LEGION and GENESIS II achieve equivalent articulation but do so via different surgical approaches to femoral external rotation.

GENESIS II offers built-in external rotation by having asymmetric posterior condyles whereas LEGION offers surgeon-directed external rotation with symmetric posterior condyles. The differing techniques produce identical implant tibio-femoral kinematics and wear performance.2

When GENESIS II was introduced in 1995, the built-in external rotation was a unique concept and design. It simplified the procedure and surgical technique for primary total knees.

Over time, the need to simplify more complex primary and revision procedures has grown. To address this the implant was modified so that the surgeon could set the external rotation based on patient anatomy, resulting in symmetric posterior condyles. At the same time, the instruments were simplified and a seamless transition to a revision procedure was created leading to the evolution of GENESIS II into the LEGION Total Knee System.

Outcome for Osteoarthritis - Patient Characteristics

Primary total knee replacement has the lowest rate of revision compared to all other classes of primary knee replacement. At 13 years, the cumulative percent revision of primary total knee replacement undertaken for osteoarthritis is 6.8% (Table KT11 and Figure KT9).

Age and Gender Age is a major factor affecting the outcome of primary total knee replacement. The rate of revision increases with decreasing age, and this difference increases with time. After four years, those aged less than 55 years have over 4.5 times the rate of revision compared to those aged 75 years or older (Table KT12 and Figure KT10).

Males have a higher rate of revision compared to females (Table KT13 and Figure KT11).

Loosening/lysis is the most common reason for revision in both males and females. Males have a higher incidence of revision for surgeon reported infection than females, with a 13 year cumulative incidence of 1.5% and 0.8% respectively (Figure KT12).

Age related differences in outcome are evident within both males and females (Table KT13 and Figures KT13 and KT14).

A 13 year cumulative percent revision is the longest reported outcome for any primary total knee replacement in the Australian Registry. GENESIS II CR CoCr has the lowest cumulative percent revision of all primary total knees with cement fixation at 13 years, 4.6%.

Primary total knee replacement has the lowest rate of revision compared to all other classes of primary knee replacement. At 13 years, the cumulative percent revision of primary total knee replacement undertaken for osteoarthritis is 6.8% (Table KT11 and Figure KT9).

Age and Gender Age is a major factor affecting the outcome of primary total knee replacement. The rate of revision increases with decreasing age, and this difference increases with time. After four years, those aged less than 55 years have over 4.5 times the rate of revision compared to those aged 75 years or older (Table KT12 and Figure KT10).

Males have a higher rate of revision compared to females (Table KT13 and Figure KT11).

Loosening/lysis is the most common reason for revision in both males and females. Males have a higher incidence of revision for surgeon reported infection than females, with a 13 year cumulative incidence of 1.5% and 0.8% respectively (Figure KT12).

Age related differences in outcome are evident within both males and females (Table KT13 and Figures KT13 and KT14).

Background: New bearing surfaces for total knee replacement have been described in an attempt to reducepolyethylenewear and secondary osteolysis and improve longevity of implants. Oxidized zirconium is a newma-terial that combines the strength of a metal with thewear properties of a ceramic. However, there are no reportsas to who documents the long term results. We report a 10 year follow-up note of a selected series of TKAs withan oxidized zirconium femoral component.Methods:We prospectively follow 98 TKAs performed in 94 patients with an oxidized zirconium femoral compo-nent. Five patients (5 knees) had died and 6 (6 knees) were lost to follow-up at a minimum of 2 years (mean,6.3 years; range, 2–9 years) after the operation. For the remaining 83 patients (87 knees), the minimumfollow-up was 10 years (mean, 11.3 years; range, 10.0–12.6 years). In 51 cases (58.6%), a cruciate-retaining im-plant with a deep-dished, more conforming PE was used, and in 36 cases (41.4%), a posterior-stabilized designwas used. The patellae were resurfaced in 32 cases (36.7%) and in 55 cases (63.3%) were left unresurfaced.Results: Survivorship was 97.8% at 10 years postoperatively. Two knees were revised for aseptic loosening of thefemoral component. No major complication was observed clinically or radiologically. Mean Knee Society scoreimproved from 36 to 84 and functional score from 37 to 83.Conclusions: Oxidized zirconium femoral component in TKA performs well over the first 10 years following im-plantation, with excellent survival rates and good clinical and radiological outcomes.Level of evidence: Level IV, therapeutic study.

1. Introduction

Polyethylene wear and osteolysis remain as major problems in thelong term survival of total knee replacements. Research has addressedthese problems by improving implant design, increasing quality ofultra-high-molecular-weight PE and developing new bearing surfaces[1–3].

Oxidized zirconium (OxZr) was introduced as an alternative bearingfor femoral components in total knee arthroplasty (TKA) in an attemptto reduce PE wear and secondary osteolysis and improve the longevityof total joint arthroplasties [4]. Preliminary studies in vitro have indicatedan excellent mechanical property for this material with a high resistanceto roughening and a reduction of PE wear from 42% to 55% when com-pared to cobalt–chromium (CoCr) femoral component for total knee re-placement. Furthermore OxZr femoral component has no nickelparticles,making thismaterial safe in patientswithmetal sensitivity [5–9].

However, all these theoretical purposes should be confirmed by clin-ical investigation. It is well known that preclinical laboratory testing ofmaterials has limitations, sometimes leading to catastrophic failure,

and that proof for all new technologies must come from long term fol-low-up clinical trials.

We previously reported clinical studies of OxZr TKA with up to5 years of follow-up that have demonstrated no adverse consequencesattributed to OxZr bearing rates of complication and radiologic loosen-ing similar to those in other studies of primary TKAs that had a similarlength of follow-up [10]. However a longer follow-up analysis is neces-sary to establishwhether OxZr femoral implantsmaintain promising re-sults but data in literature are lacking.

The purpose of our study was to evaluate the 10 year results previ-ously reported for patients who had TKAs with OxZr femoral compo-nent. In particular, we sought to determine implant survivorship, rateof complications, radiographic osteolysis and implant loosening andclinical function as defined by the Knee Society score.

2. Patients and methods

Weprospectively follow a selected series of 98 TKAs performed in 94patients at the authors' institution with an OxZr femoral component(Genesis II, OxiniumTM; Smith & Nephew, Memphis, TN).

The inclusion criteria for OxZr were age younger than 65 years, highactivity levels, and overall health status as judged by the surgeon, but

This has shown to reduce lateral release rates to ~3%12 compared to ~14%13 for competitive systems. The patella groove has been distally extended to provide full patellofemoral contact throughout the ROM.
