Community Wellness Report 2015

Carrot River/Arborfield/Zenon Park

Cumulative Report

2015 Community Wellness Report

This report is intended to be used by community-based Primary Health

Care Provider Teams, Clinic Teams, working groups, KTHR staff and

community leaders to inform and to develop strategies to address

unmet needs and emerging issues in the communities of Carrot River,

Arborfield and Zenon Park.

Health and wellness decisions are not simply a matter of personal

choice. What makes us and keeps us healthy is complex and

multifaceted. Determinants of health such as income, education,

environment, culture, coping skills, housing and lifestyle options all

influence a person’s ability to have access and to make the healthiest

choices.

The root cause of many illnesses in our communities stems from

poverty. For those with low incomes, poverty means more than just

living in a poor home, or having no access to affordable transportation.

There are a variety of detrimental impacts stemming from being

poor. One of the most devastating is the impact on health and well-

being that increasingly professionals across sectors are beginning to

identify. Looking at health more holistically has focused attention onto

the social determinants of health.

Many of our KTHR staff collaborate and partner with agencies and

other sectors to develop illness prevention and health promotion

strategies and work to improve the social determinants of health. For

example: Partners to End Poverty; Food For All Coalition; 0-3 Early

Childhood Task Force; Northeast Housing Research Project; Northeast

Regional Intersectoral Committee; Cost of Healthy Eating in

Saskatchewan; Tisdale Suicide Awareness Committee and Population

Health Working Groups: Eat Well, Play Well and Tobacco Reduction.

In Primary Health Care we are committed to a health care system that

puts patients first by providing Better Health, Better Care Better Values,

and Better Teams for our residents. We have clinic teams that work on

meeting the needs of those with illnesses or urgent care needs.

Ensuring all our residents have access to a main care provider has

become a regional priority. This helps to ensure patients have

continuity of care. Research tells us that people who have a family

doctor or nurse practitioner have better health outcomes. Better

health outcomes mean better quality of life.

Carrot River and Arborfield Clinic team strategies

include:

Impacting access to continuous and comprehensive care

Educating the public on the importance of attaching to a family

Dr/NP

Improving timely access to a family Dr/NP

Improving patient experience

Establishing and managing patient panels or roster. (A panel is a

list of patients that have an established relationship with a

Dr/NP. Once Dr/NP has a list of his/her patients, they can

better manage their care. For example, with screenings

(cancer, blood pressure, diabetes, etc.)

Promote Live Well with Chronic Conditions - a six week self-

management program for people who have an ongoing or

chronic medical condition. The program helps people with

chronic conditions build skills for managing their health, so that

they can maintain active and fulfilling lives. The program offers

information and teaches practical skills to better manage

symptoms and become an active partner in their care.

Participants gain confidence and motivation. In 2013/2014 and

2014/2015 the goal was to offer 1 session per year for the

communities of Carrot River, Arborfield and Zenon Park.

However due to having no local facilitator no classes were

offered. In 2015/2016 one class was offered with 5 participants

attending.

We also have Primary Health Care Provider Teams throughout the

region which brings health professionals and community

representatives together to work on unmet wellness needs in the

communities. Each team has conducted community wellness

assessments every two years since 2007 which allows the community

to tell us what the health and wellness concerns are. The 2015 survey

results will be discussed by the team and strategies to address the

various needs will be developed. Those needs that are beyond the

control of the team will be directed to the appropriate agency or group.

Our community representatives are our voice from the communities

and play an integral role when it comes to developing strategies to

address needs such as: cooking classes; reducing screen time;

promoting and supporting physical activities such as: Fun Fitness

Challenge and Trail Trek; posting health and team related information

on community bulletin boards and P.A.R.T.Y. Program (Prevent Alcohol

and Risk-Related Trauma in Youth).

