Knowing our role:Midwives: specialists of normal pregnancy, birth and post-partum. Respectful of natural processes

    We are pathologists

strength of prevention... must act on pulse

Nei Jing (chapter 2): The sages did not treat those already ill, but treated those not yet ill, they did not put in order what was already in disorder, but put in order what was not yet in disorder... now, when drugs are employed for therapy only after a disease has become fully developed, when restoring order is initiated only after disorder has fully developed, this is as if a well were dug when one is thirsty, and as if weapons were cast when the fight is on. Would this not be too late, too?

but we must restrain ourselves... less CAN be more... but not always.

For us, starts at: Yin and Yang unite = conception. See first month description.

when we count the months, we count from conception, not from the first day of the last menstrual period.

Both mother and babys gestation start at Jueyin, but are on different cycles

Mother is on her own, slower cycle from Shaoyin conception to Taiyang separation at birth

Baby undergoes a whole cycle from Shaoyin conception all the way around the circle back to Shaoyin.

not just rolling!JGYL 20-1: The pulse of a woman is moderate, but he pulse at the cubit is slender and weak. She is also thirsty and has no appetite. No fever or chill is observed. This indicates a pregnancy.

Mai Jing book 9, chap. 1: It is said in the classic that beating yin and distinct yang tells [the mother] carries a baby. This shows harmonious blood and qi and that yang has donated and yin is transforming. On examination, if the hand shaoyin pulse stirs in a big way, pregnancy is determined. The shaoyin is the pulse of the heart. the heart governs the blood vessels. In addition, the kidneys are named the uterine gate and infants door. The pulse in the chi is that of the kidneys. If the pulse in the chi does not expire under pressure, this should indicate a pregnancy. If the pulse remains quite the same in the superficial and deep levels in the three positions, not yielding to pressure, there is pregnancy.

    Pulses in pregnancy

Mai Jing book 9, chap.1: In the initial stage of pregnancy, the pulse is slightly small in the cun, beating 5 times for one respiration. In the third month, the pulse is rapid in the chi. If the pulse is slippery and racing but scattered under heavy pressure of the fingers, the fetus in in the third month. If the pulse is not scattered under heavy pressure of the fingers and it is racing but not slippery, the fetus is in its fifth month. When a woman is in the fourth month of pregnancy, the method of determining the sex is that a racing pulse on the left hand tells a male, while a racing pulse on the right tells a female. If the pulse is racing on both hands, this tells that two babies will be born.

Normally in this situation we would use Zhen Wu Tang or a similar formula... but here we cant because of the pregnancy (although Xu Zhi Cai recommends this type of method)

we can use Ling Gui Zhu Gan Tang and/or the Xuan Fu Hua method... but first a bit of background on this...

  • Pregnancy, Birth & Post-partum with Classical Chinese Medicine

    www.prodseminars,com 85

Zheng Qinan (translation Yaron Seidman): True Yang appear under the following names: minister fire, gate of life fire, dragons thunder fire, rootless fire, Yin fire and empty fire. When it is diseased it comes under the following descriptions: Yuan (original) Qi does not receive, original Yang scatters outwards, true fire ascends upwards, Kidney Qi does not receive, Qi does not return to its origin, solitary Yang surface upwards, empty fire rushes upwards etc all sorts of names all describing the one Yang in the middle of Kan trigram. The origin of this one Yang has transformed from the pre-heaven of Qian metal. Therefore it has the name dragon. One Yang lands in the midst of the two Yin and transform into water. It establishes the extreme range of water (Yang is then the root of water).

  • Pregnancy, Birth & Post-partum with Classical Chinese Medicine

    www.prodseminars,com 86

Water quality is to flow downward, and this is Kans fixed position in the post-heaven arrangement. It is an unchangeable principle. One should know that the dragon at this time is the new born dragon (dragon means the one Yang in the middle of Kan). It is unable to soar to excite the clouds and scatter the rain. It can only dive into the abyss where water becomes its residence. With waters nature it succeeds in becoming peaceful in this lower position and it bows its head underneath. With diseases where, for example, empty fire rushes upwards, it clearly binds with the surging water (water is Yin). If the water surges one fraction, the dragon surges as well one fraction (dragon is fire). If the water is higher one foot then the dragon is higher one foot as well. The dragon is roaming because the water is surging. It is not that the dragon fails to dive and acts contrary to its normal state.

