INTEGRATION OF WEB TECHNOLOGY IN MEDICAL PRACTICE AND

TEACHING

Past Experience in Basic Electronic systems, Instrument Design and Computer Program

Authoring.

Michael Rosengarten has a broad technical background. His degree at McGill University

in Electrical Engineering included training in information transmission and computer

programming both of which are important in the current context of transmission of

material over the Web. His summer work experience included computer maintenance in

France; one year in Radar Maintenance, and one year in Telecommunications

Engineering in the Canadian Air Force Officer Corps. After graduation, he worked for

several years in Ottawa, dealing with the supply of sophisticated electronic nuclear and

test instrumentation to the National Research Council, Atomic Energy of Canada, and

the Department of Health and Welfare of Canada. While in medical school, Dr.

Rosengarten applied his engineering skills to the medical field by designing and building

instrumentation for experimental animal studies. He authored computer programs to

interpret the data. During two research years in Paris, France, he further married

engineering and medicine: he did computer-aided analysis of human heart-rate

variability and authored both analytical and textual computer programs for medical use.

Integration of Computer Technology in the Medical Field

Dr. Rosengarten has designed, created, and currently runs medical computer

networks in the division of Cardiology at McGill University. He designed and installed

one of the first computer networks in this system to offer email, Internet access, network

FAX, and main frame access to all members of the Division. After creating a modern

working environment for the Division, he encouraged colleagues and staff to acquire

computer skills . Since then, he has been the main resource person for computer

planning, coordination, and maintenance in cardiology at the Montreal General Hospital.

He has used his programming skills to author special medical software. For example,

using a graphical software programming language, Lab View, he created a sophisticated

program for the acquisition and display of electrophysiology data generated during

invasive electrophysiology studies. This program was a forerunner of current computer

based systems.

Current Database Authoring and Management

Dr. Rosengarten has extensive experience with medical database management,

essential for the creation of powerful and user-friendly knowledge databases. He

designed and authored three relational database programs (using Acius 4D): 1) patient

demographics and scheduling 2) test scheduling and 3) an advanced pacemaker follow-

up database. These databases are in current use and are suited to the medical

environment, as they are cross-platform, handle concurrent users, provide both excellent

search tools and forms for finding data and generating reports. The pacemaker database

manages all of the pacemaker implants for the McGill University Medical Center and is

intranet based ( as well as Internet capable) so that it is easily used at multiple

geographic sites. Dr. Rosengarten is responsible for the administration and maintainance

of the server for these databases. He has also worked with access from the Internet both

with customized client software and traditional Web browsers.

Multimedia/Web teaching/Distance Learning

Dr. Rosengarten is part of the Molson Medical Informatics project that provides a forum

and infrastructure for McGill medical students and staff interested in teaching on the

Web. He has independently developed an extensive Web-based teaching resource for

patients as well as caregivers, as illustrated by the "Medical Teaching" section of the

Online Journal of Cardiology. The Online Journal of Cardiology Web site created by Dr.

Rosengarten was the first to develop techniques for the display of high resolution cardiac

electrocardiograms on the Web (using Photoshop, Go Live, and Page Mill). The Online

Journal of Cardiology has also introduced new methods of self testing to aid distance

learning.( see the Super EKG and the X-ray Puzzler). The site also includes the Web

conversion of a printed text on electrocardiography in French.

Dr. Rosengarten is currently creating medical Web teaching with the latest software

tools (Realplayer, Quicktime, Final Cut Pro, Power Point, etc.) and has developed

techniques for the display of high quality x-ray still and video images on the Web. He is

applying the latest technology and software (digital video recording, Firewire

communication, streaming Quicktime 4 and Realplayer, Final Cut Pro, Photoshop 5.5

etc) to enhance the quality of Web-based video for medical teaching.

RESEARCH

1) Canadian Implantable Defibrillator Trial (CIDS) A randomized trial that compared

therapy with an inplantable defibrillator a verses Amiodarone for the treatment of

malignant cardiac arrhythmias.

2) Tocainide/Quinidine Antiarrhythmic Holter Trial. A multicenter trial evaluating the

efficacy of Tocainide and Quinidine in a double-blind fashion.

