เดก็หญิง อายุ 3 ปี 9 เดือน, จ. ระยอง อาการส าคัญ ไข้ ไอ หอบ 2 วันก่อนมาโรงพยาบาล ประวัตปัิจจุบัน 2 วันก่อน มีไข้ ไอ หอบ พ่นยาที่คลินิก ได้ยาลดไข้ ยาปฏิชีวนะ ยาแก้ไอ ยาขายหลอดลม อาการไม่ดีขึน้ เคยมีประวัตหิอบ ใช้เคร่ืองช่วยหายใจ 7 ครัง้ ระหว่างนีใ้ช้ยา seretide MDI, ventolin MDI และ avamys เคยมีประวัตหิายใจดงัเหมือนนอนกรน

ตรวจร่างกาย T 37.1 oC BP 106/65 mmHg P 120/min RR 46/min not pale, no cyanosis HEENT : pharynx, tonsils not injected tonsils 3+ Heart : normal S1, S2, no murmur Lungs : biphasic stridor, rhonchi both lungs, suprasternal and intercostal retractions Extremities : no digital clubbing Abdomen : no hepatomegaly

OPERATION - esophagoscopy before operation : intraluminal mass of esophagus, suspected esophageal cyst - right thoracotomy and esophageal cyst identified at upper esophagus, thoracic duct identified before beginning esophageal dissection and cyst removed from esophagus - repair esophagus PATHOLOGICAL REPORT - esophageal duplication cyst
