DEVELOPMENT OF EXTENSION EDUCATION PROGRAMS ______________________________________________________________________________

Got Vegetable? 2013 Program Justification A diet high in vegetables and involvement in physical activity are associated with lower risks for numerous chronic diseases and with weight management (USDA, Healthy People 2020). Sixty-six percent of Kentuckians are obese and fifty-four percent do not engage in physical activity. Only twenty-nine percent of Kentuckians consume the recommended servings of vegetables (CDC, 2011). Most programs designed to increase vegetable intake focus on all vegetables. However, recent evidence shows specific vegetables (particularly dark green leafy, yellow/orange, and cruciferous vegetables, and tomatoes) as being leaders in the fight against chronic diseases (Hung et al., 2004). This program uses the latest research to increase vegetable

intake and improve the health of Kentuckians. Programs developed with a sound theoretical foundation are more effective than those lacking a theoretical base (Glanz & Bishop, 2010). Developing strong programs is vital if Cooperative Extension is to be effective in the future.

Program Description The goal of this program is to increase intakes of dark green leafy, yellow/orange, and cruciferous vegetables, tomatoes, and physical activity among Kentuckians. This program is based on the critical thinking model that I developed and published. See peer-reviewed journal publications on critical thinking listed below. This model and method of evaluation is used in the Got Vegetable? program.

Impact: The Got Vegetable? program increases vegetable knowledge, attitudes, critical thinking about vegetable offerings and intakes of dark green leafy, yellow/orange, and cruciferous vegetables, tomatoes, and physical activity among Kentuckians as shown in studies in Iowa.

Making Healthy Lifestyle Choices: A family and community-based obesity prevention program 2013

Program Justification This program was developed to address the high obesity rate (66.2%) and physical inactivity among Kentuckians (54.2%) (CDC, 2011). Poor dietary choices, physical inactivity, and high consumption of sugar sweetened beverages are linked to major causes of illness and death. In 2013, Kentuckians will spend $2.4 billion in obesity related medical costs (Thorpe, 2009). Only 24.4% of Kentuckians consume the recommended servings of fruit and 29.4% the recommended vegetables servings (CDC, MMWR, 2010). Frequent consumption of soft drinks is linked to weight gain, type 2 diabetes, heart disease, osteoporosis, tooth decay, and kidney stones. Each sugary drink consumed per day increases the likelihood of a child becoming obese by 60% (Ludwig, Peterson, Gortmaker, 2001).

Program Description This program is based on the United States Department of Agriculture’s 2010 Dietary Guidelines for Americans. It is the only Cooperative Extension Obesity Prevention program that uses national guidelines and recommendations to increase healthy eating habits and physical activity among Kentuckians. This evidence-based program provides Kentuckians with specific action and hands-on tools to make small, specific behavior changes that would help them consume fewer calories, make informed food choices, and become physically active. This program will help Kentuckians attain and maintain a healthy weight, reduce risk of chronic disease, and promote overall health.

Program Format Unit 1 -- MyPlate the Kentucky Way: Tools for Building a Healthy Plate. This unit teaches Kentuckians how to fill half of their plate with fruit and vegetables. (Includes Facilitator guide, Activities, PowerPoint, PowerPoint script, Evaluation, Media releases, and Marketing materials). Unit 2 -- Focus on nutrient dense foods and beverages. Individuals learn how to make nutrient dense food choices, and the benefits of such choices. Unit 3 -- Making Healthy Beverage Choices: What You Don't Know Can Hurt You. Taking Ownership of Your Diabetes 2010

