imported Period field can be used to provide a proxy

assessment date. To ensure an assessment date is available for analyses, the SIMS 2.0/3.0 Data Extract has

calculated a new field to identify the Assessment Date in a single column (ASMT_DATE) (see Calculated Fields:

Assessment Date for more information).

Features of Genie SIMS 2.0/3.0 Data Extract

SIMS Data Element Components The SIMS 2.0/3.0 Data extract splits the CEE data element components into separate columns and references the

SIMS Tools’ current naming convention and numbering for Sets and CEEs. See example depicted below and the

CEE Reference Table for the complete list of the CEE components mapped to their complete CEE ID and names

used in the SIMS Tools.

Data Element Example

Data Element Types: F = Facility Tool, C = Community Tool, A = Above-Site Tool, CS = Coversheet. Unlike

the SIMS Genie Extract, the SIMS Structure Datasets does not include the separate Coversheet data

elements because these fields have been transformed into columns (See Coversheet Data Elements

section).7

Set #: Number used to identify the Set and its sequence within each tool’s data elements. Set #’s can

represent the Set ID, but in some instances represents part of the Set ID (e.g. Facility Tool has Set 1A and

Set 1B, which are both use the same # “F_01”.)

CEE #: Number used to identify the CEE and its sequence within the Set.

CEE Short Name: An abbreviated name given to each CEE to help identify it.

Value Type: A code word or abbreviation to identify the type of value for the specific data element.

Unlike the SIMS Genie Extract, the SIMS Structure Datasets currently contain only the value of the CEE’s

final score; therefore, all data elements included in the dataset end with “_SCORE”.8

SIMS Coversheet Data Elements In order for agency headquarters systems to be able to import SIMS data into DATIM, all of the Coversheet data

for each assessment is transformed into separate rows and assigned its own unique data element. The Coversheet

data contains important attributes of an assessment used to distinguish assessments and which are important for

analysis of SIMS data (e.g., Assessment ID, Assessment Date, Assessment Type, Assessment Point Names). The

SIMS 2.0/3.0 Data Extract transforms the Coversheet data elements into columns associated with the assessments

and their CEE score values to be more readily available for analysis.

Assessment Date

This calculated field pulls the most accurate, or best known, assessment date for each assessment into a single

column. The field first pulls the assessment date from the SIMS Coversheet data element (CS_ASMT_DATE),

whenever available, or will use DATIM’s Period field as a proxy if the coversheet is null.

ASMT_DATE: Calculation’s Logic

1) If CS_ASMT_DATE is not NULL, then ASMT_DATE = CS_ASMT_DATE

2) If CS_ASMT_DATE is NULL, then ASMT_DATE = DATIM’s Period field

Assessment Period

This calculated field references the calculated Assessment Date (ASMT_DATE) field and converts the data into the

fiscal year and quarter format (YYYY-Q#).

This calculated field combines the actual Assessment Point names into a single column to support distinguishing

which Community and Above-Site locations where assessed when multiple assessments been conducted within

the same Org Unit.

Most Recent Score

Two calculated fields are available to identify and analyze the most recent score available for a CEE when multiple

scores have been given at the site level OR site-by-IM level. Depending on the type of analysis being conducted,

users can make use of one of these fields as a filter to deduplicate CEE scores.

Duplicative CEE scores are expected since all active mechanisms and PEPFAR-supported sites are required to have

at least an Initial assessment during the funding agreement and, in most cases, at least one follow-up assessment

will occur to reassess any CEEs that score red or yellow.

Sites may receive multiple assessments overtime

All high-volume sites and above-site entities should then receive Comprehensive Follow-up assessments each

subsequent year. For any Initial or Comprehensive Follow-up assessment at the facility or community level, which

does not pass the “25/50 Rule” (i.e., CEEs score more than 25% red or 50% red/yellow), USG are required to

conduct a Focused Follow-up assessment to reassess the red and yellow-scoring CEEs within six months of the

previous assessment. Following the USG assessments, implementing partners (IP) should conduct a Focused

Follow-up to reassess any red and yellow-scoring CEEs within six months of the previous assessment and submit

the new scores and documentation of improvements to their USG funding agency.

Sites with multiple mechanisms receive separate assessments

When more than one mechanism supports the same site, each mechanism should receive its own assessment;

these assessments, however, can be conducted at the same time or during the same ‘visit’ to the site (i.e.,

separate Coversheets and Sets of CEEs should be scored, as applicable, for each mechanism).

25/50 Rule

Either ≥ 25% Red CEEs or ≥ 50% Red and Yellow CEEs for a single assessment needs a USG staff from the agency

yellow or red on an initial assessment, the IP should conduct a rescore within six (6) months of the initial score

The “25/50 Rule” is only applicable for initial and comprehensive assessments where all applicable CEEs are

scored. In addition, Above-site assessments are not included because USG staff are expected to reassess the

above-site entities during their required annual comprehensive assessments.

How to Extract SIMS 2.0/3.0 Data Extract 1. Login to https://datim.org

2. Select ‘Genie’ app from the list of available apps.

3. Select the desired report in the drop down. Showing the ‘SIMS 2.0/3.0 Data Extract’ option

4. Select the Operating Unit(s) of choice from the organizational hierarchy tree.

5. Click on the ‘Run Report’ option after refining your search results by the following (if applicable): SNU1,

PSNU, Funding Agency, Mechanisms, Indicator, and Disaggregate filters.

6. Users have the option to extract the data returned by selecting the ‘Export All as XLSX’ option to

download the spreadsheet output of all the rows returned regardless of what is displayed on the Genie

main table.

How to Use Excel to Analyze Data from the Genie SIMS 2.0/3.0 Data Extract

When using Excel, users should import the data using Excel’s built-in text import functionality to prevent data

from importing incorrectly. This can be done by following these steps:

1. Open a blank Excel workbook

2. Select the Data tab in the top ribbon

3. Select ‘From Text’ in the Get External Data section of the Data tab

4. In the window that opens, navigate to select the SIMS 2.0/3.0 Data Extract you want to use.

5. In the Text Import Wizard window, select ‘Delimited’ and ‘My data has headers’, then click ‘Next’.

6. In the next window (Step 2 of 3) select ‘Tab’ in the list of available Delimiter options and click ‘Next’.

7. In the final window of the Text Import Wizard (Step 3 of 3), select ‘General’ as the Column data format

and click, ‘Finish’.

Note: Unlike the SIMS Structured Dataset, there are no columns that have specific formats that may be

misread and formatted by Excel at this time. The ASMT_DATE and CS_ASMT_DATE fields will need to be

formatted after importing to display the number as a date (e.g., Short Date).

8. Click ‘OK’ to import your data into the existing worksheet, or specific location you select.

For additional guidance on how to correctly filter data within the Genie SIMS 2.0/3.0 Data Extract, submit a

DATIM Support ticket here and select the issue top of “Data Genie” for help with Genie or “MER/SIMS Content

Questions” if it is more SIMS data-content related.

Structured Dataset

In addition to the Genie Extracts, PRIME also develops the SIMS Structured Dataset (SSD) on a quarterly basis. The

differences between the two datasets are outlined below.

Programmatic use Use the Genie 2.0/30 Extracts when you want to analyze other SIMS CEE components beyond SCORES (since all CEE components are included in the Genie Extracts). For example, if you want to see the responses to a specific question (including check box responses) within a CEE to determine why that CEE was received a SCORE of red or Yellow. Granular information such as this can help in planning improvement activities at the site level i.e. what specifically needs to improve to change from a red or yellow to a green.