4. If you answered "no", who do you think is responsible?

Going too much for people to be responsible on their own. We don't have a good system anymore

It is hard to get in to see the doctor if you have an urgent matter we

could use more doctor time and more lab days if possible lab days

should match doctor days. There is some confusions for appointments. I

have booked an appointment in CR and gone there and found out the

appointment was in Arborfield. My husband got a bad rash and by the

time he could get into the doctor here the rash was gone and then no

cause could be found as they don't know what the rash looked like

Wasn't sure if it was necessary

Tried 1st to go to health provider

I have seen a physician at a PA minor emergency clinic

High extreme blood sugar

Hard to get on locally when needed

Clinic wasn't open on Sunday

6 year old grandson had a bad headache, couldn't get anyone. Made an appointment in morning, had to go to Nipawin at 1am as couldn't wait. Was told to go home and wait for an appointment Went to Carrot River health center for a bleeding finger. Needed at least 1 stitch but no one in town does stitches

X-ray needed, not available in Arborfield

Wait times are too long in Nipawin and didn't have anything that urgent

I'm satisfied

Was told at Nipawin hospital that there was a 4 to 5 hr wait in emergency so I left

The clinic was closed and we could not put the situation off until Monday morning, it was for my son (13 yrs)

Comments:

I work 8am to 5pm away from my community

Availability

I work in a neighboring community so clinic hours not always easy to access but clinic is very accommodating

Appointments not available

Some aren't offered here

Lives 13 miles from CR-unable to access home care services daily for elderly wife

Loss of wages for my driver

Friday appointments not available

NP needs to be in Arborfield

Lack of hours available

Long waits to see doctor

Shift work

No services

The programs have been reduced. NP hours reduced, no longer have x-ray services in the community

I work away and often have difficulties getting an appointment when I am home

Health providers acquired keep leaving

There are not many services to access at times. I would drive to another center and see someone before getting in here. When I moved here I was told that no doctors were accepting patients in my surrounding area and I refuse to see different providers all the time so I kept mine in Prince Albert, seeing how I use her once a year and as needed.

Distance to travel

Not available - wait times - we go to Saskatoon for mental health for young daughter

No services available in my community

There are no health care services in my community

Long wait times to see your family doctor

Difficult when you work full time

We all work and attend school. Having to take so much time off for appointments, follow ups and tests (when told nothing appears to be wrong but we will test just because) adds up and becomes frustrating to staff, supervisors and school staff.

Lack of health care providers

811 is a bit of a joke in my opinion. In the past every call to them has resulted in a response of “you really should go to see a doctor". Not much help in my opinion! Just a waste of time and money.

At the present time our health issues are being met.

More involvement (presentations) by primary care team or other health professionals to better address these matters and increase awareness with the schools and youth in the community

More funding for senior care - there is not enough staff/time for staff at the Health Center (Nursing Home) to give the individual care & attention that the residents deserve.

I have no issues as of right now

Choice of what doctor I want to see; not a specific one because I belong to a certain area.

Quicker access when needed and more availability for mental health. Saskatoon Mental health rural program is AMAZING need more like them

I'm getting forgetful over time. A checklist of when/how often to get pap smear/physical/bone density/blood work/hormone checks/mammogram. etc. What don't I know.

None. That’s why we seek out professional help

Specialist readily available for Telehealth. Follow ups with Family doctor instead of another trip to the city.

As mentioned previously, we have to wait too long to see our doctor. We can get in to see an alternate but a person wants the reliability & comfort of seeing the same physician every time!

Lobbying the government: - healthy foods are expensive - natural or alternative therapies (that are often preventative) are expensive and not covered by insurance plans 2. Host more preventative workshops (after hours) including natural/alternative therapies in which people can learn preventative lifestyle choices 3. Promote and encourage healthy workplaces through education of employers and employees

I feel as though our Dr.'s should listen to their patients better and be a little more thorough when diagnosing. I have had issues/heard of others having issues with this. They seem to have the idea that because they are the Dr. their patients concerns or opinions are not valid. I was asked who the Dr is when suggesting a possible diagnosis for example

Programming for persons at risk. Offer people solutions to fix themselves before medicating

More caregivers and doctors x 2

With the rise in suicides, family violence, divorce. There needs to be a lot more services from Mental health and alcohol and drug abuse. there is not enough support for young families and newly separated/divorces couples

Programs going through the schools tend to be received better and attendance is a given. People tend to not go in after hours and based on their own choice to go

Stop cutting back in medical field

Access to emergency treatments

Family orientated physical activities

None x 2

Better access to providers

Education

Accessibility to screening programs, CT scans, etc.

