AREA EVALUATION
On February 12, 2019, Jeffrey H. Mitchell of Triad Engineering, Inc. (Triad) and Brandon J.
Colunga of DMY Capital LLC (DMY) conducted an asbestos-containing material (ACM) and
lead-based paint (LBP) survey of the interior portions of the Operations Building proposed for
renovation, which were identified by Mr. Farrell Owens, the Facility Manager, and the roofs of
the Operations and Maintence Buildings located at the Percy D. Miller Water Treatment Plant
addressed as 32 Pence Lane Road, Middletown, Virginia 22645.
ACM SURVEY
The ACM survey was performed by Jeffrey H. Mitchell; Asbestos Inspector Licensed by the
Virginia Department of Professional and Occupational Regulation (Attachment 1).
Following standard inspection practices, suspect ACM (SACM) samples were collected to
confirm the presence or absence of asbestos. Samples were collected from locations of SACM
using the following procedures: utilizing appropriate tools, a sufficient amount was obtained for
PLM analysis; each SACM sample was placed individually in a sealed bag; and a unique sample
identification number was assigned to each sample material and location.
As follows are the specific SACMs identified and sampled:
Roofing: roofing system layers, sealant, and caulk;
Interior Wall: ceramic tile and grout;
Windows: interior & exterior caulk;
HVAC Systems: caulk; and
Miscellaneous: fiberglass pipe insulation mastic.
SACM descriptions and locations are listed on the chain of custody (COC) documentation
included in Attachment 2. Photographs are included as Attachment 5.

Following standard inspection practices, Triad obtained a sufficient number of samples of the
suspect materials per the federal and state regulations to confirm the presence or absence of
asbestos fibers in the SACMs. Samples of the SACMs were collected using the following
procedures: utilizing appropriate tools, a sufficient amount of a suspect material was obtained for
PLM analysis; each suspect sample was placed individually in a sealed bag; and a unique sample
identification number was assigned to each sample material and location.
The friability characteristics of the material were also evaluated by the inspector during the
sampling and procuring process. The friability of the material is described in two ways. A
designation of “friable” describes material meeting the following characteristics as described in
the U.S. Environmental Protection Agency (USEPA) National Emission Standard for Hazardous
Air Pollutant (NESHAP) for Asbestos 40 Code of Federal Regulation Section 61 Subpart M,
61.141: “that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure”.
A designation of “non-friable” describes a material meeting the following characteristics as
described in the USEPA NESHAP: “that, when dry, cannot be crumbled, pulverized, or reduced
to powder by hand pressure.” Please note however that material currently in a non-friable state
could become friable during demolition activities.
During the ACM Survey, a total of 15 sample sets totaling 45 SACM samples were collected and
submitted for asbestos analysis to SanAir Technologies Laboratory, an independent NVLAP
accredited and Virginia-licensed laboratory. Samples were analyzed using polarized light
microscopy (PLM) and dispersion staining analysis methods USEPA 600/R-93/116 and a total of
64 tests were performed due to multiple layers in some samples.
The following material contained asbestos at a level greater than one percent, which by
regulatory definition is an ACM.

friable ACM.

LBP SURVEY
A copy of the LBP survey report prepared by DMY is included as Attachment 4. In summary,
the following items were identified or should be assumed to have a LBP coated surface:
All cream porcelain tiles within the Filter Operating Floor and Rear Entrance Lobby,
All blue paint located on the stair systems and guard rails,
The orange paint located on the I-beams within the columns of the Filter Operating Floor,
and
The red access doors located on the exterior of the Operations Building.
LIMITATIONS
It was attempted to perform an ACM and LBP survey of the interior portions of the Operations
Building proposed for renovation, which were identified by Mr. Farrell Owens, the Facility
Manager, and the roofs of the Operations and Maintence Buildings located at the Percy D. Miller
Water Treatment Plant located at 32 Pence Lane Road, Middletown, Virginia 22645; however,
some areas may have been hidden or impracticable to access. These areas may include, but are
not necessarily limited to, the sub-grade; flooring located below underlayments; above or behind
ceilings, walls, and roofing; and pipe and utility chases.
Any SACMs or suspect LBP coatings encountered during the renovation that were not tested
should be assumed to be ACM or LBP unless tested to determine asbestos or lead content.

This report has been prepared for the exclusive use of the Client and persons or organizations to
which the Client wishes to make this report available. This report and the findings contained
herein shall not, in whole or in part, be used by or relied upon by any other party, without the
prior written consent of Triad.