Program Justification The number of individuals with diabetes in Kentucky has more than tripled in the past 15 years; from 116,000 to 370,000 (Kentucky Cabinet for Health and Family Services, 2011). More than 127,200 individuals are undiagnosed, making a total of 497,200 individuals living with diabetes (Kentucky Diabetes Network, 2008). The problem is further exacerbated due to a high incidence of obesity and physical inactivity, which are major risk factors for the development of type 2 diabetes. An estimated 40 percent of all Kentuckians ages 40 to 74 are currently overweight or obese and can be classified as having pre-diabetes, and are likely to develop diabetes in the future (Kentucky Cabinet for Health and Family Services). Present statistics show Kentucky to have the 6th highest rate of diabetes in the nation (Trust for America’s Health, 2011). Diabetes complications are responsible for 19% (114,977) of all hospitalizations with a total cost of 2,043,000,000 in medical expenses and loss of productivity (Kentucky Cabinet for Health and Family Services, 2011). The cost and the impact of diabetes on the population can be reduced through modification of lifestyle risks, early diagnosis, appropriate health care, and self-management education (Kentucky Department of Public Health, 2005 p. 7). Recent statistics show Kentuckians falling short in the area of self-management education (Kentucky Cabinet for Health and Family Services, 2011).

Program Description This is a four-series, 12-lesson curriculum. The curriculum is based on the National Diabetes Education publication (NDEP) 4 Steps to Control Your Diabetes. For Life. The Stages of Change Model (Prochaska & DiClemente, 1983) and a model of critical thinking developed by the author (Adams, Hausafus, Hendrich, 2010) were used as the theoretical bases. Research shows that programs developed with a sound theoretical foundation are more effective and vital if Cooperative Extension is to be effective in the future.

Product Developed

Marketing Your Diabetes Program – contains letters to sponsor, PSA, and thank you letters (4 pages). Diabetes Video -- contains: (a) diabetes stories, challenges, and victories of individuals living with diabetes in Kentucky (b) health tips – Endocrinologist talk about diabetes issues and topics (c) physical activity – Contains group walk, body recall, chair exercises, stretching, and walking in place.

Diabetes Placemat – Based on The Diabetes Plate. Contains healthy eating tips and guidelines.

Health and Quality of Life Impact of Taking Ownership of Your Diabetes Program During the 18 months of implementation 3,085 individuals participated in the Taking Ownership of Your Diabetes Program. Of these individuals, 1,532 were seen by their health professional as a means of managing their diabetes, 1,218 had their A1C checked, and 1,033 took part in physical activity for 30 or more minutes on five or more days of the week. In addition, 1,206 checked their blood glucose 1 or more times a day, 999

utilized a meal plan, and over 770 set and accomplished goals for themselves. Over 591 individuals mentioned that they moved into Action and Maintenance stages of diabetes management. Moreover, based on similar studies by colleagues in other states, this program has the potential to save 7,897,600 in healthcare costs.

Partnerships 1. The University of Kentucky Center for Excellence in Rural Health (Homeplace). In

2012, Diabetes self-management education was delivered through a partnership with Family and Consumer Sciences (FCS) agents in 10 Easten Kentucky counties (Floyd, Pike, Wolfe, Clay, Knox, Johnson, Greenup, Lawrence, Morgan, Elliot, and Carter) and HomePlace nurses. The FCS Agents provided nutrition sessions from the University of Kentucky’s Cooperative Extension diabetes curriculum Taking Ownership of Your Diabetes. In 2013, a more extensive partnership was developed with FCS Agents and Homeplace in 35 to 39 Appalachian counties where 10.6%-16.3% of the population has been diagnosed with diabetes. This collaboration would allow both agencies to reach clients who rarely have an opportunity to receive self-management education.

2. Agents are part of over 30 Diabetes Coalition groups throughout the state. These

coalitions have helped to increase awareness of diabetes, maximized the use of

resources, and helped hard-to-reach families. For example, the FCS Agent in the

Meade/Breck Diabetes Coalition reported, “We meet the 1st Tuesday of each month at 2pm

here at the Extension office. This past program year we held diabetes awareness fair with cooking

demos and in March we will be hosting a 4 week session using Taking Ownership of your

Diabetes Curriculum. Our Coalition also developed a calendar with recipes and self-care tips for

individuals with diabetes, which we distribute to the general public during the county fair.”