Continue as is

Foot clinic and physiotherapy x 2

Access to professionals trained specifically in the field related to my particular issue: mental health professional, physical therapist

Chiropractor and massage therapy

Need some place to go

Increase first responders hired, equipped vehicle to respond to emergencies

More timely wait to see the nurse practitioner

Same as above

Same as above but with community members and all ages

Nothing at this time

More Dr. office hours in Arborfield. Better emergency services. Better use of the health center

Not sure how you can keep families more active. Alcohol use in very prevalent in our community. There used to be a time we just went somewhere for coffee/tea now there is always alcohol. I don't know how to change that culture.

Can't get into Carrot River Health at all on short notice if needed

The ambulance companies need to train their employees as to how to read a map, have maps of the surrounding towns they generally travel to, have RM maps with farm yard sites highlighted. and these maps need to be studied and understood. There have been too many instances of ambulances getting lost on their way. Rural people speak in terms of miles whereas younger people speak in kilometers. Are they able to convert the information?

Some youth activities

It would be helpful to have Arborfield clinic staffed with a Nurse Practitioner more often. They are very capable; we just need to have access to them more than once a week. Would appreciate being notified of public presentations 3

I feel that when a person is not feeling good it shouldn't matter which community in the district that you come from. I was ill and phoned Tisdale clinic. The lady in charge?? Informed me people from ZP couldn’t go to the Tisdale office.

a walk-in clinic in city doesn't discriminate who you can see. Why does Tisdale? Different people have told me that this has happened to them also. Even old people have talked about how rude the Tisdale clinic people are

Produce @ our local store is never as fresh as out of town. New cooling systems???

When people from Zenon Park cannot get into see Louise in Arborfield and call Tisdale, they are told that Tisdale does not take patients from

here. We are still between 2 RMs and in the middle of Nipawin and Tisdale; I really don't think that this is acceptable.

Older people don't use social media as a resource

Because I work within the Region I get most of my information at work.

public workshops/information nights

I feel the caregivers we have are all reliable well trained. I believe they are overwhelmed. We could stand more care givers in our region and at least one more doctor. Stop trying to cut Arborfield out and trying to move everything to Carrot River

22. If no, please explain:

Prefer larger community. it is a nice place just not right for me

In some ways

Safe, friendly, supportive

Town dying

Very supportive and non-judgmental, reasonable cost to live here. Kids

can be very involved and do well in school and in every sport

23. If no, please explain:

Lack of health services

When you need help it's not available

Not enough services - Mental health

Has all basic services unless health needs become too great

Access to medical professionals closer

Too expensive

Decreased services right in community, must drive for access to all health care

Neighbors and family very supportive. Care home in our town

It's quiet here

Lack of places to live if partner is in the home

I think so, but since I haven't reached the really old part of growing old, I'm not speaking from experience

not there yet

No specialists close, optometrists, etc.

Some issues take too long to resolve and must travel between 2 or 3 communities for health care

Not enough quality senior housing, far to travel for health related

Limited services

Lack of essential health care services i.e.: home care services, dentist, limited lab and x-ray

No access to pharmacy/clinics etc. Poor roads and sidewalks

not enough medical access

No hospital

Too far to travel to access Dr's and specialists

As long as you are mobile and able to drive

24. If no, please explain:

Crime rising

Amazing wonderful place to be

Unless you have an emergency

25. If no, please explain:

Mainly walk/gas too expensive

There are not a lot of sidewalks which can pose a challenge for people who no longer drive. Seniors housing is a considerable distance from Dr. office, grocery store, post office and pharmacy

We live on a farm x 2

Non-existing sidewalks

Depends where you live

Don't walk too much

No walking paths, winter icy streets

I'm in rural setting

Fairly easy, although some of the sidewalks could cause problems for those who have a walking disability

Could use some improvement - more sidewalks

If we had sidewalks

If you live in town, if you live in RM is very dusty right now and farmers