3) Disopyramide Slow Release Trial. A pharmacokinetic trial comparing slow release

to regular Rythmodan with serum levels.

4) Peri-Operative Arrhythmia Trial. A trial based on Holter monitoring for quantitating

pre and post cardiac surgery arrhythmias.

5) Peri-Operative Arrhythmia Trial, effect of cardioplegia. A trial that quantitated the

effect on arrhythmias of the potassium concentration in cold cardiac cardioplegia

on post -open heart patients.

6) The Antidepressant Arrhythmia Trial. A Holter trial comparing Trimipramine and

Doxepin. This Holter trial was a double-blind randomized trial to detect cardiac

dysrhythmias associated with antidepressant medications.

7) The Propafenone/Disopyramide Trial. This Double-Blind Antiarrhythmic Holter

Trial compared the efficacy of Propafenone to Disopyramide in patients with

frequent ventricular dysrhythmias.

8) The Sotalol/Propanolol Post Infarction Trial A Holter trial intended to treat post

infarction patients with frequent PVCs in a double blind fashion either with

Sotalol or Propranolol post infarction.

9) The effect of respiratory induced changes in vagal tone on cardiac rhythm. Computer

based project that detected respiratory/vagal variations in cardiac rhythm.

10)The A/V Node Model Study. This electrophysiologic study is designed to derive a

model of A/V nodal conduction based on premature atrial stimuli. The model is

intended to predict the steady state behavior of the A/V node from premature

atrial stimuli delivered during a PES study.

11) The Ventricular Ectopy Parasystolic Model Study. This is a study to derive a

mathematical model for the interaction of ventricular ectopy and sinus rhythm.

Data is obtained from patients during PES studies and with implanted

pacemakers.

12) The development of techniques for the recycling and the reuse of permanent cardiac

pacemakers. The project has placed the Montreal General as a leader in this field.

13) Pacemaker Reuse, A Prospective Trial. A trial looking at mortality, complications

and morbidity in a prospective fashion with a group of 22 patients who received

recycled pacemakers and 70 who received new pacemakers.

14) Spike Mapping: The detection of pacemaker insulation failure by computer aided

surface mapping techniques.

15) The use of Adenosine for the termination of supraventricular tachycardia, a

multicenter trial.

16) The use of the Telectronics 4201 and 4202 implantable defibrillators in cardiac arrest

patients. A multicenter trial.

17) The use of Recainam in patients with VT/VF as guided by electrophysiologic studies.

PUBLICATIONS

Founder, multimedia journals:

1996-Founder, Editor-in-Chief, and site manager, The Online Journal of Cardiology, an Internet journal (http://www.hrt.org).

The Online Journal of Cardiology presents a new approach to publishing and the teaching of cardiology. It is based on the idea that recent technology (computer software, the Web, high speed personal computers) has revolutionized the playing field for teaching and information exchange. The Journal illustrates how the integration of multi-media publication expands the range and impact of teaching; for example, in collaboration with The McGill Medical Informatics Program, the Journal allows the teacher and student to publish easily at low cost. The Journal also provides text, graphics (colour illustrations, high resolution electrocradiograms and rhythm strips, angiograms, x=rays etc), sound (voice, pacemaker transmissions ), as well as video teaching material; this includes state-of-the-art Web transmission of teaching conferences up to one hour in length. The Journal provides interaction with students, caregivers, and others through a number of forms: discussion groups, e-mail, and integrated Web forms. The Journal also publishes articles in more traditional form. Although modestly funded, and produce directly by Dr. Rosengarten, the site has been a great success. As of fall, 1999, it has attracted more than 150,000 visits to the Home Page and 60,000 to the Teaching Page.

This is the second of two Internet journals that Dr. Rosengarten has created. The Journal is designed to pull together caregivers in Canada, improve cardiac pacing practices, and provide patients with relevant information regarding their devices. The site is designed as a partnership site between the Universities and the device manufactures. The site offers low cost to the site's sponsors, in return for their contribution of material. The site incorporates e-mail, discussion groups and Web forms to facilitate the addition of information. Currently the site includes a comprehensive database on Canadian Device clinics and multilingual patient pacemaker information, and a list of preferred links to other sites.