  • Pregnancy, Birth & Post-partum with Classical Chinese Medicine

    www.prodseminars,com 87

Therefore the classic states: When the Yin surges the Yang must decline. One can realize that by the application of medicinals one must support the Yang and restrain the Yin. And so as soon as the town doctor see an empty fire rushes upwards, he doesnt even inspect the vital why and wherefore. He opens his mouth and say nourish the Yin and descend fire. He self proclaims to have grasped the problem and not even think of the principle where when Yin surges (Yin surges means that kidney water is exuberant) Yang is deficient (Yang deficient means that the emperor fire is weak). And so he doesnt support the Yang and to the contrary nourish the Yin even more. It is truly not less then adding frost to a snowfield. Isnt this the common hand of medicine? Whenever I see a disease such as empty fire rushes upwards where the patient loves to drink warm liquids and cant stomach anything cold, it is self evident that nourishing the Yin is a mistake.

  • Pregnancy, Birth & Post-partum with Classical Chinese Medicine

    www.prodseminars,com 88

Also when I say that cinnamon and aconite cause the fire to return to its origin, no one knows the meaning of it. They dont know that cinnamon, aconite and dry ginger are a pure ball of raging fire. When fire is vigorous then Yin disappears on its own. It is like the strong sun that scatters the clouds until they disappear. Moreover the power of cinnamon and aconite can supplement the Yang in Kan and Li.Their character is extremely upright and unyielding. They can erase completely the Yin Qi that overstepped its authority. When Yin Qi is completely erased[7] then the great space opens up a wide corridor. Above and below establish peace naturally and without unilateral flourishing. How can this not be seen as truly leading fire back into its origin! All through history commentators didnt search out the reality under the surface where one Yang is being submerged in the water. This caused later students ignorance with having nothing to depend on. Countless people died under nourishing Yin and descending fire. Truly it is a corrupt practice through the ages and a great regret to the school of medicine.

What conditions are we looking for for an easy, timely birth?

If a woman has cycled on time with nature and the pulse is harmonious, the birth should be on time.

Labor belongs to Shaoyang (alternating contractions). Before labor and during, the pulse should be wiry... its a sign that Jueyin uterus is being affected by its center Ministerial Fire.

DO NOT SUPPRESS the wiriness of the pulse

Again our archenemy is the weak pulse ( post-dates, hemorrhage...)... if we are in a Yin conformation there is not enough yang to manifest Shaoyang alternance of contractions.

Wood: strong, steady, gentle growth. If there is strong emotional distress, maybe relax and slow down Wood so the mother doesnt waste her Yang by discharging emotions... Otherwise, leave it be and STRENGTHEN THE ROOT.

In all cases, it is not only during delivery that women must worry. When they arrive at the postpartum stage, they must exercise particular caution. This is where the greatest threat to their lives is found. [...] At the time of violation [of prohibitions], [the disease] might be as tiny as autumn down. But the contracted illness will be larger than Mt. Song or Mt. Dai.

Why is this so? Post-partum illnesses are more difficult to treat than others. After women have completed a delivery, their 5 zang suffer from vacuity emaciation. You may only use supporting and supplementing and must not use shifting and draining.

[...] if a violation has occurred, it is inevitable that her body will become rigid and arched backwards, resembling opisthotonos. this condition is called childbed wind.

    JGYL 21-1:

Question: Convulsive disease, syncope and constipation are the 3 principal postpartum diseases. Can the Master explain them?

Master: After delivery, a woman will suffer blood deficiency. She will perspire a lot and is therefore easily affected by pathogenic wind. Convulsive disease is a common ailment. Loss of blood and perspiration will allow in the pathogenic cold, resulting in syncope. Loss of body fluid will bring dryness, eventually resulting in constipation. Symptoms and signs of syncope in women after delivery include weak pulse, nausea and anorexia, constipation, sweat appearing only on the forehead. Pathology of the syndrome results from deficiency in the blood. Before the syncope subsides, profuse perspiration will appear. When the blood is deficient, coldness in the extremities will occur and the solitary Yang will ascend, causing perspiration. This will bring the Yin and Yang into harmony. Syndrome of constipation, nausea and anorexia can be treated with Xiao Chai Hu Tang.