The final report cannot be reproduced, except in full, without written authorization from SanAir. Fibers smaller than 5 microns cannot be seen with this method due to scope limitations. The accuracy of the results is dependent upon the client’s sampling procedure and information provided to the laboratory by the client. SanAir assumes no responsibility for the sampling procedure and will provide evaluation reports based solely on the sample and information provided by the client. This report may not be used by the client to claim product endorsement by NVLAP or any other agency of the U.S. government. Samples are held for a period of 60 days. For NY state samples, method EPA 600/M4-82-020 is performed. LEAD-BASED PAINT SURVEY REPORT
On February 12, 2019, DMY Capitol LLC (DMY) conducted a Limited-Scope Lead-Based Paint Survey in the filter basin area in the interior of the operations structure and the roofs of the operations and maintenance structures located at 32 Pence Lane Road in Middletown,Virginia at the request of Mr. Jeffrey H. Mitchell, representing Triad Engineering, Inc. The scope of work for the Survey included an lead-based paint (LBP) analysis of only the areas identified for a pending renovation.
DMY conducted the LBP survey in the structure utilizing an X-ray fluorescence (XRF) unit in general accordance with the procedures and directives described in the U.S. Department of Housing and Urban Development (HUD) guidance document Guidelines for the Evaluation and
Control of Lead-Based Paint Hazards, June 1997. Because removal or management of any identified LBP or surface coatings is considered to be incidental to the renovation project and not intended as a lead abatement activity or to reduce or eliminate a lead-based paint hazard, testing for LBPs was conducted in a survey fashion and representative testing combinations were selected accordingly throughout the structure.
The survey consisted of testing the lead concentrations of painted surfaces using a Innov-X 6000 Alpha Series XRF analyzer. Typical tested surfaces included, but were not necessarily limited to, walls, interior and exterior door components, interior and exterior window components, and casework. Several building components were greater than 1.0 milligrams of lead per square centimeter (mg/cm2) and other building components exhibited detectable concentrations of lead that were below the 1.0 mg/cm2 threshold. With respect to the naming convention of walls within the structure, the “A” Wall refers to the side of the structure facing the addressed street, which in this case is the northwestern face of the site structure. The “B”, “C”,
and “D” Walls are subsequently named in a clockwise fashion. Additionally, in the case of structures such as doors or windows, the individual structures are denoted with sequential numerical subscripts in a clockwise fashion along a given wall. The following table includes all of the analyzed surfaces and those surfaces identified as containing greater than 1.0 mg/cm2 are denoted with bold text.

RECOMMENDATIONS
Based on the results of Limited-Scope LBP Survey at the site, DMY recommends the following:
LBP Survey – The planned renovations will not be designed as or intended to constitutea lead abatement activity or to reduce or eliminate a lead-based paint hazard. However,care should be taken during any renovation activity that will involve removal ordisturbance of LBP or lead-containing surface coatings to prevent contamination of thework area or adjacent spaces by lead dust or debris. Furthermore, any renovation workduring which employees may be occupationally exposed to lead must be conducted inaccordance with OSHA lead-safe work practices as set forth in 40 CFR 1926.62. Thisincludes any disturbance of LBP or any lead-containing surface coating.
The following items were identified or should be assumed to have a LBP coated surface: o All cream porcelain tiles within the Filter Operating Floor and Rear Entrance Lobby,o All blue paint located on the stair systems and guard rails,o The orange paint located on the I-beams within the columns of the Filter Operating
Floor, ando The red access doors located on the exterior of the Operations Building.
DMY made a reasonable effort to locate and sample all potential LBP associated with the pending renovation during the field survey as indicated by the field contact, Mr. Farrell Owens, the Facility Manager. However, DMY stresses that the LBP Survey was inherently limited to those conditions readily accessible to the DMY team during the field surveys. Conditions inside enclosed walls, pipe chases, or other inaccessible areas inside the site structure or historical conditions not previously divulged to DMY may not have been identified or otherwise assessed during this survey. LIMITATIONS
It was attempted to perform a Limited-Scope LBP Survey of the structure located at 32 Pence Lane Road; however, some areas may have been hidden or impracticable to access. These areas may include, but are not necessarily limited to, the sub-grade; above or behind ceilings and walls; and pipe and utility chases.
The demolition and/or abatement contractor has the responsibility for determining actual quantities of all materials.