Many other coalitions form diabetes support groups and diabetes advisory councils.

Coalition groups have secured over $170,000 in grant funding for their counties.

3. The YMCA. Discussion in progress as to how the University of Kentucky Cooperative

Extension could partner with the YMCA to increase awareness about diabetes and

diabetes prevention.

4. Ohio University. School of Nursing Project. Requested partnership where the Taking Ownership of Your Diabetes Curriculum would be used in 4 counties in Appalachia Kentucky, and in seven other counties in Mississippi, Ohio, and Virginia as part of a Vulnerable Population CDC funded grant.

5. FCS agent in Wolfe County partners with a local podiatrist and a food bank. Other

partnerships include a large number of physicians, nurses, dietitians, certified diabetes educators, and community members.

Fun Family Mealtimes 2008 - 2009

http://www.keha.org/ Program Justification Research shows that frequent family meals are associated with better nutritional intake. Children who take part in regular family mealtimes are less likely to be overweight, perform better at school, are at decreased risk for unhealthy weight control practices and substance abuse (Rollins, Francis & BeLue, 2007). Approximately 33% of Kentucky children are overweight or obese. Thirty-six percent of children in Kentucky consume at least one sugar-sweetened beverage a day. Only 17% of high school students consume fruits and vegetables five or more times per day. Obese children are at greater risk for developing chronic disease and other co-morbidities (CDC, 2011). The cost of hospitalizations related to childhood obesity is close to $237,600 million (Thorpe, 2009). This program stands to improve the health and quality of life for Kentucky’s families and their children.

Program Description Many families do not take advantage of mealtimes because they are too busy working or running the kids around to their many events. The program Fun Family Mealtimes provides opportunities for families to talk laugh and have fun together. It helps families find ways to make family mealtime possible by showing them how to cook once and eat twice, cook meals with five or less ingredients, use make ahead meals, and plan meals in advance. Ideas and fun activities for mealtime conversations are provided.

Ovarian Cancer Awareness Tea (O-CAT) Program 2010

http://www.keha.org/ Program Justification When ovarian cancer is found early and is localized, about 94 percent of patients live longer than 5 years after diagnosis. The University of Kentucky’s Ovarian Cancer Research Screening Program is the only program of its kind in the United States where free cancer screenings are provided for women over the age of 50. Approximately 15,000 women are screened each year. The program has identified over 400 ovarian tumors and 61 cases of ovarian cancer, the majority of which were early stage (University of Kentucky Cancer Screening Program, 2013). This Ovarian cancer Awareness Tea Program was developed specifically for use with Kentucky Extension Homemakers Association (KEHA). The majority of the 5000 KEHA members are between the ages of 65-85 and are eligible for free screenings at the University of Kentucky Centers. The FCS agent in Harrison County approached me to create this program.

Program Description The goals of the Ovarian Cancer Awareness Tea program are to: (i) raise funds to support the University of Kentucky Ovarian Cancer Research and Screening Program, (ii) increase awareness of ovarian cancer in Kentucky and, (iii) increase the rate of screenings among women who have not been screened for ovarian cancer.

Impact: This Ovarian cancer Awareness Tea Program I developed provided FCS agents with background information on the program, educational pieces for clientele, and evaluation and reporting tools. As a result of utilizing the resources I developed, Counties contributions a total $151,010.73 to Kentucky Ovarian Cancer Research and Screening Program (In 2010, a total of $51,645.14 was collected. In 2011, a total of $48,857.09 was collected. In 2012, a total of $50,508.50 was collected).

Program Justification Knowing how to read food labels is important for all consumers but especially for those who

have a chronic disease condition such as diabetes, high blood pressure, or high cholesterol (U.S.

Department of Health and Human Services, 2013).

Program Description Research shows that for consumers to benefit from label information, it is necessary to make

food labels, especially serving size and percent daily values information, easy to understand.

The Food Label F-U-Ndamental program provides information in a simple format and uses

games and activities to make food labels easier for Kentuckians to understand.
